Persistent Hydrosalpinx and Recurrent Isolated Tubal Torsion Following Perforated Appendicitis: A Case Report

Introduction: Hydrosalpinx is rare in children. It may develop following perforated appendicitis as a result of pelvic inflammation and adhesions. However, this complication is extremely uncommon and has mainly been described in isolated case reports. We present a patient who underwent elective salpingectomy for persistent hydrosalpinx associated with recurrent isolated tubal torsion.
Case Presentation: A 14-year-old girl with intermittent pelvic pain for the previous two years was admitted for elective surgery because of persistent right hydrosalpinx. She had undergone surgery for perforated appendicitis with generalized peritonitis seven years earlier and two separate laparoscopic detorsion procedures for isolated right fallopian tube torsion 16 and 8 months before the current admission. During the first procedure, a hydrosalpinx twisted through two complete turns was identified. Following detorsion, perfusion improved in the proximal portion of the tube, whereas distal circulatory impairment persisted. During the second procedure, a poorly perfused hydrosalpinx twisted through three complete turns was observed. The tube was preserved because perfusion improved after detorsion. The ipsilateral ovary appeared normal during both procedures. Doppler ultrasonography demonstrated a persistent right hydrosalpinx measuring 37×25 mm and approximately 5.5 cm in length. Magnetic resonance imaging revealed a cystic tubular structure in the right pelvis, measuring 14 mm proximally and 45 mm distally. Laparoscopic total right salpingectomy was performed. Histopathological examination was consistent with hydrosalpinx. The patient was discharged on the first postoperative day and remained asymptomatic at the three-month follow-up.
Conclusion: Hydrosalpinx following perforated appendicitis is a recognized but extremely rare complication in children. As demonstrated in this case, it may lead to recurrent isolated tubal torsion and chronic symptoms over the long term. Although tubal preservation is appropriate during isolated torsion whenever viability can be restored, recovery of tubal function is unlikely in the presence of persistent hydrosalpinx. Furthermore, the adverse effects of the inflammatory microenvironment created by hydrosalpinx fluid on embryo implantation and fertility are well established in the adult literature. Therefore, in patients with recurrent torsion, persistent tubal dilatation, and ongoing symptoms, elective salpingectomy may be considered an appropriate treatment option, taking future fertility into account.
Perfore Apandisit Sonrası Gelişen Persistan Hidrosalpenks ve Tekrarlayan İzole Tubal Torsiyon: Olgu Sunumu

Giriş: Çocuklarda hidrosalpenks nadirdir. Gelişen pelvik enflamasyon ve adezyonlar nedeniyle perfore apandisit sonrası ortaya çıkabilir. Ancak bu durum son derece seyrek görülür ve literatürde çoğunlukla olgu sunumları düzeyinde bildirilmiştir. Rekürren izole tubal torsiyona yol açan persistan hidrosalpenks nedeniyle elektif salpenjektomi uygulanan bir olgu sunulmuştur.
Olgu Sunumu: Son iki yıldır aralıklı pelvik ağrı yakınması olan 14 yaşında kız hasta, persistan sağ hidrosalpenks nedeniyle elektif cerrahi amacıyla yatırıldı. Başvurudan 7 yıl önce perfore apandisit ve generalize peritonit nedeniyle ameliyat edildiği, ayrıca, 16 ve 8 ay önce izole sağ fallop tüpü torsiyonu nedeniyle iki ayrı laparoskopik detorsiyon yapıldığı öğrenildi. İlk girişimde iki tam tur torsiyone hidrosalpenks saptanmış, detorsiyon sonrası proksimal tubal perfüzyon düzelirken distal dolaşım bozukluğu devam etmişti. İkinci girişimde üç tam tur torsiyone, dolaşımı bozulmuş hidrosalpenks izlenmiş, detorsiyon sonrasında kanlanmanın düzelmesi üzerine tuba korunmuştu. Her iki girişimde de over doğal görünümdeydi. Doppler ultrasonografide sağ tubal alanda 37×25 mm boyutlarında ve yaklaşık 5,5 cm uzunluğunda persistan hidrosalpenks; MRG’de sağ pelvik yerleşimde proksimalde 14 mm distalde 45 mm çaplı kistik, tübüler yapı saptandı. Laparoskopik sağ total salpenjektomi uygulandı. Histopatolojik inceleme hidrosalpenks ile uyumluydu. Postoperatif birinci günde taburcu edildi. Üçüncü ay kontrolünde sorunsuzdur.
Sonuç: Çocuklarda perfore apandisit sonrası hidrosalpenks gelişimi bilinen ancak son derece nadir görülen bir komplikasyondur. Bu hastada olduğu gibi, uzun dönemde rekürren izole tubal torsiyon ve kronik semptomlara yol açabilir. İzole torsiyon sırasında tüpün korunmaya çalışılması uygun bir yaklaşım olsa da, persistan hidrosalpenks varlığında tübal fonksiyonun geri kazanılma olasılığı düşüktür. Ayrıca hidrosalpenks sıvısının oluşturduğu inflamatuvar mikroçevrenin embriyo implantasyonu ve fertilite üzerinde olumsuz etkileri erişkin literatüründe iyi tanımlanmıştır. Bu nedenle rekürren torsiyon, kalıcı tubal dilatasyon ve semptom varlığında, gelecekteki fertilite de göz önünde bulundurularak elektif salpenjektomi uygun bir tedavi seçeneği olarak değerlendirilmelidir.
