Separation of ischiopagus conjoined twins with complex colorectal and urologic anomalies: a case report

Introduction
Conjoined twinning is rare, occurring in 1 per 50,000–200,000 live births. Ischiopagus twins, defined by pelvic fusion with variable visceral sharing, represent one of the most anatomically complex subtypes. Successful separation requires detailed imaging and multidisciplinary planning. We report the staged separation of ischiopagus tetrapus conjoined twins with a shared distal rectum and crossed ureteric anatomy.
Case Presentation
Female ischiopagus tetrapus conjoined twins were born to a G3P1 mother at 32+5 weeks' gestation by elective cesarean section, with a combined birth weight of 3.5 kg. Postnatal MRI and CT angiography demonstrated separate brains, spines, hearts, lungs, livers, kidneys, and urinary bladders, with fusion limited to pelvic soft tissue without bony fusion. Gastrointestinal evaluation showed separate upper gastrointestinal tracts with a shared distal rectum draining via two posterior anal openings, with an ectopic ureter crossing to the contralateral bladder. Initial endoscopic evaluation, tissue expander placement, and anal dilatation were performed at 7 months. Definitive separation followed at 11 months. The shared rectum was divided, preserving distal bowel continuity for one twin via limited posterior sagittal anorectoplasty with a double-barrel ileostomy; the second twin received a diverting ileostomy and distal Hartmann pouch. Ureteroureterostomy over a JJ stent corrected the crossed ureteric drainage; pelvic osteotomies facilitated separation. Reconstruction used local tissue and an anterior skin flap for soft-tissue coverage. Wound dehiscence and flap venous congestion resolved with conservative management. Both twins survived with preserved renal and bowel function and appropriate growth and development over 12 months' follow-up.
Conclusions
Staged separation guided by detailed imaging and early multidisciplinary coordination achieved a favorable outcome in ischiopagus twins with shared distal rectal and crossed ureteric anatomy. Adaptable reconstructive strategies, including local flap coverage for soft-tissue closure, were essential to survival and early functional recovery in both twins.

Giriş
Yapışık ikizlik nadir görülen bir durumdur ve her 50.000–200.000 canlı doğumda bir görülür. Pelvik füzyon ve değişken visseral paylaşımla karakterize ischiopagus ikizler, anatomik olarak en karmaşık alt tiplerden birini oluşturur. Başarılı bir ayırma işlemi, ayrıntılı görüntüleme ve multidisipliner planlama gerektirir. Bu olguda, ortak distal rektum ve çapraz üreter anatomisi olan ischiopagus tetrapus yapışık ikizlerin aşamalı ayrılması sunulmaktadır.
Olgu Sunumu
G3P1 anneden 32+5 gestasyon haftasında elektif sezaryen ile doğan kız ischiopagus tetrapus yapışık ikizlerin toplam doğum ağırlığı 3,5 kg idi. Postnatal MRG ve BT anjiyografide ayrı beyinler, omurgalar, kalpler, akciğerler, karaciğerler, böbrekler ve idrar keseleri saptandı; füzyon, kemik tutulumu olmaksızın pelvik yumuşak dokuyla sınırlıydı. Gastrointestinal değerlendirmede ayrı üst gastrointestinal traktüsler ve iki posterior anal açıklıkla drene olan ortak bir distal rektum izlendi; ürolojik değerlendirmede bir ikizde kontralateral mesaneye açılan ektopik üreter saptandı. Yedi aylıkken endoskopik değerlendirme, doku genişletici yerleştirilmesi ve anal dilatasyon uygulandı. Kesin ayırma işlemi 11 aylıkken gerçekleştirildi. Ortak rektum ayrılarak bir ikizde çift namlulu ileostomi ile birlikte limitli posterior sagittal anorektoplasti yoluyla distal barsak devamlılığı korundu; diğer ikize ise diverting ileostomi ve distal Hartmann güdüğü oluşturuldu. JJ stent üzerinden üreteroüreterostomi ile çapraz üreter drenajı düzeltildi; pelvik osteotomiler ayırmayı kolaylaştırdı. Yumuşak doku örtümü için lokal doku ve anterior deri flebi kullanılarak rekonstrüksiyon sağlandı. Yara ayrışması ve flep venöz konjesyonu konservatif tedavi ile geriledi. Her iki ikiz de korunmuş renal ve barsak fonksiyonu ile 12 aylık takipte uygun büyüme ve gelişim göstererek hayatta kaldı.
Sonuç
Ayrıntılı görüntüleme ve erken multidisipliner koordinasyon rehberliğinde yapılan aşamalı ayırma, ortak distal rektum ve çapraz üreter anatomisi olan ischiopagus ikizlerde olumlu bir sonuç sağlamıştır. Yumuşak doku kapatılmasında lokal flep kullanımını içeren esnek rekonstrüktif stratejiler, her iki ikizin de hayatta kalması ve erken fonksiyonel iyileşmesi için gerekli olmuştur.
