Minimally invasive versus open gastric pull-up for esophageal replacement in children: A retrospective comparative cohort study

Purpose:
Among esophageal replacement techniques, gastric pull-up (GPU) is considered the most suitable for minimally invasive surgery (MIS). Although MIS-GPU has been increasingly adopted, direct comparisons with the conventional open approach remain limited. This study aimed to compare early perioperative and postoperative outcomes between these two techniques in pediatric patients.
Methods:
A retrospective review was conducted of cases that underwent GPU in our clinic between Nov 2012 and Feb 2025. The patients were divided into two groups according to the surgical method. Perioperative and early postoperative parameters were compared between the two groups. Statistical analyses were performed using IBM SPSS Statistics version 27, with propensity score adjustment and regression analyses.
Results:
The study included 75 patients (laparotomy:54; MIS:21). The median age at operation was 23 (8-185) months. The indications for the GPU were esophageal atresia (53) and corrosive esophageal stricture (22). Of the MIS procedures, 6 were performed robotically and 15 laparoscopically. Perioperative inotropic support (p 0.016) and blood transfusion requirements (p 0.003) were higher in the laparotomy group. Operative time was longer in the MIS group (p 0.008); however, this group was also characterized by earlier extubation (p 0.008), shorter ICU stays (p 0.004), and earlier initiation of enteral feeding (p 0.001). Rates of postoperative early systemic complications (SIRS, sepsis, hypertension, postoperative inotropic support), surgical site infection, and reoperation were comparable between groups. Anastomotic leak and anastomotic stricture rates were comparable between the two groups. (p>0.05) No mortality occurred.
Conclusion:
MIS-GPU provides functional outcomes comparable to the open approach, with similar rates of anastomotic leak and stricture. However, it is associated with faster and smoother postoperative recovery. In experienced, high-volume centers, MIS should be considered the preferred approach for pediatric patients requiring GPU.
Çocuklarda özofagus replasmanında minimal invaziv ve açık gastrik pull-up sonuçlarının karşılaştırılması; retrospektif kohort çalışması

Amaç:
Özofagus replasman yöntemleri arasında gastrik pull-up (GPU), minimal invaziv cerrahiye (MIS) en uygun yöntem olarak kabul edilmektedir. MIS-GPU uygulamaları giderek yaygınlaşmakla birlikte, MIS ve açık cerrahiyi doğrudan karşılaştıran çalışmalar sınırlıdır. Bu çalışmada, çocuk hastalarda MIS ve açık GPU uygulanmış hastaların perioperatif ve postoperatif sonuçlarının karşılaştırılması amaçlandı.
Yöntem:
Kasım 2012-Şubat 2025 tarihleri arasında kliniğimizde GPU uygulanan hastalar retrospektif olarak incelendi. Hastalar uygulanan cerrahi yönteme göre iki gruba ayrıldı. Gruplar arasında perioperatif ve erken postoperatif parametreler karşılaştırıldı. İstatistiksel analizler, propensity score düzeltmesi ve regresyon analizleri IBM SPSS Statistics sürüm 27 kullanılarak gerçekleştirildi.
Bulgular:
Çalışmaya toplam 75 hasta dahil edildi (laparotomi:54, MIS:21). Ameliyat sırasındaki ortanca yaş 23 (8–185) aydı. GPU endikasyonları: özofagus atrezisi (53) ve korozif özofagus darlığı (22) idi. Minimal invaziv girişimlerin 6’sı robotik, 15’i laparoskopik olarak gerçekleştirildi. Perioperatif inotrop destek gereksinimi (p=0,016) ve kan transfüzyonu ihtiyacı (p=0,003) laparotomi grubunda daha yüksekti. Ameliyat süresi minimal invaziv cerrahi grubunda daha uzun olmakla birlikte (p=0,008), bu grupta ekstübasyon daha erken (p=0,008), yoğun bakım yatış süresi daha kısa (p=0,004) ve enteral beslenmeye başlama zamanı daha erkendi (p=0,001). Erken postoperatif sistemik komplikasyonlar (SIRS, sepsis, hipertansiyon ve postoperatif inotrop destek gereksinimi), cerrahi alan enfeksiyonu ve yeniden ameliyat gereksinimi oranları gruplar arasında benzerdi. Anastomoz kaçağı ve anastomoz darlığı oranları da iki grup arasında benzer bulundu (p>0,05). Perioperatif ya da postoperatif mortalite görülmedi.
Sonuç:
MIS-GPU, anastomoz kaçağı ve darlığı açısından açık cerrahi ile benzer sonuçlar sağlarken daha hızlı ve rahat bir postoperatif iyileşme süreci ile ilişkilidir. Deneyimli ve yüksek hasta hacmine sahip merkezlerde, GPU gerektiren çocuk hastalarda minimal invaziv yaklaşım öncelikli cerrahi yöntem olarak tercih edilmelidir.
