The Effect of Virtual Reality Headset Use on Pain, Comfort, and Psychosocial Symptoms in Children Undergoing Hypospadias Surgery: A Randomized Controlled Trial

Aim:

The aim of this study was to examine the effect of using virtual reality (VR) headsets on pain, comfort, and psychosocial symptoms in children following hypospadias surgery.

Materials and Methods:

The sample for this randomized controlled study consisted of 62 school-age children who underwent hypospadias surgery (study group: 31, control group: 31). Data were collected using a Descriptive Information Form, the Wong-Baker FACES Pain Scale, the Pediatric Comfort Assessment Scale, and the Scale for Identifying Psychosocial Symptoms in Hospitalized Children. The data were analyzed using IBM SPSS Statistics 22.0 software, employing descriptive statistics, the independent samples t-test, the chi-square test, and mixed-design repeated measures analysis. A significance level of p<0.05 was accepted.

Results:

Sixty-two children aged 6–12 years were included in the study. Twenty-four children (77.4%) in the control group and 22 (71%) in the study group were undergoing hypospadias repair. Nineteen children (61.3%) in the control group and 22 (71%) in the study group had no prior knowledge of VR headsets. While there was no significant difference between the groups regarding Wong-Baker Pain Scale scores before the procedure, the study group’s pain scores were significantly lower after the procedure (p<0.001; d=-1.30). Although Pediatric Comfort Assessment Scale scores were similar before the procedure, the post-procedure score was 92.32±5.44 for the study group and 79.29±12.1 for the control group, representing a significant difference (p<0.001; d=1.39). CPSS scores were similar before the procedure. After the procedure, the study group's score decreased to 9.32±8.06, whereas the control group's score rose to 17.10±12.41; this difference was statistically significant (p=0.005).

Conclusion:

It was determined that the use of VR headsets following hypospadias surgery was effective in reducing pain and psychosocial symptoms and in increasing comfort among children.

Ethics Committee Approval (decision number 1371; E-10840098-202.3.02-170) dated December 26, 2024, and institutional permission (E-97248701-300-1497694) from the training and research hospital where the study was conducted were obtained.

Amaç:
Bu çalışmanın amacı, sanal gerçeklik (VR) gözlüğü kullanımının hipospadias ameliyatı sonrası çocuklarda ağrı, konfor ve psikososyal semptomlar üzerine etkisini incelemektir.

Gereç ve Yöntem:
Bu randomize kontrollü çalışmanın örneklemini hipospadias ameliyatı geçiren 62 okul çağı çocuğu oluşturdu (çalışma grubu: 31, kontrol grubu: 31). Veriler Tanıtıcı Bilgi Formu, Wong-Baker FACES Ağrı Ölçeği, Pediatrik Konfor Değerlendirme Ölçeği ve Hastanede Yatan Çocuklar İçin Psikososyal Semptomları Tanımlama Ölçeği ile toplandı. Veriler IBM SPSS Statistics 22.0 programında tanımlayıcı istatistikler, bağımsız örneklem t-testi, ki-kare testi ve karma tekrarlı ölçümler analizi kullanılarak değerlendirildi. Anlamlılık düzeyi p<0,05 olarak kabul edildi.

Bulgular:
Çalışmaya yaşları 6-12 arasında değişen 62 çocuk dahil edildi. Kontrol grubundaki çocukların 24’ü (%77,4), çalışma grubundakilerin 22’si (%71) hipospadias onarım olgusuydu. Kontrol grubunun 19’u (%61,3), çalışma grubunun ise 22’si (%71) daha önce VR gözlüğü hakkında bilgi sahibi değildi. İşlem öncesinde Wong-Baker Ağrı Ölçeği puanları açısından gruplar arasında anlamlı fark bulunmazken, işlem sonrasında çalışma grubunun ağrı puanları anlamlı düzeyde daha düşük bulundu (p<0,001; d=-1,30). Pediatrik Konfor Değerlendirme Ölçeği puanları işlem öncesinde benzerken, işlem sonrasında çalışma grubunun puanı 92,32±5,44, kontrol grubunun puanı 79,29±12,1 olup fark anlamlıydı (p<0,001; d=1,39). ÇPSTÖ puanları işlem öncesinde benzerdi. İşlem sonrasında çalışma grubunun puanı 9,32±8,06'ya düşerken, kontrol grubunun puanı 17,10±12,41'e yükseldi ve fark istatistiksel olarak anlamlı bulundu (p=0,005).

Sonuç:
Hipospadias ameliyatı sonrası uygulanan VR gözlüğünün çocuklarda ağrı ve psikososyal semptomların azaltılmasında ve konforun artırılmasında etkili olduğu belirlendi.
