High Body Mass Index independently predicts wound complications after appendectomy in children

OBJECTIVE

The effect of a high body mass index (BMI) percentile on pediatric appendectomy outcomes remains unclear, especially when postoperative wound complications are evaluated separately from overall postoperative morbidity. This study investigated the relationship between BMI percentile and postoperative wound complications after appendectomy and evaluated whether the effect of a high BMI percentile on these complications was independent of appendicitis severity.

MATERIALS AND METHODS

913 pediatric patients who underwent appendectomy due to acute appendicitis between January 2021 and January 2024 were evaluated. Patients were classified as normal weight, overweight, and obese according to age and gender-specific BMI percentiles. The primary endpoint was postoperative wound complications, defined as wound exudation and/or incision dehiscence. The independence of the relationship between BMI percentile and postoperative wound complications was evaluated using multivariate logistic regression analysis adjusted for appendicitis severity and gender. RESULTS

Postoperative wound complications occurred in 72 patients (7.9%). The complication rate was 1.1% in normal-weight children, while it was 27.4% and 21.0% in overweight and obese patients, respectively (p<0.001). This association was maintained even after adjusting for appendicitis severity and gender; being overweight (aOR: 32.23; 95% CI: 14.08–73.76), obesity (aOR: 19.02; 95% CI: 7.78–46.53), and complicated appendicitis (aOR: 2.66; 95% CI: 1.54–4.60) were independently associated with postoperative wound complications (all p<0.001). This association was maintained in both complicated and uncomplicated appendicitis subgroups. Culture positivity was detected in 25% of patients who developed wound complications. CONCLUSION

High BMI percentile is a strong predictor of postoperative wound complications, including wound exudation and incision dehiscence, after appendectomy in children, independent of appendicitis type and gender. Therefore, overweight and obese children should be considered a high-risk patient group for postoperative wound complications, even in cases of uncomplicated appendicitis. The fact that culture positivity is detected in only a limited percentage of wound complications suggests that these complications do not always reflect surgical site infection and that obesity-related wound healing disorders may also play a significant role.

AMAÇ

Yüksek vücut kitle indeksi (BMI) persentilinin pediatrik apendektomi sonuçları üzerindeki etkisi, özellikle postoperatif yara komplikasyonları genel postoperatif morbiditeden ayrı değerlendirildiğinde belirsizliğini korumaktadır. Bu çalışmada BMI persentili ile apendektomi sonrası postoperatif yara komplikasyonları arasındaki ilişki araştırılmış ve yüksek BMI persentilinin bu komplikasyonlar üzerindeki etkisinin apandisit şiddetinden bağımsız olup olmadığı değerlendirilmiştir.

GEREÇ VE YÖNTEM

Ocak 2021–Ocak 2024 tarihleri arasında akut apandisit nedeniyle apendektomi uygulanan 913 pediatrik hasta değerlendirildi. Hastalar yaş ve cinsiyete özgü BMI persentillerine göre normal kilolu, fazla kilolu ve obez olarak sınıflandırıldı. Primer sonlanım noktası, yara akıntısı ve/veya insizyon açılması olarak tanımlanan postoperatif yara komplikasyonuydu. BMI persentili ile postoperatif yara komplikasyonları arasındaki ilişkinin bağımsızlığı, apandisit şiddeti ve cinsiyet için düzeltme yapılan çok değişkenli lojistik regresyon analizi ile değerlendirildi.

BULGULAR

Postoperatif yara komplikasyonu 72 hastada (%7,9) gelişti. Komplikasyon oranı normal kilolu çocuklarda %1,1 iken, fazla kilolu ve obez hastalarda sırasıyla %27,4 ve %21,0 olarak saptandı (p<0,001). Bu ilişki, apandisit şiddeti ve cinsiyet için düzeltme yapıldıktan sonra da korundu; fazla kilolu olmak (aOR:32,23; %95 GA:14,08–73,76), obezite (aOR:19,02; %95 GA:7,78–46,53) ve komplike apandisit (aOR:2,66; %95 GA:1,54–4,60) postoperatif yara komplikasyonları ile bağımsız olarak ilişkili bulundu (tümü p<0,001). Bu ilişki hem komplike hem de komplike olmayan apandisit alt gruplarında korundu. Yara komplikasyonu gelişen hastaların %25'inde kültür pozitifliği saptandı.

SONUÇ

Yüksek BMI persentili, çocuklarda apendektomi sonrası yara akıntısı ve insizyon ayrışmasını içeren postoperatif yara komplikasyonlarının, apandisit tipi ve cinsiyetten bağımsız güçlü bir belirleyicisidir. Bu nedenle fazla kilolu ve obez çocuklar, komplike olmayan apandisit olgularında dahi postoperatif yara komplikasyonları açısından yüksek riskli hasta grubu olarak değerlendirilmelidir. Yara komplikasyonlarının yalnızca sınırlı bir bölümünde kültür pozitifliği saptanması, bu komplikasyonların her zaman cerrahi alan enfeksiyonunu yansıtmadığını ve obeziteye bağlı yara iyileşme bozukluklarının da önemli rol oynayabileceğini düşündürmektedir.
