LIVER-SPARING SURGERY AND EXTENDED RESECTIONS PROVIDE COMPARABLE LONG-TERM SURVIVAL IN HEPATOBLASTOMA

Objective
The aim of hepatoblastoma surgery is to achieve complete tumor resection while preserving as much functional liver parenchyma as possible. With increasing surgical experience and advances in multimodal treatment strategies, liver-sparing resections can now be performed in selected patients without compromising oncological principles. This study compared the surgical and oncological outcomes of extended and liver-sparing resections in patients with POSTTEXT II and III hepatoblastoma.
Methods
Patients who underwent surgery for hepatoblastoma between 2005 and 2025 were retrospectively reviewed. Patients with POSTTEXT I or IV disease were excluded. Patients with POSTTEXT II or III disease who underwent extended resection (Group A) or liver-sparing resection (Group B) were compared regarding surgical complications, recurrence, and survival. Five-year event-free survival was analyzed using the Kaplan–Meier method.
Results
Of 61 patients treated for hepatoblastoma, 36 had POSTTEXT II or III disease. Three underwent primary liver transplantation because of major vascular involvement or proximity precluding resection. Among the remaining 33 patients, 20 underwent extended resection (lobectomy/extended hepatectomy) and 13 underwent liver-sparing resection. Recurrence occurred in one patient (5%) in Group A and three patients (23.1%) in Group B; however, the difference was not statistically significant (p>0.05). Notably, two patients with recurrence in the liver-sparing group had multifocal tumors (2/3). Five-year event-free survival was 88.9% in Group A and 69.2% in Group B, with no significant difference (p=0.132). (Figure1)
Conclusion
Although recurrence appeared more frequent after liver-sparing surgery, survival outcomes were comparable between the groups. With current multimodal treatments and appropriate surgical principles, liver-sparing resections may provide oncological efficacy comparable to extended resections and may be preferred in selected patients at experienced centers.

Amaç
Hepatoblastom cerrahisinde amaç tam tümör rezeksiyonu sağlarken fonksiyonel karaciğer parankiminin olabildiğince korunmasıdır. Geçmişte geniş hepatik rezeksiyon gerektirdiği düşünülen birçok hepatoblastom olgusunda günümüzde artan cerrahi deneyim ve gelişen multimodal tedavi stratejileri sayesinde onkolojik prensiplerden ödün vermeden karaciğer koruyucu rezeksiyonlar uygulanabilmektedir. Bu çalışmada, tedavi sonrası tümör yaygınlığını tanımlayan evreleme sistemine göre POSTTEXT II ve III hepatoblastomlu hastalarda geniş ve karaciğer koruyucu rezeksiyonların cerrahi ve onkolojik sonuçlarının karşılaştırılması amaçlandı.
Yöntem
2005–2025 yılları arasında kliniğimizde hepatoblastom nedeniyle opere edilen hastalar retrospektif olarak incelendi. POSTTEXT I ve IV olgular çalışma dışı bırakıldı. POSTTEXT II ve III hastalarda geniş (GrupA) ve karaciğer koruyucu rezeksiyon (GrupB) uygulanan olgular karşılaştırıldı. Cerrahi komplikasyonlar, nüks ve sağkalım sonuçları analiz edildi. Beş yıllık olaysız sağkalım Kaplan–Meier yöntemi ile değerlendirildi, gruplar arasında karşılaştırıldı.
Bulgular
Çalışma döneminde hepatoblastom nedeniyle tedavi edilen 61 hastanın 36'sı POSTTEXT II veya III evresindeydi. Üç hastada rezektabiliteyi engelleyen majör vasküler tutulum/komşuluk nedeniyle primer karaciğer nakli uygulandı. Cerrahi uygulanan 33 hastanın 20'sine geniş rezeksiyon (lobektomi/extended hepatektomi), 13'üne ise karaciğer koruyucu rezeksiyon uygulandı. Grup A’da bir (%5), Grup B’deyse üç (%23,1) nüks gözlenmesine karşın gruplar arasında istatistiksel olarak anlamlı fark saptanmadı (p>0.05). Karaciğer koruyucu cerrahi grubunda nüks gelişen 3 olgunun 2’sinde tümör multifokal özellikteydi. Olaysız 5 yıllık sağkalım oranı Grup A’da %88,9, Grup B’de ise %69,2 olup gruplar arasında anlamlı farklılık izlenmedi (p:0.132) (Resim 1).
Sonuç: Literatürde PRETEXT II ve III hastalarda 5 yıllık sağkalım sırasıyla ortalama %85 ve %72 olarak verilmektedir. Her ne kadar karaciğer koruyucu cerrahide nüks oranları istatistik fark olmamakla beraber daha yüksek görünüyorsa da, serimizde aynı hasta grubu için yaşam oranları benzer olup güncel tedavi modaliteleri ve cerrahi prensiplerle karaciğer koruyucu rezeksiyonların genişletilmiş rezeksiyonlar ile benzer etkinlikte olduğu görülmüştür. Hepatoblastoma’da uygun hastalarda ve deneyimli merkezlerde karaciğer koruyucu cerrahinin geniş rezeksiyonlara tercih edilebileceği sonucuna ulaşılmıştır.
