Congenital Neuroblastoma Mimicking Sacrococcygeal Teratoma: A Case Report

Introduction: Neuroblastoma is the most common extracranial solid malignancy of childhood, most frequently arising from the adrenal medulla and sympathetic ganglia; sacrococcygeal involvement is rare. In the neonatal period, sacrococcygeal neuroblastomas can be difficult to distinguish from sacrococcygeal teratomas due to overlapping clinical and radiological features, posing a diagnostic challenge. We present a newborn who underwent surgery with a presumptive diagnosis of sacrococcygeal teratoma but was ultimately diagnosed with neuroblastoma on histopathological examination.

Case: A female infant, born at 39+3 weeks of gestation weighing 3470g with no mass detected antenatally, was found to have a sacrococcygeal mass on the first day of life. Ultrasonography revealed a 31×29×22mm well-lobulated solid lesion adjacent to the distal sacrum. MRI demonstrated a 47×24×30mm heterogeneously enhancing mass encircling the coccyx and extending into the pelvis, favored to represent sacrococcygeal teratoma; AFP was within age-appropriate limits while NSE was elevated (31.3ng/mL). On day six of life, total mass excision with coccygectomy was performed; intraoperatively, the mass was predominantly cystic, containing fat and hair components. Histopathology revealed poorly differentiated neuroblastoma with a mitotic-karyorrhectic index <2%; PHOX2B was diffusely positive, and the surgical margin was positive in the main specimen. Molecular analysis showed no MYCN or ALK amplification, 11q23 loss, and a DNA ploidy index of 1 (near-diploid). Bilateral bone marrow biopsy and aspiration showed no infiltration. NSE normalized on follow-up, and postoperative whole-body MRI showed no residual, recurrent, or metastatic disease apart from postsurgical changes at the coccygeal level. Based on the combined MRI and pathology findings, the patient was staged as L1 according to the TPOG 2026 protocol, and observation without adjuvant therapy was decided.

Conclusion: Sacrococcygeal neuroblastoma is rare in the neonatal period. As in this case, radiological and intraoperative macroscopic features may mimic sacrococcygeal teratoma, complicating preoperative diagnosis, and definitive diagnosis relies on histopathological examination. Despite a positive surgical margin, the favorable molecular profile and absence of disseminated disease supported L1 staging and an observation strategy. Neuroblastoma should always be considered in the differential diagnosis of congenital sacrococcygeal masses.

Sakrokoksigeal Teratomu Taklit Eden Konjenital Nöroblastom: Bir Olgu Sunumu

Giriş: Nöroblastom çocukluk çağının en sık ekstrakraniyal solid malignitesidir; en sık adrenal medulla ve sempatik ganglionlardan kaynaklanır. Sakrokoksigeal yerleşim nadirdir. Neonatal dönemde sakrokoksigeal nöroblastomlar klinik ve radyolojik özellikleri nedeniyle sakrokoksigeal teratomlarla karışabilmekte ve tanısal güçlük oluşturabilmektedir. Bu olgu sunumunda sakrokoksigeal teratom ön tanısıyla opere edilen ve histopatolojik inceleme sonucunda nöroblastom tanısı alan bir yenidoğan sunulmuştur.

Olgu: Antenatal dönemde kitle saptanmayan, takipli gebelik sonrası 39+3 haftalık gebelikten 3470 g doğan kız hastada postnatal sıfırıncı günde sakrokoksigeal bölgede kitle fark edildi. Ultrasonografide sakrum distal ucu ile komşu, 31×29×22 mm boyutlarında lobüle konturlu solid lezyon izlendi. MR'da presakral alandan koksiksi çevreleyen ve pelvise uzanan 47×24×30 mm boyutlarında heterojen kontrastlanan, sakrokoksigeal teratomla uyumlu değerlendirilen kitle saptandı; AFP yaşa göre normal, NSE yüksekti (31,3 ng/mL). Postnatal 6. günde total kitle eksizyonu ve koksigektomi uygulandı; kitle büyük ölçüde kistik yapıda olup yağ ve kıl komponentleri içeriyordu. Histopatolojik incelemede az diferansiye nöroblastom, mitotik-karyorektik indeks <%2 saptandı; PHOX2B diffüz pozitifti, ana kitlede cerrahi sınır pozitifti. Moleküler incelemede MYCN ve ALK amplifikasyonu negatif, 11q23 kaybı pozitif, DNA ploidi indeksi DI=1 (near diploid) idi. Bilateral kemik iliği biyopsi ve aspirasyonunda infiltrasyon saptanmadı. Postoperatif dönemde yara yeri revizyonu uygulandı, izlemde NSE normale geriledi. Postoperatif tüm vücut MR'da koksiks düzeyinde cerrahiye bağlı değişiklikler dışında rezidü, rekürren kitle veya metastaz bulgusu izlenmedi. Postoperatif MR ve patoloji sonuçları birlikte değerlendirilerek hasta TPOG 2026 protokolüne göre L1 olarak evrelendirildi ve adjuvan tedavi uygulanmaksızın izlem kararı alındı.

Sonuç: Sakrokoksigeal yerleşimli nöroblastomlar neonatal dönemde nadir görülmekte; radyolojik bulgular ve intraoperatif makroskopik özellikler sakrokoksigeal teratomu düşündürerek preoperatif tanıyı güçleştirebilmektedir. Kesin tanı histopatolojik incelemeyle konulmaktadır. Cerrahi sınır pozitifliğine rağmen düşük risk moleküler profil ve yaygın metastaz olmaması, L1 evrelemesi ve izlem kararını desteklemiştir. Sakrokoksigeal kitlelerin ayırıcı tanısında nöroblastom mutlaka akılda tutulmalıdır.
