Surgical management of rectal injuries in children: a series of four cases

Aim: Rectal injuries in children are rare and usually result from penetrating trauma. Most are extraperitoneal. Owing to the scarcity of cases in the literature, there is no consensus on management. In this study, we aimed to present the management of patients who underwent surgical treatment for rectal injury.
Methods: The records of patients operated on for rectal injury at our clinic between 2016 and 2026 were reviewed retrospectively. The site of injury, whether it extended to the peritoneum, the repair method, and the use of a diverting colostomy were evaluated.
Results: A total of four patients were operated on during this period. The first was a 4-year-old girl with blunt trauma from a traffic accident, with injury to the extraperitoneal rectum and the vagina and a left femoral fracture; repair and sigmoid colostomy were performed, and reconstruction was later carried out because of a patulous anus. The second, a 15-year-old boy, sustained a gunshot wound in which the bullet entered through the inguinal region, shattered the extraperitoneal rectum, and exited through the anus; after repair and sigmoid colostomy, the colostomy was closed during follow-up with no continence problem. The third, a 13-year-old boy, perforated the rectal wall with a stick inserted into the rectum, causing an intraperitoneal injury extending to the peritoneum, with omentum prolapsing through the anus; laparoscopic rectal repair and sigmoid colostomy were performed. The fourth, a 12-year-old, sustained a brush handle impaling the perineum after a fall, injuring the extraperitoneal rectum; repair without colostomy was performed, with no continence problem on follow-up. In the three patients with acute injuries involving the sphincter muscles, a muscle stimulator was used during repair.
Conclusion: Because rectal injuries are very rare, there is no standard treatment approach in children. Treatment should be planned according to the degree of injury and the clinical condition. In injuries involving the sphincter muscles, early intervention and the use of a muscle stimulator during the procedure may help prevent the risk of incontinence. The decision for colostomy can be made according to the degree of injury and the clinical condition of the patient.

Amaç: Çocuklarda rektum yaralanmaları oldukça nadir görülen ve genellikle penetran yaralanmalara bağlı olarak gelişmektedir. Rektum yaralanmalarının çoğunluğu ekstraperitonealdir. Literatürdeki vaka azlığı sebebiyle yönetimi konusunda farklı görüşler mevcuttur. Bu çalışmada amacımız rektum yaralanması sebebiyle cerrahi tedavi uyguladığımız hastaların yönetimini ortaya koymaktır.
Yöntem: Rektum yaralanması sebebiyle 2016-2026 yılları arasında kliniğimizde opere edilen hastaların verileri retrospektif olarak incelenmiştir. Yaralanmanın yeri ve peritona uzanıp uzanmadığı, onarım yöntemi ve koruyucu kolostomi uygulanması değerlendirildi.
Bulgular: Belirtilen sürede toplam dört hasta ameliyat edildi. İlk hasta trafik kazasına bağlı künt yaralanması olan dört yaşında bir kızdı; rektumun ekstraperitoneal kısmında ve vajinada yaralanma ile sol femur kırığı mevcuttu. Onarım ve sigmoid kolostomi uygulandı; takibinde geniş anüs nedeniyle rekonstrüksiyon yapıldı. On beş yaşında bir erkek olan ikinci hastada inguinal bölgeden giren kurşun, rektumun ekstraperitoneal kısmını parçalayarak anüsten çıkmıştı; onarım ve sigmoid kolostomi sonrası takipte kolostomi kapatıldı ve kontinans problemi olmadı. On üç yaşında bir erkek olan üçüncü hasta, rektumundan soktuğu sopa ile rektum duvarını delerek peritona uzanan intraperitoneal bir yaralanmaya sebep olmuştu ve omentumu anüsten dışarı prolabeydi; laparoskopik rektum onarımı ve sigmoid kolostomi yapıldı. On iki yaşında olan dördüncü hastada düşme sonucu perineye fırça sapı batmış ve rektumun ekstraperitoneal kısmı yaralanmıştı; bu hastaya kolostomisiz onarım uygulandı ve takibinde kontinans problemi olmadı. Akut dönemde sfinkter kaslarını içeren yaralanması olan üç hastada onarım esnasında kas stimülatörü kullanıldı.
Sonuç: Rektum yaralanmaları çok nadir görüldüğünden çocuklarda standart bir tedavi yaklaşımı yoktur. Tedavi, hastanın yaralanma derecesi ve klinik durumuna göre planlanmalıdır. Sfinkter kaslarını içeren yaralanmalarda erken dönemde müdahale ve işlem esnasında kas stimülatörünün kullanılması kontinans riskini önlemek için faydalı olabilir. Kolostomi kararı yaralanmanın derecesine ve hastanın klinik durumuna göre alınabilir.
