CHEMICAL BURN CASE CAUSED BY RANUNCULUS ARVENSIS

Ranunculus arvensis,family is a wild growing plant, used traditionally in China and the Far East for the treatment of rheumatoid arthritis and psoriasis. We present a patient who applied the plant R. arvensis externally to his left leg due alleviate leg pains, and suffered chemical burns as a consequence.

The 60-year old male patient was admitted to the burn treatment center of our hospital with a diagnosis of a 20 x 10 cm second degree burn lesion on the posterior surface of his left thigh. According to the story reported by the patient, he had applied a plant, which grows in the region where he lives, to the painful area and then wrapped the area with cloth. He had left this in place for 5 hours, and upon unwrapping his leg, had observed fluid filled sacs in the area where the plant had been applied. He had gone to the hospital for treatment, and although the lesion was treated for 14 days, there had been very little improvement. Upon physical examination of the patient, we established a 20 x 10 cm second degree burn injury on the posterior surface of the left thigh. The patient was evaluated by the dermatological clinic, and they also confirmed that the lesion was a burn injury. The hemogram and biochemical values of the patient were normal. Pseudomonas aeruginosa proliferated in the culture taken from the lesion. This proliferation was assessed by the Infectious Diseases clinic, and was evaluated as colonization as the lesion was clean and the laboratory values were normal. No antibiotics were started. During the time of his hospital stay, the patient’s injury was covered with paraffin-impregnated gauze dressings containing chlorhexidine. Our hospital determined from samples that the plant that had caused the injury was R. arvensis. The patient’s injury became 90% epithelized after 11 days in the hospital and he was discharged.

Very few chemical burn cases resulting from R. arvensis have appeared in the literature. In the Turkish and English literature, two chemical burn cases have been reported to have developed in connection with R. arvensis (3,4). In both cases, similar to the case we present in this paper, second degree chemical burns were caused but were completely recovered with treatment. Our case is the third such case in literature. People need to be made aware that these types of plants, although used throughout history as well as currently in Turkey and other regions of the world for the traditional treatment of some diseases, can have undesirable effects such as was seen in our case, and that use of modern medical methods should be encouraged for the treatment of illnesses.  

RANUNCULUS ARVENSİSİN SEBEB OLDUÄžU KİMYASAL YANIK VAKASI

Ranunculaceae familyasından olan Ranunculus arvensis yabani bir tür bitki olup, Çin’de ve Uzakdoğu’da geleneksel olarak romatroid artritte ve psöriazis tedavisinde kullanılmaktadır.Biz sol bacak ağrısı nedeniyle Ranunculus arvensis bitkisini bacağının üzerine uygulayan ve bunun neticesinde kimyasal yanık oluşan bir hastayı sunacağız.

60 yaşında erkek hasta hastanemiz yanık tedavi merkezine sol uyluk ön yüzde bulunan yaklaşık 20*10 cm boyutlarında ikinci derece yanık yarası tanısı ile yatırıldı . Hastadan alınan hikayede 10 gün önce sol bacağındaki ağrı nedeniyle yaşadığı bölgede bulunan bir bitkiyi ağrı olan bölgeye 5 saat süre ile bir bez ile sardığı ve bacağını açtığında bitkiyi uyguladığı alanda içi sıvı dolu kesecikler oluştuğunu gördüğü öğrenildi. Bunun üzerine hastaneye giden hastaya 10 gün pansuman yapılmasına rağmen yarasında çok az düzelme olduğu belirtildi. Hastaya yaptığımız muayenede sol uyluk ön yüzde 20*10 cm ebatlarında ikinci derece yanık yarası tespit ettik. Hasta dermatoloji kliniğince değerlendirildi ve bu lezyonun bir yanık vakası olduğu onlar tarafından da teyid edildi. Hastanın hemogram ve biyokimyasal değerleri normaldi. Yaradan alınan kültürde P.Aeroginosa üredi. Enfeksiyon hastalıkları kliniğince değerlendirilen hastadaki bu üreme yaranın temiz olması ve laboratuar değerlerinin normal olmasından dolayı kolonizasyon olarak değerlendirildi. Hastaya herhangi bir antibiyotik başlanmadı. Kliniğimizde yattığı süre içerisinde yara üzerine klorhekzidin içeren parafin emdirilmiş tül sargı ile pansuman yaptık. Hastamızda yanık oluşturan bitki Atatürk üniversitesi ziraat fakültesi öğretim elemanlarınca değerlendirilerek Ranunculus arvensis   olduğu belirtildi. Kliniğimizde 11 gün yatan hastanın yarası %90 oranında epitelize oldu ve taburcu edildi.

Literatürde Ranunculus arvensis’e bağlı kimyasal yanık vakası oldukça az sayıdadır.Türkçe ve İngilizce literatürde  Ranunculus arvensise bağlı olarak gelişen iki kimyasal yanık vakası tespit edebildik. Bizim vakamız literatürdeki üçüncü vakadır. Tarih boyunca ülkemizde ve dünyanın değişik bölgelerinde geleneksel olarak bazı hastalıkların tedavisinde kullanılan bu tür bitkilerin vakamızda olduğu gibi istenmeyen etkilerinin olduğu konusunda insanlar bilgilendirilmeli ve modern yöntemlerle tedaviler açısından teşvik edilmelidirler.
