A Case Report of an Atypically Located Intra-abdominal Testis

Objective:
In cases of intra-abdominal testes, extension of the vas deferens into the inguinal canal does not necessarily indicate that the testis is located in the same region. This case highlights that exploration based solely on the course of the vas deferens may result in incomplete surgical exploration.

Case Presentation:
A 7-year-old boy was referred to our outpatient clinic with bilateral undescended testes. On physical examination, the left testis was non-palpable, whereas the right testis was palpable within the inguinal canal. The penis was normal, and no additional abnormalities were identified. Laparoscopic exploration was planned.

During laparoscopy, the vas deferens and testicular vessels on the right side were observed entering the inguinal canal. On the left side, only the vas deferens extended into the inguinal canal, while the testicular vessels could not be identified. Consequently, a meticulous search for the left testis was undertaken. Following mobilization of the sigmoid colon, the left testis was identified extending toward the pelvis from the point where the left ureter crossed the external iliac vessels, corresponding to Stage V of the Eckstein classification. The descending colon, sigmoid colon, and the retroperitoneal attachments, including the hepatic flexure, were mobilized. The left testicular vessels were carefully dissected up to the level of the renal vein while preserving the ureteral blood supply. After adequate mobilization, the testis was brought to the scrotum through a route medial to the medial umbilical ligament and fixed within a subdartos pouch. Exploration of the contralateral testis was performed via an inguinal approach six weeks later, and the testis was placed in the scrotum without tension. At the one-month postoperative follow-up, both testes were located within the scrotum and demonstrated comparable sizes.

Conclusion:
In patients with intra-abdominal testes, exploration based solely on the course of the vas deferens may be insufficient and can lead to incomplete surgery. Particularly when testicular vessels cannot be identified, the possibility of an ectopic or unusually located testis should be considered, and a thorough intra-abdominal exploration should be performed.

Atipik Yerleşimli İntraabdominal Testis Olgu Sunumu

Amaç: İntraabdominal yerleşimli testis olgularında vaz deferensin inguinal kanala uzanması, testisin de aynı lokalizasyonda bulunacağını her zaman göstermemektedir. Bu olgu ile vaz deferense dayalı eksplorasyonun eksik cerrahi ile sonuçlanabileceği vurgulanması amaçlandı.

Olgu Sunumu: Yedi yaşında erkek hasta, bilateral inmemiş testis nedeniyle polikliniğimize başvurdu. Fizik muayenede Sol testis nonpalpable, sağ testis inguinalde palpable, penis normal, ek bulgu yoktu. Hastaya laparoskopik eksplorasyon planlandı. Laparoskopide sağ tarafta vaz deferens ve testiküler damarların inguinal kanala girdiği, sol tarafta ise yalnızca vaz deferensin inguinal kanala uzandığı ve testiküler damarların izlenemediği görüldü. Bunun üzerine sol testis ayrıntılı olarak araştırıldı. Sigmoid kolonun mobilizasyonunu takiben, testisin sol üreterin eksternal iliak damarları çaprazladığı yerden pelvise doğru uzandığı saptandı (Eckstein klasifikasyonu evre V). Sol desandan kolon, sigmoid kolon ve hepatik fleksura dahil retroperiteniumdan ayrıldı. Sol testis damarları renal vene kadar dikkatlice diseke edildi. Üreter vaskülerleri korundu. Yeterli mobilizasyon sağlandıktan sonra testis, medial umbilikal ligamanın medialinden geçirilerek skrotuma oluşturulan poşa indirildi. Kontralateral testise yönelik eksplorasyon altı hafta sonra inguinal yaklaşımla gerçekleştirildi ve testis gerginlik oluşturmayacak şekilde skrotuma yerleştirildi. Postoperatif birinci ay kontrolünde her iki testisin de skrotumda olduğu ve testis boyutlarının birbirine benzer olduğu gözlendi.

Sonuç: İntraabdominal testis olgularında yalnızca vaz deferensin seyrine dayanılarak yapılan eksplorasyon yetersiz kalabilir ve eksik cerrahiye neden olabilir. Özellikle testiküler damarların izlenemediği durumlarda, testisin ektopik veya alışılmadık lokalizasyonlarda bulunabileceği akılda tutulmalı ve ayrıntılı intraabdominal eksplorasyon yapılmalıdır.
