Duodenal perforation associated with iron chelator therapy in a pediatric patient with beta-thalassemia major: A case report and review of the literature

Objective: Oral iron chelation therapy with deferasirox is widely used to reduce iron overload in patients with transfusion-dependent beta-thalassemia major. Although rare, cases of duodenal perforation associated with deferasirox have been reported in the literature. This study aims to present a case of duodenal perforation, a life-threatening complication, and compare it with previously reported pediatric cases.

Case Presentation: A 4-year-old girl with transfusion-dependent beta-thalassemia major presented with a 2-day history of abdominal pain, fever, vomiting, and diarrhea. She had been receiving deferasirox therapy since the age of 3 years, and the dose had been increased three weeks before admission. Abdominal computed tomography revealed free intraperitoneal air, prompting emergency laparotomy. Intraoperatively, an approximately 3-cm perforation was identified in the first portion of the duodenum. Primary two-layer repair with omentoplasty was performed successfully. The patient had an uneventful postoperative course and was discharged on postoperative day 10.

Discussion and Conclusion: Duodenal perforation is a rare but potentially life-threatening complication in pediatric patients with beta-thalassemia major receiving deferasirox therapy. Previously reported pediatric cases were presented with abdominal pain, vomiting, and signs of an acute abdomen. The clinical presentation of our patient was consistent with these reports. As observed in previously published cases, the onset of symptoms shortly after an increase in the deferasirox dose raises suspicion of a causal relationship between the medication and duodenal perforation. High-dose deferasirox has been suggested to induce duodenal mucosal injury, potentially leading to ulcer formation and subsequent perforation. Although various surgical techniques have been described in the literature, our patient was successfully treated with primary repair and omentoplasty. Furthermore, the successful reintroduction of iron chelation therapy three weeks after surgery, together with proton pump inhibitor (PPI) therapy and without recurrence or complications, provides valuable follow-up data. In conclusion, duodenal perforation should be considered in children receiving deferasirox who present with acute abdominal symptoms, particularly following dose escalation. Early diagnosis, prompt surgical intervention, and a multidisciplinary approach are essential for improving patient outcomes.

Beta-talasemi major tanılı pediatrik hastada demir şelatörü kullanım birlikteliğinde gelişen duodenal perforasyon: Olgu sunumu ve literatür incelemesi

Amaç: Transfüzyon bağımlı beta-talasemi major hastalarında demir yükünü azaltmak amacıyla yaygın olarak oral demir şelatörü deferasiroks kullanılmaktadır. Bu hasta grubunda nadir olsa da literatürde duodenal perforasyon olguları bildirilmiştir. Çalışmamız yaşamı tehdit eden bir komplikasyonu olan duodenal perforasyon olgusunu ve literatürdeki benzer pediatrik vakalarla karşılaştırmayı hedeflemektedir.

Olgu Sunumu: Dört yaşında beta-talasemi major tanılı kız hasta; iki gündür devam eden karın ağrısı, ateş, kusma ve ishal yakınmalarıyla başvurdu. Hastanın 3 yaşından itibaren demir şelatörü (Deferasiroks) kullandığı ve başvurudan 3 hafta önce dozun artırıldığı öğrenildi. Batın BT’de intra-abdominal serbest hava saptanması üzerine acil laparotomiye alınan hastada, duodenumun birinci kıtasında yaklaşık 3 cm'lik perforasyon alanı izlendi. İki kat primer onarım ve omentoplasti uygulanan hasta, postoperatif 10. günde komplikasyonsuz taburcu edildi.

Tartışma ve Sonuç: Deferasiroks kullanımı olan beta-talasemi major tanılı pediatrik hastalarda duodenal perforasyon, yaşamı tehdit edebilen bir komplikasyondur. Literatürde bildirilen pediatrik olgular karın ağrısı, kusma ve akut batın bulguları ile başvurmuştur. Olgumuzun klinik özellikleri bu vakalarla uyumludur. Daha önce bildirilen olgularda olduğu gibi, semptomların deferasiroks doz artışını takip eden dönemde ortaya çıkması ilaçla nedensellik ilişkisi yönünden düşündürücü bir klinik durumdur. Yüksek doz deferasiroksun duodenal mukozal hasara neden olup ülser olşumunu tetikleyebileceği literatürde değinilmektedir. Literatürde farklı cerrahi yaklaşımlar tanımlanmış olmakla birlikte, olgumuzda uygulanan primer onarım ve omentoplasti ile başarılı sonuç elde edilmiştir. Ayrıca postoperatif üçüncü haftada şelasyon tedavisinin yeniden başlanabilmiş olması ve PPI kullanımı ile süreçte herhangi bir sorun yaşanmamış olması önemli bir takip verisi sunmaktadır. Sonuç olarak, deferasiroks kullanan çocuklarda, özellikle doz artışı sonrasında gelişen akut karın semptomlarında duodenal perforasyon akılda tutulmalı; erken tanı, cerrahi müdahale ile multidisipliner yaklaşım prognozu belirgin şekilde iyileştirebilmektedir.
