Unexpected Histopathological Findings in Pediatric Cholecystectomy Specimens

Aim: Pediatric cholelithiasis has become increasingly common in recent years, and chronic cholecystitis is the predominant histopathological finding in most cholecystectomy specimens. Nevertheless, occasional metaplastic, polypoid, and even neoplastic lesions may be encountered. This study aimed to describe the histopathological spectrum of pediatric cholecystectomy specimens and to draw attention to uncommon findings identified during routine pathological evaluation.

Methods: A retrospective review was conducted of all pediatric patients who underwent cholecystectomy at our institution between 2022 and 2026. Demographic data, presenting symptoms, radiological findings, surgical outcomes, and histopathological results were analyzed. Histopathological diagnoses were grouped as chronic inflammatory, metaplastic, polypoid, neoplastic, or other pathological lesions.

Results: A total of 90 children underwent cholecystectomy during the study period. The median age was 15 years (range, 1–17.9 years), and 57 patients (63%) were female. Cholelithiasis was the primary indication for surgery in 86 patients (96%), whereas gallbladder polyps were identified in four patients (4%). Abdominal pain was the most common presenting complaint (81%), and multiple gallstones were detected in 63 patients (70%).

Histopathological examination revealed chronic cholecystitis in 87 patients (96.7%), making it the predominant pathological diagnosis. Two specimens demonstrated additional histopathological alterations, including pyloric metaplasia (n=1) and pseudopyloric gland metaplasia (n=1). Among the four patients who underwent surgery for gallbladder polyps, two showed chronic cholecystitis on histological examination; one of these also exhibited adenomatous hyperplasia and gastric foveolar metaplasia. Of the remaining two patients, one was diagnosed with a cholesterol polyp and the other with an intracholecystic papillary neoplasm.

Conclusions: Although most pediatric cholecystectomy specimens demonstrated chronic cholecystitis, rare metaplastic and neoplastic lesions were occasionally encountered. Routine histopathological evaluation may reveal unexpected pathology that would otherwise remain unrecognized, reinforcing its importance in the assessment of pediatric gallbladder specimens.

Pediatrik Kolesistektomi Örneklerinde Beklenmedik Histopatolojik Bulgular

Amaç: Pediatrik uygulamada kolelityazis sıklığı gün geçtikçe artmaktadır ve çoğu kolesistektomi spesmeninde beklenen patolojik bulgu kronik kolesistittir. Bununla birlikte, nadir görülen metaplastik, polipoid ve neoplastik lezyonlar da zaman zaman tespit edilebilir. Bu çalışma, pediatrik kolesistektomi örneklerinin histopatolojik spektrumunu karakterize etmeyi ve rutin patolojik incelemede saptanan beklenmedik bulguları vurgulamayı amaçlamıştır.

Yöntem: 2022 ve 2026 yılları arasında bölümümüzde kolesistektomi yapılmış olan çocuk hastalar retrospektif olarak incelenmiştir. Demografik özellikler, klinik tablo, görüntüleme bulguları, ameliyat sonuçları ve histopatolojik tanılar analiz edilmiştir. Histopatolojik bulgular kronik inflamatuar, metaplastik, polipoid, neoplastik veya diğer lezyonlar olarak sınıflandırılmıştır.

Bulgular: Çalışma döneminde toplam 90 çocuğa kolesistektomi uygulandı. Ortanca yaş 15 yıl (1-17,9) olup, 57 hasta (%63) kızdı. Cerrahi endikasyon 86 hastada (%96) safra kesesi taşı iken, dört vakada (%4) safra kesesi polipleri mevcuttu. Karın ağrısı en sık görülen başvuru semptomu (%81) olup, 63 hastada (%70) çoklu safra kesesi taşı tespit edildi.

Histopatolojik inceleme, 87 hastada (%96,7) kronik kolesistit olduğunu ortaya koyarak, bu durumu en yaygın patolojik tanı haline getirdi. İki örnekte, pilorik metaplazi (n=1) ve psödo-pilorik bez metaplazisi (n=1) dahil olmak üzere ek histopatolojik değişiklikler görüldü. Safra kesesi polipleri nedeniyle ameliyat geçiren dört hastadan ikisinde histolojik incelemede kronik kolesistit saptandı; bunlardan birinde ayrıca adenomatöz hiperplazi ve gastrik foveolar metaplazi de görüldü. Geri kalan iki hastadan birine kolesterol polipi, diğerine ise intrakolesistik papiller neoplazm tanısı kondu.

Sonuç: Pediatrik kolesistektomi örneklerinin çoğunda kronik kolesistit saptanmasına rağmen, nadir de olsa metaplastik ve neoplastik lezyonlara rastlanmıştır. Rutin histopatolojik değerlendirme, aksi takdirde fark edilmeyecek beklenmedik patolojileri ortaya çıkarabilir ve bu da pediatrik safra kesesi örneklerinin değerlendirilmesinin önemini pekiştirir.
