Post-Pull-Through Enterocolitis in Hirschsprung Disease: Association With Extended Aganglionosis And Obstructive Complications

AIM
Hirschsprung-associated enterocolitis remains an important cause of morbidity after pull-through surgery. This study evaluated whether postoperative enterocolitis was associated with aganglionic segment extent, obstructive postoperative complications and surgical technique.
METHODS
Children who underwent definitive or redo surgery for Hirschsprung disease between 2006 and 2025 were retrospectively reviewed. Postoperative enterocolitis, segment extent, obstructive complications and operative pathway were coded from operative and follow-up records. Comparisons were performed using chi-square, Fisher’s exact or Fisher-Freeman-Halton exact tests, as appropriate. Mean follow-up was 36 months.
RESULTS
A total of 180 patients were included. Postoperative enterocolitis was documented in 30 patients (16.7%). Segment extent was classified as short-segment disease in 147 patients (81.7%), long-segment disease in 10 (5.6%) and total colonic/extended disease in 23 (12.8%). Enterocolitis occurred in 14/147 patients (9.5%) with short-segment disease, 4/10 patients (40.0%) with long-segment disease and 12/23 patients (52.2%) with total colonic/extended disease, showing the highest rate in the total colonic/extended group (p<0.001). Obstructive postoperative complications were identified in 27 patients (15.0%). Enterocolitis was present in 8/27 patients (29.6%) with obstructive complications and in 22/153 patients (14.4%) without documented obstruction (p=0.088). According to surgical pathway, enterocolitis occurred after transanal endorectal pull-through in 11/90 patients (12.2%), Duhamel procedure in 11/57 (19.3%), redo pull-through in 6/22 (27.3%), Swenson procedure in 2/5 (40.0%) and other operative pathways in 0/6 (0%) (p=0.148).
AMA
In this 20-year single-center cohort, postoperative enterocolitis was significantly associated with segment extent and was most frequent in patients with total colonic/extended Hirschsprung disease. Enterocolitis was also numerically more common in patients with obstructive postoperative complications. Children presenting with enterocolitis after pull-through should be systematically evaluated for disease extent, residual obstruction and anatomical failure.

Amaç
Hirschsprung hastalığı ile ilişkili enterokolit, pull-through cerrahisi sonrasında önemli bir morbidite nedeni olmaya devam etmektedir. Bu çalışmada, postoperatif enterokolit gelişiminin agangliyonik segmentin yaygınlığı, postoperatif obstrüktif komplikasyonlar ve uygulanan cerrahi teknikle ilişkisinin değerlendirilmesi amaçlandı.
Yöntem
2006–2025 yılları arasında Hirschsprung hastalığı nedeniyle definitif veya redo cerrahi uygulanan çocuk hastalar retrospektif olarak incelendi. Postoperatif enterokolit varlığı, agangliyonik segmentin yaygınlığı, obstrüktif komplikasyonlar ve uygulanan cerrahi yöntemler ameliyat ve takip kayıtları üzerinden değerlendirildi. Karşılaştırmalarda uygunluğuna göre ki-kare, Fisher kesinlik veya Fisher–Freeman–Halton kesinlik testleri kullanıldı. Ortalama takip süresi 36 aydı.
Bulgular
Çalışmaya toplam 180 hasta dâhil edildi. Postoperatif enterokolit 30 hastada (%16,7) saptandı. Hastalık yaygınlığına göre 147 hastada (%81,7) kısa segment Hirschsprung hastalığı, 10 hastada (%5,6) uzun segment Hirschsprung hastalığı ve 23 hastada (%12,8) total kolonik/uzamış segment hastalığı mevcuttu. Enterokolit; kısa segment hastalığı bulunan 147 hastanın 14’ünde (%9,5), uzun segment hastalığı bulunan 10 hastanın 4’ünde (%40,0) ve total kolonik/uzamış segment hastalığı bulunan 23 hastanın 12’sinde (%52,2) gelişti. Enterokolit oranı total kolonik/uzamış segment grubunda anlamlı olarak daha yüksekti (p<0,001).
Postoperatif obstrüktif komplikasyonlar 27 hastada (%15,0) saptandı. Enterokolit, obstrüktif komplikasyonu bulunan 27 hastanın 8’inde (%29,6), belgelenmiş obstrüksiyonu bulunmayan 153 hastanın ise 22’sinde (%14,4) görüldü (p=0,088).
Uygulanan cerrahi yönteme göre enterokolit; transanal endorektal pull-through uygulanan 90 hastanın 11’inde (%12,2), Duhamel prosedürü uygulanan 57 hastanın 11’inde (%19,3), redo pull-through uygulanan 22 hastanın 6’sında (%27,3), Swenson prosedürü uygulanan 5 hastanın 2’sinde (%40,0) ve diğer cerrahi yöntemlerin uygulandığı 6 hastanın hiçbirinde (%0) saptanmadı (p=0,148).
Sonuç
Bu 20 yıllık tek merkezli kohortta, postoperatif enterokolitin agangliyonik segmentin yaygınlığıyla anlamlı düzeyde ilişkili olduğu ve en yüksek oranda total kolonik/uzamış segment Hirschsprung hastalığı bulunan hastalarda görüldüğü saptandı. Enterokolit, postoperatif obstrüktif komplikasyonu bulunan hastalarda da sayısal olarak daha sık görülmekle birlikte bu farklılık istatistiksel anlamlılığa ulaşmadı. Pull-through cerrahisi sonrasında enterokolit gelişen çocuklar, hastalığın yaygınlığı, rezidüel obstrüksiyon ve anatomik başarısızlık açısından sistematik olarak değerlendirilmelidir.
