Congenital Ureteral Shortening with Kinking Causing Functional Obstruction and Recurrent Urolithiasis in a Child with Hemihyperplasia

Introduction:Hemihyperplasia is characterized by asymmetrical overgrowth in one half of the body and is currently considered a clinical spectrum rather than a single syndrome. Genitourinary system anomalies may be present due to mesodermal developmental disorders.In the literature, isolated congenital ureteral shortening is reported quite rarely among urinary system anomalies associated with hemihyperplasia.

Case:A pediatric patient with known hemihyperplasia presented to our clinic with a history of recurrent right ureteral stones. The left half of the body was longer and hypertrophic, while the right side was relatively shorter. The patient's mental development was consistent with his/her peers, and no syndromic dysmorphism was detected in his/her facial appearance.The patient had previously undergone multiple endoscopic lithotripsy procedures due to right ureteral stones. Although each procedure was technically successful, stone recurrence and clinical complaints persisted. Magnetic resonance urography revealed segmental stenosis along the right ureter outside the area of ​​stone location. A decision was made to perform surgical exploration. During the operation, it was determined that the right ureter was congenitally short, abnormally fixed to the surrounding tissues at the level of the anterior superior iliac spine, and formed a significant kink.The affected segment was resected and ureteroureterostomy was performed. No complications developed in the postoperative period, and no obstruction or stone recurrence was observed.

Discussion:Kink formation due to congenital ureteral shortening leads to functional obstruction through a different mechanism than intrinsic ureteral strictures or inflammatory strictures due to stones. In this case, the cause of the obstruction was a mechanical angulation due to the length and abnormal fixation of the ureter, rather than the ureteral lumen. It is thought that hemihyperplasia may be a reflection of ipsilateral mesodermal developmental disorder, and this condition may affect the length, mobility, and anatomical relationships of the ureter. Focusing solely on endoscopic stone treatments in the presence of recurrent urolithiasis may delay the identification of underlying anatomical causes.

Conclusion:Kink formation due to congenital ureteral shortening is a rare cause of functional obstruction and recurrent urolithiasis in children. In patients with hemihyperplasia and a history of recurrent stones, underlying anatomical anomalies should always be considered in the differential diagnosis.

Hemihiperplazili Bir Çocukta Konjenital Üreter Kısalığına ve Darlığına Bağlı Gelişen Fonksiyonel Obstrüksiyon ve Tekrarlayan Ürolitiazis

Giriş

Hemihiperplazi, vücudun bir yarısında asimetrik aşırı büyüme ile karakterize olup, günümüzde tek bir sendromdan ziyade klinik bir spektrum olarak kabul edilmektedir. Mezodermal gelişim bozukluğuna bağlı olarak genitoüriner sistem anomalileri eşlik edebilmektedir.

Literatürde hemihiperplazi ile ilişkili üriner sistem anomalileri arasında izole konjenital üreter kısalığı oldukça nadir bildirilmiştir.

Olgu

Bilinen hemihiperplazisi olan pediatrik hasta, tekrarlayan sağ üreter taşı öyküsü ile kliniğimize başvurdu. Sol vücut yarısı daha uzun ve hipertrofik olup, sağ taraf görece kısa idi. Hastanın mental gelişimi yaşıtları ile uyumluydi ve yüz görünümünde sendromik dismorfizm saptanmadı.

Hasta, sağ üreter taşları nedeniyle daha önce birden fazla kez endoskopik taş kırma işlemi geçirmişti. Her bir girişim teknik olarak başarılı olmasına rağmen, taş nüksü ve klinik yakınmalar devam etmekteydi. Yapılan manyetik rezonans ürografide, taşın yerleşim alanı dışında sağ üreter boyunca segmental darlık izlendi. Bunun üzerine cerrahi eksplorasyon kararı alındı. Operasyon sırasında sağ üreterin konjenital olarak kısa olduğu, anterior superior iliak spina düzeyinde çevre dokulara anormal şekilde fikse olduğu ve belirgin kink yaptığı saptandı.Fonksiyonel obstrüksiyona ve kronik üriner staza yol açmaktaydı. Etkilenen segment rezeke edilerek üreteroüreterostomi uygulandı. Postoperatif dönemde komplikasyon gelişmedi, obstrüksiyon veya taş nüksü izlenmedi.

Tartışma

Konjenital üreter kısalığına bağlı kink oluşumu, intrinsik üreteral darlıklar veya taşlara bağlı inflamatuar striktürlerden farklı bir mekanizma ile fonksiyonel obstrüksiyona yol açmaktadır. Bu olguda obstrüksiyonun nedeni, üreter lümeninden ziyade üreterin uzunluğu ve anormal fiksasyonuna bağlı mekanik bir angülasyondu.

Hemihiperplazinin, ipsilateral mezodermal gelişim bozukluğunun bir yansıması olabileceği ve bu durumun üreterin uzunluğu, mobilitesi ve anatomik ilişkilerini etkileyebileceği düşünülmektedir. Tekrarlayan ürolitiazis varlığında yalnızca endoskopik taş tedavilerine odaklanılması, altta yatan anatomik nedenlerin tanınmasını geciktirebilir.

Sonuç

Konjenital üreter kısalığına bağlı kink oluşumu, çocuklarda fonksiyonel obstrüksiyon ve tekrarlayan ürolitiazisin nadir nedenidir. Hemihiperplazisi olan ve tekrarlayan taş öyküsü bulunan hastalarda, altta yatan anatomik anomaliler mutlaka ayırıcı tanıda düşünülmelidir. Erken tanı ve uygun cerrahi düzeltme, gereksiz tekrarlayan girişimleri ve uzun dönem morbiditeyi önleyebilir.
