Reinforcement of cruroplasty with a polypropylene mesh during fundoplication in children with primary/acquired hiatal hernia and neurological ımpairment: A retrospective cohort study

Aim

Although favorable outcomes after fundoplication have been reported in children with or without hiatal hernia, particularly those with neurological impairment, anatomical recurrence and wrap migration remain significant problems. Re-enlargement of the hiatal opening and crural disruption are among the most common causes of anatomical failure. Therefore, reinforcement of the crural repair with prosthetic material has been recommended in adult surgery. However, experience with polypropylene mesh in children is limited. This study aimed to evaluate the safety and mid-term outcomes of reinforcing the crural repair with a polypropylene mesh during fundoplication.

Methods

Patients who underwent polypropylene mesh reinforcement during hiatal hernia repair and fundoplication or during cruroplasty performed for recurrent hiatal hernia or wrap migration between January 2023 and February 2026 were retrospectively reviewed. The mesh was fashioned into a U shape according to the size of the hiatus, placed over the crural sutures without direct contact with the esophagus, and secured with the same sutures. Demographic characteristics, comorbidities, surgical indications, perioperative findings, and postoperative outcomes were evaluated.

Results

Data from 11 patients were analyzed. The mean age was 7.9±5.7 years, and the female-to-male ratio was 6:5. The median follow-up was 24 months (IQR, 12–24). The mean operative time was 90±30 minutes, and the mean hospital stay was 9±5.2 days. Four patients had neurological impairment. Six had previously undergone hiatal hernia repair and were reoperated on for recurrent hiatal hernia or wrap migration. In five patients, prophylactic polypropylene mesh was used during primary fundoplication because of a large hiatal defect. No intraoperative complications occurred. The mean time to enteral feeding was 4.2±2.7 days. One patient developed dysphagia and gas-bloat syndrome during follow-up. One patient died because of the underlying disease. No mesh-related complications, hiatal hernia recurrence, or wrap migration were observed during follow-up.

Conclusion

In selected pediatric patients undergoing fundoplication, particularly those with neurological impairment and a high risk of recurrence, reinforcement of the cruroplasty with a polypropylene mesh is a safe and feasible surgical technique. The low complication rate and absence of recurrence suggest that this approach may improve anatomical success in appropriately selected patients.

Primer/edinsel hiatal herni ve nörolojik defisitli çocuklarda fundoplikasyon sırasında kruroplastinin polipropilen yama ile güçlendirilmesi: Retrospektif kohort çalışması

Amaç

Hiatal herni bulunan veya bulunmayan, özellikle nörolojik defisiti olan çocuklarda fundoplikasyon sonrası başarılı sonuçlar bildirilmesine rağmen anatomik nüks ve wrap migrasyonu önemli sorunlar olmaya devam etmektedir. Hiatal açıklığın yeniden genişlemesi ve krural ayrılma anatomik başarısızlığın en sık nedenleri arasındadır. Bu nedenle erişkin cerrahisinde krural onarımın protez materyallerle güçlendirilmesi önerilmektedir. Ancak çocuklarda polipropilen yama kullanımına ilişkin deneyim sınırlıdır. Bu çalışmanın amacı, fundoplikasyon sırasında uygulanan krural onarımın polipropilen yama ile güçlendirilmesinin güvenilirliğini ve orta dönem sonuçlarını değerlendirmektir.

Yöntem

Ocak 2023– Şubat 2026 tarihleri arasında kliniğimizde hiatal herni ve fundoplikasyon sırasında veya nüks/wrap migrasyonu nedeniyle yapılan kruroplasti sırasında güçlendirmek amacıyla polipropilen yama uygulanan hastalar retrospektif olarak incelendi. Polipropilen yama, hiatus boyutuna uygun olarak U şeklinde hazırlanarak özofagus ile doğrudan temas etmeyecek şekilde krural sütürler üzerine yerleştirildi ve aynı sütürlerle tespit edildi. Demografik özellikler, eşlik eden hastalıklar, cerrahi endikasyonlar, perioperatif bulgular ve postoperatif sonuçlar değerlendirildi.

Bulgular

Toplam 11 hastanın verileri analiz edildi. Hastaların ortalama yaşı 7,9±5,7 yıl olup kız/erkek oranı 6/5 idi. Medyan takip süresi 24 ay (IQR: 12–24 ay) olarak bulundu. Ortalama ameliyat süresi 90±30 dakika, hastanede kalış süresi ise 9±5,2 gün idi. Dört hastada nörolojik defisit mevcuttu. Altı hasta daha önce hiatal herni onarımı geçirmiş olup nüks hiatal herni veya wrap migrasyonu nedeniyle yeniden opere edildi. Beş hastada ise primer fundoplikasyon sırasında geniş hiatal defekt nedeniyle profilaktik polipropilen yama kullanıldı. İntraoperatif komplikasyon olmadı. Postoperatif enteral beslenmeye geçiş süresi ortalama 4.2±2.7 gündü. Geç postoperatif dönemde bir hastada disfaji ve gaz-bloat sendromu gelişti. Bir hasta altta yatan hastalığına bağlı nedenlerle kaybedildi. İzlem süresince hiçbir hastada yamaya bağlı bir komplikasyon, hiatal herni nüksü veya wrap migrasyonu saptanmadı.

Sonuç

Fundoplikasyon endikasyonu konulmuş seçilmiş çocuk olgularda, özellikle nörolojik defisit varlığında ve nüks riski yüksek olanlarda, işlem sırasında kruroplastinin polipropilen yama ile güçlendirilmesi güvenli ve uygulanabilir bir cerrahi yöntemdir. Düşük komplikasyon oranı ve nüks izlenmemesi, bu yaklaşımın uygun hasta grubunda anatomik başarının artırılmasına katkı sağlayabileceğini düşündürmektedir.
