Indications for Stoma Formation and Timing of Stoma Closure in Neonates: A Single-Center Retrospective Experience

Objective: To evaluate the indications for intestinal stoma formation and the timing of stoma closure in neonates treated at our institution.
Methods: A retrospective review was performed of 72 neonates who underwent intestinal stoma formation between January 2020 and June 2026. Demographic characteristics, primary diagnoses, stoma type, associated anomalies, rectal biopsy findings, dates of stoma formation and closure, and postoperative complications were analyzed. Surgical complications were graded according to the Clavien–Madadi classification. Only patients with documented stoma closure dates were included in the analysis of stoma duration.
Results: Of the 72 patients, 52 (72.2%) underwent ileostomy, 16 (22.2%) colostomy, and 4 (5.5%) jejunostomy. Stoma configuration was available for 55 patients: 31 (56.3%) had a diverting stoma, 16 (29.1%) a loop stoma, and 8 (14.5%) a distal Hartmann stoma. The most common indication was meconium ileus (17, 23.6%), followed by necrotizing enterocolitis (NEC)/NEC-related perforation (16, 22.2%) and anorectal malformations (14, 19.4%). Other indications included intestinal perforation (12, 16.7%), intestinal atresia (5, 6.9%), volvulus (4, 5.6%), and Hirschsprung disease (4, 5.6%). Among the 37 patients with documented closure dates, the mean stoma duration was 199.8 days and varied according to the underlying diagnosis. Rectal biopsy was performed before stoma closure in 28 patients; ganglion cells were identified in 27, while one specimen was inadequate. Cardiovascular anomalies were the most common associated anomalies (29 patients), followed by genetic (4) and neurological (3) anomalies. Thirty-two patients had no associated anomalies. According to the Clavien–Madadi classification, complications were graded as Stage 1b (n=6), Stage 2 (n=2), Stage 3b (n=16), and Stage 5 (n=1). Reoperation was required for ileus in 8 patients and anastomotic leakage in 4, while 4 patients underwent stoma revision.
Conclusions: Meconium ileus and NEC were the leading indications for neonatal intestinal stoma formation. The timing of stoma closure and the need for pre-closure rectal biopsy should be individualized. The Clavien–Madadi classification provides an objective method for evaluating postoperative morbidity.
Yenidoğanlar Döneminde Stoma Oluşturma Endikasyonları ve Kapatma Zamanlaması: Tek Merkezli Retrospektif Bir Deneyim

Amaç: Bu çalışmada, merkezimizde yenidoğan döneminde oluşturulan barsak stomalarının açılma endikasyonları ile stoma kapatılma zamanlamasının değerlendirilmesi amaçlandı.
Yöntem: Ocak 2020–Haziran 2026 tarihleri arasında merkezimizde yenidoğan döneminde stoma oluşturulan 72 hasta retrospektif olarak incelendi. Hastaların demografik özellikleri, primer tanıları, stoma tipi, eşlik eden anomaliler, rektal biyopsi durumu, stoma oluşturulma ve kapatılma tarihleri ile postoperatif komplikasyonları değerlendirildi. Postoperatif cerrahi komplikasyonlar Clavien-Madadi sınıflamasına göre evrelendirildi. Stoma kapatılma zamanı analizine yalnızca kapatılma tarihi mevcut olan hastalar dahil edildi ve analizler mevcut kayıtlar üzerinden gerçekleştirildi.
Bulgular: Toplam 72 hastanın 52’sine (%72,2) ileostomi, 16’sına (%22,2) kolostomi ve 4’üne (%5,5) jejunostomi oluşturuldu. Stoma konfigürasyonu bilgisine ulaşılan 55 hastanın 31’inde (%56,3) diverjan, 16’sında (%29,1) loop ve 8’inde (%14,5) distal Hartmann stoma mevcuttu. En sık endikasyon 17 (%23,6) hasta ile mekonyum ileusu olup, bunu 16 (%22,2) hasta ile NEK/NEK perforasyonu ve 14 (%19,4) hasta ile anorektal malformasyonlar izledi. İntestinal perforasyonlar 12 (%16,7), intestinal atreziler 5 (%6,9), volvulus 4 (%5,6) ve Hirschsprung hastalığı 4 (%5,6) hastada görüldü. Stoma kapatılma tarihi bulunan 37 hastada ortalama açık kalma süresi 199,8 gün olarak hesaplandı. Stoma açık kalma süresi primer tanıya göre değişkenlik göstermekteydi. Stoma kapatılmadan önce 28 hastaya rektal biyopsi yapıldı. 27 hastada gangliyon pozitif, 1 hastada yetersiz numune olarak değerlendirildi. En sık eşlik eden anomaliler kardiyovasküler sistem anomalileri olup 29 hastada izlendi; 4 hastada genetik, 3 hastada nörolojik anomaliler saptandı. Hastaların 32’sinde ek anomali izlenmedi. Clavien- Madadi sınıflamasına göre hastaların 6’sı Evre 1b, 2’si Evre 2, 16’sı Evre 3b ve 1’i Evre 5 olarak saptandı. 8 hasta ileus, 4 hasta anastomoz kaçağı nedeniyle reopere edilirken, 4 hastaya stoma revizyonu yapıldı.
Sonuçlar: Mekonyum ileusu ve NEK en sık stoma nedenidir. Kapatma zamanlaması ve kapatma öncesi rektal biyopsi planı bireyselleştirilmeli, postoperatif morbidite yönetiminde ise Clavien-Madadi sınıflaması objektif bir standart olarak tercih edilmelidir.
