Changing Trends in Gastrostomy Techniques and Comparison of Clinical Outcomes in Children

Objective: To evaluate the impact of gastrostomy indications on the choice of surgical technique and clinical outcomes in children requiring long-term enteral nutrition.
Materials and Methods: We retrospectively reviewed 43 pediatric patients who underwent primary gastrostomy between May 2021 and May 2026. Revision procedures were also evaluated. Patients were divided into three groups according to the surgical technique: laparoscopic gastrostomy (LG), open gastrostomy (OG), and laparoscopy-assisted percutaneous endoscopic gastrostomy (Lap-PEG). Demographic characteristics, gastrostomy indications, concomitant fundoplication, time to postoperative enteral feeding, length of hospital stay, postoperative complications, complication-related readmissions, and revision requirements were compared.
Results: A total of 48 primary gastrostomies were performed: 27 LG, 17 OG, and 4 Lap-PEG. Concomitant Nissen fundoplication was performed in 3 LG patients and 12 OG patients. The most common indication was long-term enteral nutrition, predominantly in children with cerebral palsy. Open gastrostomy with concomitant fundoplication was more frequently performed in patients with esophageal atresia. The use of laparoscopic gastrostomy increased during the study period. Four patients who underwent LG required revision surgery with conversion to open gastrostomy. No significant differences were observed among the groups regarding time to initiation of enteral feeding, overall complication rates, revision requirements, or complication-related readmissions. The most frequent complications were peristomal leakage, granulation tissue formation, and tube dislodgement, which appeared to be related mainly to patient characteristics and long-term stoma care.
Conclusions: In pediatric gastrostomy, the choice of surgical technique is influenced primarily by the need for concomitant surgical procedures rather than the indication for gastrostomy. Although the use of laparoscopic gastrostomy increased over time, complication rates, revision requirements, and complication-related readmissions were comparable among the techniques. These findings suggest that long-term outcomes are influenced more by patient-related factors and stoma care than by the surgical approach.

Amaç: Uzun süreli enteral beslenme gereksinimi olan çocuklarda gastrostomi endikasyonlarının cerrahi yöntem seçimine etkisini ve klinik sonuçlarını değerlendirmek.
Gereç ve Yöntem: Mayıs 2021–Mayıs 2026 tarihleri arasında primer gastrostomi uygulanan 43 çocuk hastanın verileri retrospektif olarak incelendi. Revizyon cerrahileri de değerlendirildi. Hastalar uygulanan yönteme göre laparoskopik gastrostomi (LG), açık gastrostomi (AG) ve laparoskopi eşliğinde perkütan endoskopik gastrostomi (Lap-PEG) olmak üzere üç gruba ayrıldı. Demografik özellikler, gastrostomi endikasyonları, eş zamanlı fundoplikasyon gereksinimi, postoperatif enteral beslenmeye başlama zamanı, hastanede yatış süresi, komplikasyonlar, komplikasyona bağlı yeniden yatışlar ve revizyon gereksinimi karşılaştırıldı.
Bulgular: Toplam 48 primer gastrostomi uygulandı; 27 hastaya LG, 17 hastaya AG ve 4 hastaya Lap-PEG gerçekleştirildi. LG grubunda 3, AG grubunda 12 hastaya eş zamanlı Nissen fundoplikasyonu uygulandı. En sık endikasyon, büyük bölümünü serebral palsi sekelli hastaların oluşturduğu uzun süreli enteral beslenme gereksinimiydi. Özofagus atrezili olgularda açık gastrostomi ve eş zamanlı fundoplikasyon daha sık tercih edildi. Çalışma süresince laparoskopik yaklaşımın kullanımında artış gözlendi. Laparoskopik gastrostomi uygulanan dört hastada revizyon gerekmiş ve açık gastrostomiye geçilmiştir. Gruplar arasında postoperatif enteral beslenmeye başlama zamanı, toplam komplikasyon oranı, revizyon gereksinimi ve komplikasyona bağlı yeniden yatış açısından anlamlı farklılık saptanmadı. En sık görülen komplikasyonlar peristomal akıntı, granülasyon dokusu gelişimi ve tüp çıkması olup, bu durumların daha çok hasta özellikleri ve uzun dönem stoma bakımı ile ilişkili olduğu değerlendirildi.
Sonuç: Pediatrik gastrostomide cerrahi yöntem seçimi, gastrostomi endikasyonundan ziyade eşlik eden cerrahi gereksinimlere göre şekillenmektedir. Çalışma süresince laparoskopik gastrostomi uygulamalarının belirgin olarak arttığı gözlenmiştir. Bununla birlikte, uygulanan cerrahi yöntemler arasında komplikasyon, revizyon gereksinimi ve komplikasyona bağlı yeniden yatış oranları açısından anlamlı farklılık saptanmamıştır. Bu bulgular, uzun dönem sonuçların cerrahi teknikten çok hasta özellikleri ve stoma bakımının kalitesinden etkilendiğini düşündürmektedir.
