Surgical management of gastrointestinal perforations in neonates: Is stoma still the safest option?

Aim:To evaluate neonates who developed gastrointestinal (GI) perforation during neonatal intensive care unit (NICU) hospitalization and underwent surgical treatment.
Methods:Medical records of neonates who developed GI perforation during NICU hospitalization and underwent surgery at our institution over the past ten years were retrospectively reviewed. Demographic characteristics, clinical presentation, radiological and operative findings, and surgical treatment methods were analyzed.
Results:Twenty-eight neonates (19 males/9 females) underwent surgery for GI perforation. The mean gestational age was 31.3±4.2 (23–38) weeks, and the mean birth weight was 1641±806.2 (550–3700) gram. The most common preoperative clinical finding was abdominal distension (n=26), the most frequent radiological finding was pneumoperitoneum (n=16), and the most common laboratory abnormality was acidosis (n=14). The mean age at surgery was 13±15.7 (1–69) days. Perforation was identified in the stomach in 4 neonates, the small intestine in 14, the large intestine in 7, and both the small and large intestines in 3. Twenty-three neonates had a single perforation, whereas 5 had multiple perforations. A stoma was created in 24 neonates (85.7%) (gastrostomy/ileostomy/colostomy=3/18/3), whereas primary repair without stoma was performed in only 4. Nine neonates were diagnosed with necrotizing enterocolitis, 2 with spontaneous intestinal perforation, 3 with perforation secondary to meconium ileus, and 1 with perforation associated with Hirschsprung disease. No specific etiology other than prematurity was identified in the remaining patients. Five neonates (17.8%) died a mean of 10.4±9 (1–24) days after surgery, whereas the remaining 23 were discharged a mean of 53.6±38.5 (10–167) days postoperatively. Among the 19 surviving neonates who underwent intestinal stoma formation, stoma closure was performed a mean of 155.3±95.2 (23–283) days after the initial operation.
Conclusions:A stoma was created in the vast majority of patients, and the mortality rate was lower than that reported in the literature. Although stoma formation increases the number of surgical procedures, it may shorten the initial operation and contribute to lower mortality. Therefore, stoma formation may still be considered the preferred first-line surgical option, even in patients who are candidates for primary anastomosis.

Amaç: Bu çalışmada, yenidoğan yoğun bakım ünitesinde (YYBÜ) izlenirken gastrointestinal sistem (GİS) perforasyonu saptanan ve cerrahi tedavi uygulanan hastaların incelenmesi amaçlanmıştır.
Yöntem: Kliniğimizde, son 10 yıl içinde YYBÜ’de izlenirken GİS’de perforasyon saptanan ve opere edilen hastaların kayıtları geriye yönelik incelenmiş; demografik özellikleri, klinik, radyolojik ve operasyon bulguları ile cerrahi tedavi yöntemleri kaydedilmiştir.
Bulgular: Toplam 28 bebek (19 erkek/9 kız) GİS’de perforasyon saptanarak opere edilmiştir. Bebeklerin ortalama doğum haftaları 31,3±4,2 (23-38) hafta, ortalama doğum kiloları 1641±806,2 (550-3700) gramdır. Operasyon öncesi en sık klinik bulgu karında distansiyonken (n=26), en sık grafi bulgusu serbes hava (n=16), en sık laboratuvar bulgusu ise asidozdur (n=14). Bebeklerin ortalama operasyon yaşları 13±15,7 (1-69) gündür. Operasyonlarda 4 bebekte midede perforasyon saptanırken, 14 bebekte ince bağırsakta, 7 bebekte kalın bağırsakta, 3 bebekte de hem ince hem de kalın bağırsakta perforasyon görülmüştür. Bebeklerden 23’ünde tek bir perforasyon varken 5 bebekte birden çok perforasyon saptanmıştır. 24 bebeğe (%85,7) stoma yapılırken (gastrostomi/ileostomi/kolostomi=3/18/3) sadece 4 bebekte stomasız onarım uygulanmıştır. Bebeklerden 9’u nekrotizan enterokolit, 2 bebek ise spontan intestinal perforasyon olarak değerlendirilmiş, 3 bebekte mekonyum ileusuna, 1 bebekte de Hirschsprung hastalığına bağlı perforasyon görülmüştür. Diğer bebeklerde prematürite dışında spesifik bir neden bulunamamıştır. Bebeklerden 5 (%17,8)’i operasyondan ortalama 10,4±9 (1-24) gün sonra kaybedilirken, yaşayan 23 bebek operasyondan ortalama 53,6±38,5 (10-167) gün sonra taburcu edilmiştir. İntestinal perforasyon nedeniyle stoma yapılan ve yaşayan 19 bebeğin stomaları ilk operasyondan ortalama 155,3±95,2 (23-283) gün sonra kapatılmıştır.
Sonuç: Hastalarımızın büyük çoğunluğunda stoma yapılmıştır ve mortalite oranı literatüre göre daha düşüktür. Her ne kadar stoma, hastaların operasyon sayısını artırsa da, ilk operasyondaki sürenin ve mortalite oranının düşmesine katkı sağlayabileceği için, primer anastomoz şansı olan hastalar da dahil olmak üzere, stomanın hala ilk seçenek olduğu düşünülebilir.
