A Rare Migration Anomaly: Experience with Ipsilateral Orchiopexy in Crossed Testicular Ectopia

Introduction
Crossed testicular ectopia (CTE) is a rare testicular migration anomaly characterized by the descent of both gonads through the same inguinal canal into the ipsilateral inguinal region or the same hemiscrotum. Its estimated incidence is 1 in 4,000,000, with fewer than 150 reported cases. We present a patient with CTE diagnosed laparoscopically and successfully treated by ipsilateral orchiopexy.
Case Presentation
A nine-month-old boy presented with a non-palpable right testis. Physical examination revealed a palpable left testis in the inguinal canal, while the right testis was non-palpable. Ultrasonography demonstrated both testes within the left inguinal canal.
Laparoscopy showed both testes within a common mesorchium-like structure; the left testis was located in the inguinal canal and the right adjacent to the left internal inguinal ring. No Mullerian remnants were identified. After laparoscopic separation with preservation of the testicular vessels and vas deferens, sufficient cord length was achieved and bilateral transperitoneal orchiopexy was performed. The patient was discharged on the day of surgery. At one-year follow-up, both testes remained normally positioned within the scrotum, and genetic evaluation was unremarkable.
Conclusion
Although the embryological basis of CTE remains unclear, proposed mechanisms include Wolffian duct fusion, gubernacular abnormalities, and the influence of Müllerian remnants on testicular descent. The aim of surgery is to place both testes in the scrotum without tension while preserving vascular and vasal integrity. In transseptal orchiopexy, the ectopic testis is transferred through the scrotal septum to the opposite hemiscrotum; however, this may cause tension or compromise vascularity. In our patient, no Müllerian remnants were identified, and adequate length of the vessels and vas deferens was achieved after laparoscopic dissection. Therefore, bilateral transperitoneal ipsilateral orchiopexy was performed. When sufficient cord length and vascular supply are present, ipsilateral orchiopexy is a safe, anatomical, and effective treatment option.

Giriş
Çapraz testis ektopisi (ÇTE),her iki gonadın aynı inguinal kanal aracılığıyla ipsilateral inguinal bölgeye veya aynı hemiskrotuma yerleşmesiyle karakterize, nadir bir testiküler migrasyon anomalisidir. Tahmini insidansı 1/4.000.000 olup literatürde 150'den az olgu bildirilmiştir. Bu olgu sunumunda laparoskopi yardımıyla tanı alan ve ipsilateral orşiopeksi ile tedavi edilen bir ÇTE olgusu sunulmuştur.
Olgu Sunumu
Dokuz aylık erkek hasta sağ testisin olmaması şikayeti ile başvurdu. Fizik muayenede sol testis inguinal kanalda palpe edilirken sağ testis palpe edilemedi. Ultrasonografide her iki testisin sol inguinal kanalda olduğu rapor edildi.Tanı ve tedavi amacıyla yapılan laparoskopide her iki testisin ortak mezorkiyum benzeri yapı içerisinde seyrettiği sol testisin inguinal kanalda, sağ testisin ise sol internal inguinal halka komşuluğunda olduğu izlendi. Müllerian kanal kalıntısı saptanmadı. Testis damarları ve vas deferensler korunarak her iki testis laparoskopik olarak ayrıldı. Yeterli kord uzunluğu elde edilmesi üzerine bilateral transperitoneal orşiopeksi yapıldı. Hasta postoperatif aynı gün taburcu edildi. Bir yıllık takibinde her iki testis skrotumda normal olarak değerlendirildi. Tıbbi genetik değerlendirmesi normal raporlandı.
Sonuç
ÇTE’nin embriyolojik mekanizması tam olarak açıklanamamış olmakla birlikte Wolff kanalı füzyonu, gubernaküler yönlendirme bozukluğu ve Müllerian kalıntılarının testiküler migrasyon üzerine mekanik etkisi öne sürülmektedir. Cerrahi tedavide temel amaç testis vaskülaritesini ve vas deferens bütünlüğünü koruyarak her iki gonadı gerginlik oluşturmadan skrotuma yerleştirmektir. Geleneksel yaklaşım olan transseptal orşiopekside ektopik testis skrotal septumdan geçirilerek karşı hemiskrotuma alınır. Bu yöntem özellikle kord uzunluğu sınırlı olgularda uygulanabilir; ancak septal geçiş sırasında kord yapılarında açılanma, gerilim ve buna bağlı testiküler vaskülaritede bozulma görülebilir. Transperitoneal ipsilateral orşiopekside ise testis, laparoskopik mobilizasyon sonrası daha doğal anatomik hatta hemiskrotuma yönlendirilir. Bizim olgumuzda Müllerian kalıntı saptanmaması, laparoskopik disseksiyon sonrası her iki testiste yeterli damar ve vas deferens uzunluğunun sağlanması ve testislerin dolaşımının iyi olması nedeniyle transperitoneal ipsilateral orşiopeksi tercih edildi. Laparoskopide vasküler yapının uygun olması durumunda ipsilateral orşiopeksi güvenli, anatomik ve etkili bir tedavi seçeneğidir.
