A Giant Mature Teratoma Arising from the Porta Hepatis Diagnosed Prenatally in a Neonate: A Rare Case Report

A Giant Mature Teratoma Arising from the Porta Hepatis Diagnosed Prenatally in a Neonate: A Rare Case Report

Introduction:

Congenital teratomas are rare germ cell tumors in the neonatal period. Although they most commonly arise in the sacrococcygeal region, abdominal teratomas are uncommon, and those originating from the porta hepatis are exceptionally rare. Due to their close relationship with the hepatobiliary structures, surgical management of these tumors may be technically challenging. We report a prenatally diagnosed giant mature teratoma arising from the porta hepatis in a neonate.

Case:

A female neonate with an intra-abdominal mass detected on prenatal ultrasonography underwent surgery on the 10th day of life. Intraoperative exploration revealed a 13-cm solid-cystic mass originating from the porta hepatis. Complete tumor excision was successfully performed. Because of the intimate relationship between the tumor and the biliary structures, preservation of the gallbladder was not feasible, and cholecystectomy was performed. An intraoperative common bile duct injury occurred during dissection and was repaired primarily. The postoperative course was uneventful, and the patient was discharged after establishing full oral feeding. Histopathological examination confirmed the diagnosis of a mature teratoma. Four weeks after surgery, the patient was readmitted with acholic stools, and biliary obstruction was suspected. Biliary reconstruction was planned; however, the family requested transfer to another tertiary referral center before further surgical intervention.

Conclusion:

Congenital mature teratomas arising from the porta hepatis are exceptionally rare and pose significant surgical challenges because of their close anatomical relationship with the hepatobiliary tract. This case highlights the importance of prenatal diagnosis, meticulous surgical planning, and careful postoperative surveillance for biliary complications in neonates with porta hepatis tumors.

Yenidoğan Döneminde İntrauterin Tanı Alan Porta Hepatis Kaynaklı Dev Matür Teratom: Nadir Bir Olgu Sunumu

Giriş:

Konjenital teratomlar yenidoğan döneminde nadir görülen germ hücreli tümörlerdir. En sık sakrokoksigeal bölgede izlenirken, abdominal yerleşimli olgular oldukça seyrektir. Porta hepatis kaynaklı teratomlar ise literatürde son derece az sayıda bildirilmiş olup, anatomik lokalizasyonları nedeniyle tanı ve cerrahi tedavide önemli güçlükler oluştururlar. Bu olguda, prenatal dönemde tanı alan, porta hepatisten köken alan dev matür teratomlu bir yenidoğan sunulmuştur.

Olgu:

Prenatal ultrasonografide batın içerisinde kitle saptanan kız bebek, postnatal değerlendirme sonrasında yaşamının 10. gününde opere edildi. Cerrahi eksplorasyonda yaklaşık 13 cm çapında, porta hepatisten köken aldığı görülen solid-kistik özellikte kitle total olarak eksize edildi. Tümörün hepatobilier yapılara yakın komşuluğu nedeniyle diseksiyon sırasında safra kesesinin korunması mümkün olmadığından kolesistektomi uygulandı. Cerrahi sırasında gelişen koledok yaralanması primer olarak onarıldı. Postoperatif dönemi sorunsuz seyreden hasta oral beslenmeye geçilerek taburcu edildi. Histopatolojik inceleme sonucu matür teratom olarak raporlandı. Ameliyattan yaklaşık dört hafta sonra akolik gaita yakınması ile tekrar başvuran hastada yapılan değerlendirmede biliyer obstrüksiyon düşünüldü ve biliyer rekonstrüksiyon planlandı. Ancak aile, tedavinin devamını kendi isteği ile başka bir merkezde sürdürmeyi tercih ettiğinden hasta merkezimizden ayrıldı.

Sonuç:

Porta hepatis kaynaklı konjenital matür teratomlar, yenidoğan döneminde son derece nadir görülen tümörler olup cerrahi tedavileri bölgenin kompleks anatomisi nedeniyle ciddi teknik zorluklar içermektedir. Özellikle hepatobilier yapıların tümörle yakın ilişkisi, rezeksiyon sırasında safra yolu komplikasyonları açısından önemli risk oluşturmaktadır. Prenatal tanı, cerrahi planlamaya önemli katkı sağlamakta; bu olgu ise porta hepatis yerleşimli teratomlarda multidisipliner yaklaşımın ve uzun dönem biliyer izlemin önemini vurgulamaktadır.
