Analysis of Clinical Conditions with Signal Loss in Indocyanine Green (ICG) Fluorescent Imaging

Aim:To investigate the potential mechanisms underlying the fluorescence signal loss in ICG-guided surgery.

Methods:Five patients who were scheduled for ICG-guided surgery with different surgical indications but failed to yield fluorescent imaging were included in the study. Three patients scheduled for cholecystectomy received intravenous 0.4 mg/kg ICG 10-16 hours before surgery, 1 patient diagnosed with urachal cyst received 25 mg intraoperative intravesical ICG, 1 patient with a lung infection(fungus ball) received 5 mg intratracheal ICG during intraoperative anesthesia induction.

Results:In two cholecystectomy cases, low-level ICG fluorescent imaging was observed only in intestines and mildly in distal CBD. In one cholecystectomy patient, no-ICG fluorescent image could be obtained. All three cases had history of severe cholecystitis. In all three cholecystectomy patients, duodenum and omentum were completely adherent to gallbladder. Dense abscess and stones were present inside gallbladder in all three patients, and no bile was detected within gallbladder in any of them. In all three cases, it was evaluated that newly synthesized ICG-stained bile could not reach gallbladder due to permanent fibrotic/inflammatory obstructions at gallbladder, and thus no-fluorescence occurred. In the patient who underwent intravesical administration for a urachal cyst, the fluorescent signal could not be visualized due to the tissue penetration barrier. In the case operated on for a fungus ball with ICG administration, ICG protocol failed to achieve the clinical objective because the deep-located, poorly demarcated infectious/fungal pathology exhibited fluorescent emission similar to that of normal parenchyma.

Conclusions:ICG fluorescent signal can be masked by anatomical and inflammatory obstacles such as fibrotic obstructions due to severe cholecystitis, bladder wall thickness, and deep-located parenchymal infections. In cases with history of severe cholecystitis, reducing pre-operative ICG injection time to 1-2 hours may optimize anatomical visualization and signal quality by creating negative fluorescence in gallbladder and providing CBD-parenchyma hyperfluorescence advantage.

İndosiyanin Yeşili (ICG) Floresan Görüntüleme Sinyal Kaybının Olduğu Klinik Durumların Analizi

Amaç: Bu çalışmanın amacı, ICG kılavuzluğunda yapılan cerrahilerde floresan sinyal kaybına yol açan olası nedenlerini incelemektir.

Yöntem: Çalışmaya farklı cerrahi endikasyonlarla ICG kılavuzluğunda operasyon planlanan ancak floresan görüntüleme elde edilemeyen toplam 5 hasta dahil edildi. Kolesistektomi planlanan 3 hastaya operasyondan 10-16 saat önce intravenöz 0.4 mg/kg dozunda ICG; urakal kist tanısı olan 1 hastaya intraop intravezikal 25 mg ICG; akciğer enfeksiyonu (fungus topu) olan 1 hastaya ise intraoperatif anestezi indüksiyonu sırasında intratrakeal 5 mg dozunda ICG uygulandı.

Bulgular: Gerçekleştirilen cerrahi girişimlerde, uygulanan ICG protokollerine rağmen hedeflenen dokularda floresan sinyal cerrahi olarak izlenemedi. İki kolesistektomi olgusunda ICG floresan görüntülemesi düşük düzeyde sadece barsaklarda ve hafif düzeyde koledok distalde izlendi. Bir kolesistektomi hastasında ise hiç bir şekilde ICG floresan görüntüsü elde edilemedi. Her üç olgunun da ciddi kolesistit atakları öyküsü vardı. Her üç kolesistektomi hastasında duodenum ve omentum kesenin her tarafına yapışık idi. Her üç hastada safra kesesi içinde yoğun abse ve taşlar olup hiç birinde kese içinde safra saptanmadı. Her üç olgumuzda da safra kesesi ve sistik kanal düzeyinde kalıcı fibrotik/inflamatuar tıkanıklıklar nedeniyle yeni sentezlenen ICG’li safranın keseye ulaşamadığı ve buna bağlı olarak floresan ışımanın gerçekleşmediği değerlendirilmiştir. Urakal kist nedeniyle, intravezikal yolla uygulanan hastada doku penetrasyon bariyeri nedeniyle floresan sinyali vizüalize edilememiştir. Fungal topu nedeniyle cerrahi yapılan ve ICG uygulanan olguda derin yerleşimli, belirsiz sınırlı enfeksiyöz/fungal patolojinin normal parankimle benzer floresan ışıma sergilemesi nedeniyle ICG protokolü klinik hedefe ulaşamamıştır.

Sonuç: ICG floresan sinyali; şiddetli kolesistite bağlı fibrotik tıkanıklıklar, mesane duvar kalınlığı ve derin yerleşimli parankimal enfeksiyonlar gibi anatomik/inflamatuar engellerle maskelenebilir. Şiddetli kolesistit öykülü olgularda, cerrahi öncesi ICG enjeksiyon süresinin 1-2 saate indirilmesi; safra kesesinde negatif floresan oluşturup koledok-parankim hiperfloresan avantajı sağlayarak kolesistektomide anatomik vizüalizasyonu ve sinyal kalitesini optimize edebilir.
