Non operative treatment of acute appendicitis: what we have learned after 10 years experience

Introduction: Acute appendicitis continues to be the first emergency in the world,it is still one of the most common indication for emergency abdominal surgery in children, despite the fact that the appendectomy procedure is not without risk and the risk of post-operative complications can reach up to 30% according to some studies, despite this, appendectomy by conventional surgery or laparoscopic surgery is still the only treatment for acute appendicitis in several centers, our study aims to show the effectiveness of medical treatment provided that patients are well selected. Materials and methods: We conducted a retrospective and prospective study in the pediatric surgery department of the CHU of Sétif over a period of 10 years from 2015 to 2025. All children admitted to our department for the diagnosis of uncomplicated acute appendicitis were included in this study. Résults:the distribution of acute appendicitis cases based on gender, age group, and type of treatment during the study shows a male predominance ( up to 70%) the most found age group was the age between 9-13 years, the rate of surgical treatment proposed as first-line treatment decreased from 54% between 2015 and 2017 to 30% after 2017. Conclusion : Studies have shown that non-operative treatment has its place in the treatment of uncomplicated acute appendicitis and that the percentages will probably be improved thanks to more suitable algorithms, depending on our resources. The discussion and the information of the parents is a decisive and important element to propose our attitude and that makes it possible to decrease the rate of the acute appendicitis treated surgically in the departments of pediatric surgery

Akut apandisit vakalarında ameliyatsız tedavi: 10 yıllık deneyimimizden öğrendiklerimiz

Giriş: Akut apandisit dünyada en sık karşılaşılan acil durum olmaya devam etmektedir ve çocuklarda acil karın cerrahisi gerektiren durumlar arasında hala en yaygın olanlardan biridir. Apendektomi işleminin risksiz olmamasına ve bazı çalışmalara göre ameliyat sonrası komplikasyon riskinin %30'a kadar çıkabilmesine rağmen, birçok merkezde akut apandisit için hala tek tedavi yöntemi konvansiyonel veya laparoskopik cerrahi ile yapılan apendektomidir; çalışmamız, hastaların doğru seçilmesi koşuluyla medikal tedavinin etkinliğini göstermeyi amaçlamaktadır.

Materyal ve Metot: Sétif Üniversitesi Hastanesi (CHU) Çocuk Cerrahisi Bölümü'nde, 2015-2025 yılları arasındaki 10 yıllık bir süreçte retrospektif ve prospektif bir çalışma yürüttük. Komplike olmayan akut apandisit tanısıyla bölümümüze başvuran tüm çocuklar çalışmaya dahil edildi.

Bulgular: Çalışma süresince akut apandisit vakalarının cinsiyet, yaş grubu ve tedavi türüne göre dağılımı incelendiğinde erkek hastaların çoğunlukta olduğu (%70'e varan oranla) görülmüştür; en sık rastlanan yaş grubu 9-13 yaş aralığı olmuştur. İlk basamak tedavi olarak önerilen cerrahi tedavi oranı, 2015-2017 yılları arasında %54 iken, 2017 sonrasında %30'a düşmüştür.

Sonuç: Çalışmalar, komplike olmayan akut apandisit tedavisinde cerrahi dışı tedavinin bir yeri olduğunu ve mevcut kaynaklara bağlı olarak daha uygun tedavi algoritmaları sayesinde başarı oranlarının muhtemelen artacağını göstermiştir. Ebeveynlerle yapılan görüşmeler ve onlara sağlanan bilgilendirme, izlenecek yaklaşımın belirlenmesinde belirleyici ve önemli bir unsurdur; bu süreç, çocuk cerrahisi bölümlerinde cerrahi yolla tedavi edilen akut apandisit vakalarının oranını düşürmeyi mümkün kılmaktadır.
