Fatal Abdominal Burden: A Case Report of a 17-Year-Old Body Packer

Introduction
Body packing refers to the concealment of illicit drugs within the gastrointestinal tract by swallowing or rectally inserting drug-filled packets for the purpose of illegal transportation. Although relatively uncommon in the pediatric and adolescent population, rupture of these packets may result in acute toxicity and fatal outcomes, necessitating prompt, multidisciplinary, and highly cautious management.
Case Presentation
A 17-year-old male was apprehended at an airport because of suspicious behavior and was brought to the emergency department under police custody. The patient reported swallowing 40 drug packets the previous night and inserting an additional 10 packets rectally, for a total of 50 packets. Physical examination revealed abdominal distension and tenderness on palpation. Digital rectal examination was deliberately avoided because of the potential risk of packet rupture. Upright abdominal radiography and non-contrast abdominal-pelvic computed tomography (CT) demonstrated numerous packet-like foreign bodies extending throughout the colon and into the pelvis. As there were no indications for emergency surgery, conservative management with high-volume enemas was initiated. The patient was admitted to the pediatric surgery ward and monitored for spontaneous passage of the packets under police supervision. The expelled packets were counted daily by law enforcement officers. By the morning ward round on May 22, 2026, a total of 106 drug packets had been recovered. Follow-up non-contrast abdominal-pelvic CT confirmed complete clearance of the gastrointestinal tract with no retained foreign bodies, and the patient was discharged in good clinical condition.
Conclusion
This case highlights the marked discrepancy between the patient's reported number of ingested packets (50) and the actual number recovered (106), demonstrating that self-reported histories in body packers should not be considered reliable. In hemodynamically stable patients without evidence of bowel obstruction or packet rupture, conservative management with enemas and laxatives is a safe and effective first-line approach. Complete gastrointestinal clearance should always be confirmed by non-contrast CT before discharge. In adolescent body packers, discharge decisions should rely not only on clinical improvement or patient history but also on meticulous radiological confirmation of complete evacuation.

Giriş
'Body packing', uyuşturucu maddelerin yasadışı yollarla taşınması amacıyla paketlenerek gastrointestinal sisteme yerleştirilmesi durumudur. Çocuk ve adolesan yaş grubunda nispeten nadir görülmekle birlikte, paketlerin rüptürü halinde akut toksisite ve ölümcül sonuçlar doğurabilmesi nedeniyle acil, multidisipliner ve çok dikkatli bir yaklaşım gerektirir.
Olgu Sunumu
Kabul: 17 yaşında erkek hasta, havaalanında şüpheli durum üzerine tutuklanarak acil servis kırmızı alana getirildi.
Anamnez: Bir gece önce 40 adet poşet yutma ve rektumdan 10 adet poşet yerleştirme (toplam 50 poşet) öyküsü mevcut.
Fizik Muayene: Batın distansiyonu ve palpasyonla hassasiyet saptandı. Poşetlerin rüptüre olma riski göz önüne alınarak rektal tuşe (RT) yapılmadı.
Görüntüleme: Ayakta Direkt Batın Grafisi (ADBG) ve Kontrastsız Batın-Pelvis BT'de kolon trasesi boyunca uzanan ve pelvise inen çok sayıda poşet benzeri yabancı cisim izlendi.
Müdahale ve Klinik Seyir: Hastaya acil cerrahi endikasyonu konulmadı, konservatif yaklaşım tercih edilerek yüksek lavman uygulandı. Çocuk cerrahisi servisine yatırılan hasta, polis memurları eşliğinde defekasyon takibine alındı. Çıkarılan paketlerin sayımı günlük olarak güvenlik güçlerince yapıldı.
Sonuç: 22.05.2026 tarihindeki sabah vizitine kadar hastadan *toplam 106 adet poşet* çıkarıldı. Kontrol abdomen ve pelvis BT'sinde gastrointestinal sistemde yabancı cisim saptanmaması üzerine hasta şifa ile taburcu edildi.
Sonuç
Anamnez Güvenilirliği: Bu olgudaki en kritik nokta, hastanın beyanı (50 poşet) ile vücuttan çıkarılan gerçek sayı (106 poşet) arasındaki dramatik uyumsuzluktur. Bu durum, 'body packer' olgularında hastanın verdiği bilgiye kesinlikle güvenilmemesi gerektiğini açıkça göstermektedir.
Konservatif Yaklaşım: Mekanik bağırsak tıkanıklığı veya zehirlenme bulguları (rüptür şüphesi) olmayan stabil hastalarda, yüksek lavman ve laksatiflerle yapılan konservatif takip güvenli ve etkili ilk seçenektir.
Radyolojinin Rolü: Gastrointestinal sistemin tam olarak temizlendiğinin teyit edilmesi, hayati öneme sahiptir. Kontrastsız BT ve 3D rekonstrüksiyon görüntüleri, paketlerin lokalizasyonunu ve sayısını belirlemede, ayrıca taburculuk kararı vermede altın standarttır.
Adolesan 'body packer' olgularında komplikasyon gelişmediği sürece cerrahi dışı konservatif yönetim başarılıdır. Ancak, hastaların eksik veya yanıltıcı bilgi verme eğiliminde oldukları unutulmamalı; taburculuk kararı sadece anamneze veya klinik iyileşmeye göre değil, mutlaka titiz ve ileri radyolojik değerlendirmelerle (BT) doğrulanarak verilmelidir.
