The Value of the Preoperative Cyst-to-Ovary Ratio in Predicting Ovarian-Sparing Surgery in Pediatric Benign Ovarian Masses

Aim: Ovarian-sparing surgery (OSS) has become the standard approach for managing ovarian masses in children and adolescents. However, in large, particularly centrally located cysts, normal ovarian tissue may not be macroscopically identifiable, often leading surgeons to perform oophorectomy. This study aimed to evaluate the value of the preoperative cyst-to-ovary ratio in predicting the feasibility of ovarian preservation.
Methods: Fifty-one girls (0–18 years) who underwent surgery for ovarian masses between 2020 and 2025 were retrospectively reviewed. On preoperative magnetic resonance imaging (MRI), the craniocaudal (CC) and anteroposterior (AP) diameters of the cyst and residual ovarian tissue were measured, and cyst-to-ovary ratios were calculated. Patients were divided into ovarian-sparing surgery and oophorectomy groups. The predictive performance of the preoperative CC and AP cyst-to-ovary ratios for ovarian preservation was evaluated using receiver operating characteristic (ROC) curve analysis. A p-value <0.05 was considered statistically significant.
Results: Ovarian tissue was preserved in 39 patients (76.4%), whereas 12 (23.6%) underwent oophorectomy. The preoperative CC cyst-to-ovary ratio significantly predicted ovarian preservation (AUC=0.757, 95% CI: 0.595–0.920; p=0.007). At a cut-off value of 0.855, sensitivity and specificity were 74.4% and 25.0%, respectively. The AP cyst-to-ovary ratio was also a significant predictor (AUC=0.741, 95% CI: 0.581–0.902; p=0.012). Using a cut-off value of 0.850, sensitivity was 69.2% and specificity was 33.3%.
Conclusions: Preoperative cyst-to-ovary ratio appears to be a valuable parameter for predicting the feasibility of ovarian preservation in children with ovarian masses. Based on our findings, cyst-to-ovary dimensions measured in the CC and AP planes that fall below the established cut-off values may contribute to the long-term preservation of ovarian reserve and future fertility potential. Therefore, ovarian-sparing surgery should be preferentially considered in patients with ratios below these thresholds to optimize long-term reproductive outcomes.
Pediatrik Over Kitlelerinde Over Korunumu ve Over Rejenerasyonunun Öngörülmesinde Preoperatif Kist/Over Oranının Değeri

Amaç: Çocukluk ve adölesan dönemde görülen over kitlelerinin tedavisinde over koruyucu cerrahi (OKC) güncel yaklaşımın temelini oluşturmaktadır. Büyük ve özellikle santral yerleşimli over kistlerinde, makroskopik olarak belirgin over dokusu seçilemeyebileceğinden, cerrahlar bu olgularda ooferektomiyi tercih edebilmektedir. Bu çalışmada, preoperatif kist/over oranının, normal over dokusunun korunabilmesi ve postoperatif fonksiyonel over dokusunun miktarını öngörmedeki değerinin araştırılması amaçlandı.
Yöntem: 2020–2025 yıllarında over kitlesi nedeniyle opere edilen 0–18 yaş arası 51 kız olgu retrospektif değerlendirildi. Preoperatif Magnetik Rezonans (MR) görüntülerinden kraniokaudal (KK) ve anteroposterior (AP) over dokusu ve kist çapları ayrı ayrı ölçülerek, kist/over oranları hesaplandı. Hastalar OKC ve ooferektomi yapılanlar olarak 2 gruba ayrıldı. Preoperatif KK ve AP kist/parankim boyut oranlarının, postoperatif kalacak olan normal over dokusu hacminin öngörülmesindeki değeri analiz edildi. Bunun için Receiver Operating Characteristic (ROC) eğrisi analizi yapıldı; p<0.05 istatistiksel olarak anlamlı kabul edildi.
Bulgular: Olguların %76.4’ünde (n=39) over dokusu başarıyla korunurken, %23.6’sında (n=12) ooferektomi yapıldı. ROC analizinde, preoperatif KK kist/parankim oranının over korunmasını öngörmede istatistiksel olarak anlamlı olduğu belirlendi (AUC: 0.757, %95 GA: 0.595-0.920, p=0.007). Bu parametre için optimal cut-off değeri 0.855 olarak saptandığında, duyarlılık %74.4, özgüllük %25 olarak bulundu. Benzer şekilde, preoperatif AP kist/parankim oranının da cerrahi sonucu öngörmede istatistiksel açıdan anlamlı olduğu saptandı (AUC: 0.741, %95 GA: 0.581-0.902, p=0.012). AP boyutu için belirlenen 0.850 cut-off değerinde duyarlılık %69.2, özgüllük %33.3 olarak hesaplandı.
Sonuç: Preoperatif kist/over oranı, over kitlesi olan çocuklarda normal over dokusunun korunabilmesini öngörmede yararlı bir parametredir. Çalışmamızın sonuçlarından yola çıkarak, KK ve AP olarak değerlendirilen kist/over boyutlarının, belirlenen cutoff altındaki değerlerde uzun dönemdeki over rezervinin korunması ve fertilite potansiyelinin arttırılması üzerine olumlu etkisi olduğunu düşünmekteyiz. Bu nedenle, özellikle belirlenen cutoff değerinin altındaki oranlarda OKC tercih edilmesinin, uzun dönmede daha iyi sonuçlar elde edilebileceği kanısındayız.
