Posterior sagittal approach in pediatric retrorectal masses: surgical and long-term functional outcomes

Aim:
The posterior sagittal approach provides adequate exposure for pediatric retrorectal masses, particularly lesions distal to the third sacral vertebra. However, data on its long-term effects on bowel and urinary functions are limited because of its close relationship with pelvic floor anatomy. This study evaluated surgical outcomes and long-term functional status in pediatric patients operated on for retrorectal masses using this approach.

Methods:
Patients with retrorectal masses located distal to the third sacral vertebra, without abdominal extension, who underwent posterior sagittal surgery between January 2016 and January 2026 were retrospectively reviewed. Demographic data, clinical and radiological findings, histopathological diagnoses, perioperative complications, and follow-up outcomes were evaluated. Long-term bowel and urinary functions were assessed using a structured telephone questionnaire based on the Rintala Bowel Function Score and Pediatric Lower Urinary Tract Symptom Score.

Results:
Seven patients ; 3 were male and 4 were female. The median age at surgery was 15 years (6 months–16 years), and the median follow-up was 60 months (48–144 months). Histopathological diagnoses were immature teratoma (n=2), retrorectal mature teratoma (n=1), yolk sac tumor (n=1), dermoid cyst (n=1), epidermoid cyst (n=1), and tailgut cyst (n=1). Macroscopic complete resection and coccygectomy were performed in all patients. Intraoperative mass perforation occurred in one patient, and wound infection resolved with conservative treatment in one patient. Adjuvant chemotherapy was administered to the patient with yolk sac tumor and microscopic positive surgical margin. No local recurrence was observed. In long-term assessment, bowel and urinary functions were normal in 6 patients (85.7%). Preoperative functional status could not be evaluated in the patient operated on during infancy.

Conclusions:
With appropriate patient selection and meticulous reconstruction, the posterior sagittal approach is safe and effective for pediatric retrorectal masses, with satisfactory long-term functional outcomes.

Pediatrik retrorektal kitlelerde posterior sagittal yaklaşım: cerrahi ve uzun dönem fonksiyonel sonuçlar

Amaç:
Posterior sagittal yaklaşım, çocukluk çağı retrorektal kitlelerinde özellikle sakral 3. vertebra distalinde yerleşen lezyonlarda yeterli cerrahi görüş sağlayan etkili bir yöntemdir. Ancak pelvik taban anatomisi ile yakın ilişkisi nedeniyle uzun dönem bağırsak ve üriner fonksiyonlar üzerindeki etkilerine ilişkin veriler sınırlıdır. Bu çalışmada posterior sagittal yaklaşımla ameliyat edilen pediatrik retrorektal kitleli hastalarda cerrahi sonuçlar ve uzun dönem fonksiyonel durum değerlendirildi.

Yöntem:
Ocak 2016–Ocak 2026 arasında posterior sagittal yaklaşımla ameliyat edilen, sakral 3. vertebra distalinde yerleşimli ve batına uzanımı olmayan retrorektal kitleli hastalar retrospektif olarak incelendi. Demografik veriler, klinik-radyolojik bulgular, histopatolojik tanılar, perioperatif komplikasyonlar ve takip sonuçları değerlendirildi. Uzun dönem bağırsak ve üriner fonksiyonlar, Rintala Bowel Function Score ve Pediatric Lower Urinary Tract Symptom Score temel alınarak oluşturulan yapılandırılmış telefon anketi ile değerlendirildi.

Bulgular:
Çalışmaya 7 hasta dahil edildi; 3’ü erkek, 4’ü kızdı. Medyan ameliyat yaşı 15 yıl (6 ay–16 yıl), medyan takip süresi 60 ay (48–144 ay) idi. Histopatolojik tanılar immatür teratom (n=2), retrorektal matür teratom (n=1), yolk sac tümörü (n=1), dermoid kist (n=1), epidermoid kist (n=1) ve tailgut kisti (n=1) idi. Tüm hastalarda makroskopik tam rezeksiyon ve koksigektomi uygulandı. Bir hastada intraoperatif kitle perforasyonu, bir hastada konservatif tedavi ile düzelen yara yeri enfeksiyonu gelişti. Mikroskopik cerrahi sınır pozitifliği saptanan yolk sac tümörlü hastaya adjuvan kemoterapi verildi. Takipte lokal nüks izlenmedi. Uzun dönem değerlendirmede 6 hastada (%85,7) bağırsak ve üriner fonksiyonlar normaldi. İnfantil dönemde ameliyat edilen matür teratomlu olguda preoperatif fonksiyonel durum değerlendirilemedi.

Sonuç:
Uygun hasta seçimi ve titiz anatomik rekonstrüksiyon ile posterior sagittal yaklaşım, pediatrik retrorektal kitlelerde güvenli ve etkili bir cerrahi seçenek olup uzun dönem fonksiyonel sonuçlar açısından tatmin edici görünmektedir.
