Transthoracic Belsey Mark IV fundoplication in pediatric patients with a history of complex abdominal surgery: experience with two cases

Introduction:
Laparoscopic fundoplication is a standard surgical approach for gastroesophageal reflux disease in children. However, an abdominal approach may be technically challenging in patients with previous multiple abdominal operations or esophageal reconstruction. This report presents the early outcomes of transthoracic Belsey Mark IV fundoplication in two pediatric patients with complex surgical histories.
Case presentation:
The first case was a 13-year-old boy with a history of left hepatectomy and right medial segmentectomy for hepatoblastoma, and laparotomy for small bowel perforation. He had severe vomiting and inadequate oral intake. Upper gastrointestinal contrast study showed pathological gastroesophageal reflux, hiatal hernia, and distal esophageal stricture. Endoscopy revealed distal esophageal stricture and Los Angeles Grade D esophagitis. The DeMeester score was 14.83. Antireflux surgery was performed after five esophageal dilatations.
The second case was a 16-year-old boy who had undergone Stamm gastrostomy, traction fistula technique, and subsequent esophagoesophagostomy for type A long-gap esophageal atresia. He had multiple dilatations for anastomotic stricture and presented with severe vomiting and feeding intolerance. Contrast study showed pathological reflux, and endoscopic biopsy revealed intestinal metaplasia. The DeMeester score was 57.86.
Because of their complex surgical histories, transthoracic Belsey Mark IV fundoplication was preferred in both patients. Operative time was approximately 60 minutes. Oral feeding was started on postoperative day 3, and chest drains were removed on day 5, followed by discharge on the same day. In the first case, reflux symptoms regressed during 12 weeks of follow-up without complications. In the second case, two dilatations were performed for anastomotic stricture due to pain and dysphagia after solid food intake during 5 months of follow-up.
Conclusion:
In selected pediatric patients with complex abdominal surgery or esophageal reconstruction histories, transthoracic Belsey Mark IV fundoplication may be a safe and feasible alternative when the abdominal approach is challenging.
Kompleks abdominal cerrahi öyküsü bulunan pediatrik hastalarda transtorasik Belsey Mark IV fundoplikasyonu: iki olguluk deneyim

Giriş:
Laparoskopik fundoplikasyon, çocuklarda gastroözofageal reflü hastalığının cerrahi tedavisinde standart yaklaşımlardandır. Ancak çoklu abdominal cerrahi veya özofagus rekonstrüksiyonu öyküsü bulunan hastalarda abdominal yaklaşım teknik olarak güç olabilir. Bu bildiride, kompleks cerrahi öyküsü bulunan iki pediatrik olguda transtorasik Belsey Mark IV fundoplikasyonu sonrası erken dönem sonuçlar sunuldu.
Olgu sunumu:
Birinci olgu, hepatoblastom nedeniyle sol hepatektomi ve sağ medial segmentektomi, ayrıca ince bağırsak perforasyonu nedeniyle laparotomi öyküsü bulunan 13 yaşında erkek hastaydı. Şiddetli kusma ve oral alım yetersizliği mevcuttu. Özofagus-mide-duodenum grafisinde patolojik gastroözofageal reflü, hiatal herni ve distal özofageal darlık saptandı. Endoskopide distal özofageal darlık ve Los Angeles Grade D özofajit izlendi. DeMeester skoru 14,83 idi. Beş kez özofagus dilatasyonu sonrası antireflü cerrahisi uygulandı.
İkinci olgu, tip A long-gap özofagus atrezisi nedeniyle yenidoğan döneminde Stamm gastrostomi, sütür fistülü tekniği ve sonrasında özofagoözofagostomi uygulanmış 16 yaşında erkek hastaydı. Anastomoz darlığı nedeniyle çok sayıda dilatasyon öyküsü vardı. Şiddetli kusma ve beslenememe yakınmaları mevcuttu. Grafide patolojik reflü, endoskopik biyopside intestinal metaplazi saptandı. DeMeester skoru 57,86 idi.
Her iki hastada kompleks cerrahi öykü nedeniyle transtorasik Belsey Mark IV fundoplikasyonu tercih edildi. Operasyon süresi yaklaşık 60 dakika idi. Hastalar postoperatif üçüncü günde oral beslendi; toraks drenleri beşinci günde çekilerek aynı gün taburcu edildi. Birinci olguda 12 haftalık takipte reflü semptomları geriledi ve komplikasyon izlenmedi. İkinci olguda postoperatif beş aylık dönemde katı gıda sonrası ağrı ve takılma nedeniyle iki kez anastomoz darlığına dilatasyon uygulandı.
Sonuç:
Kompleks abdominal cerrahi veya özofagus rekonstrüksiyonu öyküsü bulunan seçilmiş pediatrik hastalarda transtorasik Belsey Mark IV fundoplikasyonu, abdominal yaklaşımın güç olduğu durumlarda güvenli ve uygulanabilir bir alternatif olabilir.
