A Novel Nutritional Screening Tool for the Assessment of Preoperative Malnutrition and Prediction of Surgical Complications in Children: Pediatric Perioperative Nutrition Screen (Pediatric-PONS)

Objective: Because of age- and sex-dependent variation in body mass index (BMI) in children, the standard Perioperative Nutrition Screen (PONS) cannot be applied directly to pediatric patients. This study retrospectively evaluated a BMI z-score–based pediatric PONS for detecting preoperative malnutrition and its association with postoperative complications.
Methods: Demographic, anthropometric, and complication data of pediatric surgical patients were reviewed retrospectively. Malnutrition was defined by World Health Organization (WHO) criteria. In addition to >10% weight loss, >50% reduced oral intake, and albumin <3.0 g/dL, a weight-for-age z-score (<5 years) or BMI z-score <−2 SD (≥5 years) replaced the adult BMI criterion. Cases with at least one positive parameter were considered PONS-positive. Malnourished/non-malnourished and PONS-positive/negative cases were compared for complications, surgical site infections, and length of stay.
Results: Ninety-four patients (male/female: 55/39; mean age 9.2 years, range 1–18) were included. Malnutrition was detected in 17 patients (18%), and 44% (41/94) were at risk by the modified PONS. PONS parameters were present as: >10% weight loss in 16 (17%), reduced oral intake in 28 (30%), albumin <3.0 g/dL in 2 (2%; not measured in 4), z-score <−2 SD in 19 (20%), and loss of appetite in 35 (37%). In the malnutrition group, BMI, BMI z-score, body weight, and mid-arm circumference were significantly lower (p<0.05). In the PONS-positive group, age, height, weight, BMI, and z-scores were significantly lower, and length of stay was longer (9 vs 6 days, p=0.003); loss of appetite was more frequent (p<0.001). No significant difference was found in surgical site infection or other complications.
Conclusions: The pediatric PONS modified with BMI z-score identifies malnutrition risk associated with lower anthropometric measurements and longer hospital stay. In this cohort, complication and infection rates did not differ significantly, so findings warrant confirmation in larger series. Pediatric PONS is a practical tool for perioperative nutritional assessment.
Çocuklarda Preoperatif Malnütrisyonun Değerlendirilmesi ve Cerrahi Komplikasyonların Öngörülmesinde Yeni Bir Nütrisyon Tarama Aracı: Pediatrik-PONS

Amaç: Çocuklarda yaşa ve cinsiyete bağlı vücut kitle indeksi (VKİ) değişimleri nedeniyle standart Perioperative Nutrition Screen (PONS) pediatrik hastalarda doğrudan uygulanamaz. Bu çalışmada VKİ yerine VKİ z-skoru kullanılan pediatrik PONS'un preoperatif malnütrisyonu saptamadaki rolü ve postoperatif komplikasyonlarla ilişkisi geriye dönük incelenmiştir.
Yöntem: Cerrahi planlanan çocukların demografik, antropometrik ve komplikasyon verileri geriye dönük incelendi. Malnütrisyon Dünya Sağlık Örgütü (DSÖ) kriterleriyle tanımlandı. PONS skorlamasında son 6 ayda >%10 kilo kaybı, son bir haftada >%50 oral alım azalması ve albümin <3,0 g/dL kriterlerine ek olarak, erişkindeki VKİ ölçütü yerine <5 yaşta yaşa göre ağırlık z-skoru, ≥5 yaşta VKİ z-skoru <−2 SD alındı. Erişkin PONS'ta olduğu gibi en az bir parametresi pozitif olan olgular PONS pozitif kabul edildi. Malnütrisyonlu/malnütrisyonsuz ve PONS pozitif/negatif olgular komplikasyon, cerrahi alan enfeksiyonu ve yatış süresi açısından karşılaştırıldı.
Bulgular: Yaş ortalaması 9,2 yıl (1–18) olan 94 olgu (erkek/kız: 55/39) dahil edildi. Olguların 17'sinde (%18) malnütrisyon saptandı; VKİ z-skoru dahil modifiye PONS'a göre %44'ü (41/94) riskli bulundu. Hasta grubunda PONS parametrelerinin dağılımı; >%10 kilo kaybı 16 (%17), oral alım azalması 28 (%30), albümin <3,0 g/dL 2 (%2; 4 olguda ölçüm yok), z-skoru <−2 SD 19 (%20) ve iştahsızlık 35 (%37) olguda saptandı. Malnütrisyonlu grupta VKİ, VKİ z-skoru, vücut ağırlığı ve orta kol çevresi anlamlı düşüktü (p<0,05). PONS pozitif grupta yaş, boy, kilo, VKİ ve z-skorları anlamlı düşük, yatış süresi ise anlamlı uzundu (9 vs 6 gün, p=0,003); iştahsızlık daha sıktı (p<0,001). Cerrahi alan enfeksiyonu ve diğer komplikasyonlar açısından anlamlı fark saptanmadı.
Sonuç: VKİ z-skoru ile modifiye pediatrik PONS, daha düşük antropometrik ölçümlerle giden malnütrisyon riskini saptamakta ve daha uzun hastanede yatışla ilişkili bulunmuştur. Bu kohortta komplikasyon ve enfeksiyon sıklığı açısından fark izlenmemiş olup bulgular daha geniş serilerde doğrulanmalıdır. Pediatrik PONS, perioperatif beslenme değerlendirmesinde uygulanabilir bir tarama aracıdır.
