Standardized Nonoperative Management for Pediatric Acute nonperforated Appendicitis: Clinical Outcomes From a Tertiary Referral Center

Aım: Despite growing evidence on nonoperative management (NOM) of acute non-perforated appendicitis, this approach has not gained widespread acceptance. We reviewed the current literature, developed our institutional NOM protocol, and analyzed our outcomes.

Methods: We retrospectively reviewed the records of 298 children with acute appendicitis treated between November 2021 and May 2025. Patients aged 4–18 years, with symptom duration ≤72 hours, white blood cell (WBC) count <18,000/mm³, appendix diameter ≤11 mm, and no fecalith were included. NOM success or failure was determined based on symptom evaluation at 12, 24, 36, and 48 hours; surgery was performed earlier if symptoms progressed before 48 hours.

Results: NOM succeeded in 241 of 298 patients (80.9%) and failed in 57 (19.1%). Early failure occurred in 43 patients (14.4%) during treatment. Fourteen patients (4.7%) presented with recurrence after discharge, at a median of 5 months (range, 0–48 months). Median age was higher in the failed NOM group (13 vs. 12 years, p=0.003). Time to hospital discharge (48 vs. 30 hours, p<0.001) and time to starting oral feeding (36 vs. 24 hours, p<0.001) were longer in the failed group. WBC (p=0.034) and appendix diameter (p=0.004) were higher in the failed group. Sex, symptom duration, and C-reactive protein (CRP) did not differ between groups.

Conclusıon: NOM of non-perforated appendicitis achieved an 80.9% success rate in our cohort, supporting it as an effective alternative to appendectomy in selected pediatric patients. Higher WBC, larger appendix diameter, and older age were independently associated with NOM failure, suggesting these parameters may help identify children less likely to respond to nonoperative treatment.

Amaç: Akut perfore olmayan apandisitte nonoperatif tedavinin (NOM) etkinliğine dair artan kanıtlara rağmen, bu yaklaşım henüz yaygın kabul görmemiştir. Güncel literatürü gözden geçirdik, kendi kurumumuza özgü bir NOM protokolü oluşturduk ve sonuçlarımızı analiz ettik.

Yöntem: Kasım 2021–Mayıs 2025 tarihleri arasında akut apandisit tanısı alan 298 çocuğun kayıtları retrospektif olarak incelendi. 4–18 yaşında, semptom süresi ≤72 saat, beyaz küre sayısı (WBC) <18.000/mm³, apendiks çapı ≤11 mm ve fekalit saptanmayan hastalar çalışmaya dahil edildi. NOM başarı veya başarısızlığı 12, 24, 36 ve 48. saatlerdeki semptom değerlendirmesine göre belirlendi; 48 saatten önce semptomlarda ilerleme olması durumunda cerrahiye daha erken karar verildi.

Bulgular: NOM, 298 hastanın 241'inde (%80,9) başarılı, 57'sinde (%19,1) başarısız oldu. Tedavi sürecinde 43 hastada (%14,4) erken başarısızlık görüldü. On dört hasta (%4,7), taburculuk sonrası ortanca 5 ay (0–48 ay) içinde semptom nüksü ile acil servise başvurdu. Başarısız NOM grubunun ortanca yaşı daha yüksekti (13 vs. 12 yıl, p=0,003). Hastaneden taburculuk süresi (48 vs. 30 saat, p<0,001) ve oral beslenmeye başlama süresi (36 vs. 24 saat, p<0,001) başarısız grupta daha uzundu. WBC (p=0,034) ve apendiks çapı (p=0,004) başarısız grupta daha yüksekti. Cinsiyet, semptom süresi ve C-reaktif protein (CRP) gruplar arasında farklılık göstermedi.

Sonuç: Perfore olmayan apandisitte NOM, kohortumuzda %80,9 başarı oranı ile seçilmiş pediatrik hastalarda apendektomiye etkili bir alternatif olarak desteklenmektedir. Başvuruda yüksek WBC, büyük apendiks çapı ve ileri yaş, NOM başarısızlığı ile bağımsız olarak ilişkiliydi.
