The Value of the Postoperative Modified nSOFA Score for Predicting Mortality and Composite Morbidity in Neonatal Surgical Patients

Aim

The Neonatal Sequential Organ Failure Assessment (nSOFA), adapted from the SOFA score, is a dynamic organ dysfunction tool incorporating respiratory, cardiovascular, and hematologic components. Although it predicts mortality in neonatal sepsis and necrotizing enterocolitis, its prognostic value after neonatal surgery remains unclear. We compared the prognostic performance of the postoperative modified nSOFA score with biochemical markers for predicting mortality and composite adverse outcomes.

Methods

This retrospective single-center study included 55 neonates undergoing major surgery. Because FiO₂ data were unavailable, the respiratory component of nSOFA was modified, and 48-hour scores were unavailable for non-survivors. Preoperative, postoperative 24-hour, 48-hour, and maximum nSOFA scores were compared with CRP, lactate, procalcitonin, and albumin. Mortality, prolonged mechanical ventilation, reoperation, prolonged intensive care unit stay, and composite adverse outcomes were evaluated.

Results

Median gestational age was 34.4 weeks (IQR 28.5–37.0), postnatal age 4 days (IQR 1–13), and birth weight 2005 g (IQR 1200–2792.5); 60% were female. The most common diagnoses were necrotizing enterocolitis (n=18), intestinal atresia (n=16), and esophageal atresia/tracheoesophageal fistula (n=14). Mortality was 27.3% (15/55), with most deaths occurring in stage III necrotizing enterocolitis (10/15). Maximum nSOFA was significantly higher in non-survivors and showed a stronger association with mortality than biochemical markers. Higher nSOFA scores were also observed in patients requiring prolonged mechanical ventilation. CRP showed stronger associations with reoperation and composite morbidity. Composite adverse outcomes occurred in all infants weighing <1500 g (20/20) versus 80% of those weighing ≥1500 g (p=0.040).

Conslusion

Postoperative modified nSOFA predicted mortality and prolonged mechanical ventilation better than biochemical markers, whereas CRP provided complementary prognostic information for composite morbidity. As a simple bedside score, modified nSOFA has the potential to provide robust prognostic information beyond biochemical markers. Multicenter studies are needed to validate these findings.

Amaç

Neonatal Sekansiyel Organ Yetmezliği Değerlendirme (nSOFA) skoru, SOFA skorundan uyarlanmış; solunum, kardiyovasküler ve hematolojik bileşenlerden oluşan dinamik bir organ disfonksiyonu ölçeğidir. Sepsis ve nekrotizan enterokolitte mortaliteyi öngörmesine karşın, postoperatif neonatal cerrahi hastalarındaki prognostik değeri net değildir. Bu çalışmada postoperatif modifiye nSOFA skorunun mortalite ve kompozit kötü klinik sonlanımı öngörmedeki prognostik performansı biyokimyasal belirteçlerle karşılaştırıldı.

Gereç ve Yöntem

Majör cerrahi uygulanan 55 yenidoğanı içeren retrospektif, tek merkezli çalışma gerçekleştirildi. FiO₂ kayıtlarının bulunmaması nedeniyle nSOFA'nın solunum bileşeni modifiye edildi; eksitus olan hastalarda 48. saat skorları hesaplanamadı. Preoperatif, postoperatif 24. ve 48. saat ile maksimum nSOFA skorları CRP, laktat, prokalsitonin ve albümin ile karşılaştırıldı. Mortalite, uzamış mekanik ventilasyon, reoperasyon, uzamış yoğun bakım yatışı ve kompozit kötü klinik sonlanım değerlendirildi.

Bulgular

Medyan gestasyonel hafta 34,4 (IQR 28,5–37,0), medyan postnatal yaş 4 (IQR 1–13) gün ve medyan doğum ağırlığı 2005 (IQR 1200–2792,5) g idi; hastaların %60'ı kızdı. En sık tanılar nekrotizan enterokolit (n=18), intestinal atrezi (n=16) ve özofagus atrezisi/trakeoözofageal fistül (n=14) idi. Mortalite %27,3 (15/55) olup ölümlerin çoğu Evre III nekrotizan enterokolitte görüldü (10/15). Maksimum nSOFA skoru eksitus olan hastalarda anlamlı olarak daha yüksekti ve mortalite ile biyokimyasal belirteçlerden daha güçlü ilişki gösterdi. Uzamış mekanik ventilasyon gereksinimi olan hastalarda da nSOFA skorları daha yüksekti. CRP, reoperasyon ve kompozit morbidite ile daha güçlü ilişki gösterdi. Kompozit kötü klinik sonlanım, doğum ağırlığı <1500 g olan tüm bebeklerde (20/20), ≥1500 g olanların ise %80'inde görüldü (p=0,040).

Sonuç

Postoperatif modifiye nSOFA, neonatal cerrahi hastalarında mortalite ve uzamış mekanik ventilasyonu biyokimyasal belirteçlere göre daha iyi öngördü. CRP ise kompozit morbidite için tamamlayıcı prognostik bilgi sağladı. Basit ve yatak başında uygulanabilen bir skor olan modifiye nSOFA, biyokimyasal belirteçlerin ötesinde güçlü prognostik bilgi sağlama potansiyeline sahiptir. Bulguların doğrulanması için çok merkezli çalışmalara ihtiyaç vardır.
