Diagnostic challenges in incomplete duodenal obstruction: a case of fenestrated duodenal web and concomitant malrotation in a 7-month-old infant

Introduction

A fenestrated duodenal web is one of the rare congenital causes of duodenal obstruction; depending on the degree of obstruction, clinical findings may vary from the neonatal period to late infancy. The purpose of this report is to present a case of a fenestrated duodenal web with associated malrotation, diagnosed at seven months.

Case Presentation

A 7-month-old female patient, who was under follow-up at another center, presented to our clinic with complaints of vomiting that had persisted since birth. An ultrasound revealed dilation of the stomach and the first and second portions of the duodenum. An-upper-gastrointestinal-series showed findings consistent with a duodenal web. Doppler ultrasound revealed an inverted superior mesenteric vein/superior mesenteric artery relationship, a hepatic artery originating from the superior mesenteric artery, and suspected heterotaxy. During surgery, the Ladd procedure was performed to correct malrotation, and a duodenoduodenostomy was performed. The patient's follow-ups were uneventful.

Conclusions

In infants with persistent vomiting and partial duodenal obstruction, the presence of malrotation should be investigated first. Furthermore, it should be kept in mind that fenestrated duodenal webs with large apertures, which initially allow the passage of breast milk, may become symptomatic only after the introduction of thicker/solid foods.

İnkomplet duodenal obstrüksiyonun tanısal güçlüğü: yedi aylık bebekte duodenal web ve eşlik eden malrotasyon birlikteliği

Giriş:

Duodenal delikli web, duodenal obstrüksiyonun nadir konjenital nedenlerinden biri olup, obstrüksiyon derecesine bağlı olarak klinik bulgular yenidoğan döneminden ileri bebeklik dönemine kadar değişkenlik gösterebilir. Burada tanısı yedi aylıkken konulan duodenal delikli web ve eşlik eden malrotasyon birlikteliği olgusunun sunulması amaçlandı.

Olgu Sunumu:

Başka bir merkezde takipli yedi aylık kız hasta, doğumdan beri devam eden kusma yakınmasıyla kliniğimize başvurdu. Ultrasonografide mide ile duodenumun birinci ve ikinci kısımlarında dilatasyon saptandı. Özefagus-mide-duodenum grafisinde duodenal web ile uyumlu görünüm izlendi. Doppler ultrasonografide superior mezenterik venin superior mezenterik arterin anteriorunda yerleştiği, hepatik arterin superior mezenterik arterden köken aldığı ve heterotaksi açısından şüpheli olduğu bildirildi. Operasyonda malrotasyon için Ladd prosedürü uygulandı ve duodenoduodenostomi yapıldı. Hastanın postoperatif dönemi ve uzun dönem takipleri sorunsuz seyretti.

Sonuç:

Persistan kusma ile başvuran infantlar dikkatli bir şekilde değerlendirilmeli ve özellikle kısmi duodenal obstrüksiyon olgularında malrotasyon varlığı araştırılmalıdır. Duodenal atrezi genellikle yenidoğan döneminde tanı ve tedavisi yapılan bir konjenital anomali olmakla birlikte, özellikle anne sütünün geçişine imkân verecek genişlikteki duodenal delikli weblerin, bebek daha koyu kıvamlı gıdalarla beslenmeye başlayınca bulgu verebileceği de akılda tutulmalıdır.
