Preoperative biliary ultrasonography before laparoscopic cholecystectomy: May it guide the reduction of complications?

Aim: To evaluate the effect of biliary system ultrasonography (US) performed before gallbladder surgery on complication rates.
Methods: Patients who underwent laparoscopic cholecystectomy between 2021 and 2025 were retrospectively evaluated. All patients had undergone biliary system US within the last 24 hours before surgery. Gallbladder dimensions, wall thickness, presence/localization/size of stones, presence of pericholecystic fluid, and extrahepatic/intrahepatic bile duct diameter were assessed. Surgical planning and the perioperative process were evaluated.
Results: A total of 94 patients (median age 14) were included in the study. Surgery for choledocholithiasis (n=18), acute cholecystitis (n=32), or acute pancreatitis (n=6) was scheduled at a median of 6 weeks later. Surgery was postponed in 8 patients due to acute cholecystitis and in 2 patients due to choledochal dilation and stones. Follow-up US was performed at a median of 6 weeks, and surgery was rescheduled accordingly. On US, stones at the gallbladder neck were detected in 8 patients, gallbladder hydrops in 4 patients, Mirizzi syndrome in 6 patients, and chronic cholecystitis in 2 patients. In these patients, intraoperative ICG or contrast cholangiography was performed. All patients underwent laparoscopic cholecystectomy in accordance with the principles of safe cholecystectomy, and no intraoperative or postoperative complications were observed.
Conclusion: Preoperative evaluation of the gallbladder and biliary system with US has a significant impact on determining the surgical strategy. Depending on US findings, postponing surgery, planning additional interventions (ERCP, etc.), or using intraoperative imaging methods (ICG, cholangiography, etc.) may prevent potential biliary complications. It may also assist in patient selection at centers without access to intraoperative imaging.

Amaç:
Safra kesesi cerrahisi öncesinde uygulanan biliyer sistem ultrasonografisinin (US), komplikasyon oranları üzerindeki etkisini değerlendirmek.
Yöntem:
2021-2025 yılları arasında laparoskopik kolesistektomi uygulanan hastalar retrospektif olarak değerlendirildi. Tüm hastalara ameliyattan önceki son 24 saat içinde biliyer sistem US yapılmıştı. Safra kesesi boyutları, duvar kalınlığı, taş varlığı-lokalizasyonu ve boyutu, safra kesesi çevresinde sıvı varlığı, ekstra/intrahepatik safra yolu çapı değerlendirildi. Cerrahi planlama ve perioperatif süreç değerlendirildi.
Bulgular:
Çalışmaya toplam 94 hasta (medyan yaş 14) dahil edildi. Koledokolitiazis (n=18), akut kolesistit (n=32) veya akut pankreatit (n=6) nedeniyle cerrahi, medyan 6 hafta sonra planlandı. 8 hastada akut kolesistit, 2 hastada koledokal dilatasyon ve taş nedeniyle cerrahi ertelendi. Medyan 6 hafta sonra kontrol US yapılarak cerrahi yeniden planlandı. US'de 8 hastada safra kesesi boynunda taş, 4 hastada safra kesesi hidropsu, 6 hastada Mirizzi sendromu ve 2 hastada kronik kolesistit saptandı. Bu hastalarda intraoperatif ICG veya kontrastlı kolanjiografi uygulandı. Tüm hastalara güvenli kolesistektomi ilkelerine uygun laparoskopik kolesistektomi uygulandı ve intraoperatif veya postoperatif komplikasyon gözlenmedi.
Sonuç:
Safra kesesi ve biliyer sistemin preoperatif US ile değerlendirilmesi, cerrahi stratejinin belirlenmesinde önemli etkiye sahiptir. US bulgularına bağlı olarak cerrahinin ertelenmesi, ek girişimlerin (ERCP vb.) planlanması veya intraoperatif görüntüleme yöntemlerinin (ICG, kolanjiografi vb.) kullanılması olası biliyer komplikasyonları önleyebilir. İntraoperatif görüntüleme imkanı olmayan merkezlerde hasta seçimi için yardımcı olabilir.
