Reconstruction of severe burn injuries of the face in children by using the collagen-elastin matrix(Matriderm®)
Dermal substitutes have been developed in order to improve the quality of the reconstructed skin, to reduce scarring and prevent wound contraction, they serves as a foundation for split thickness skin graft, thus enhancing short and long-term results.
We present a series of 9 pediatric patients with severe facial burns which were total face in 4, 75% surface area of the face in 3 and half of face in 2 patients treated with application the collagen-elastin matrix (Matriderm®) and simultaneously an unmeshed split-thickness skin graft. Five out of 9 patients were late, and 4 were acute admissions.
Graft quality was close to normal skin in terms of elasticity, pliability, texture and the color. Good contour was also achieved and no hypertrophic scar occurred. Aesthetic and functional results have been encouraging.
Matriderm® 1mm, as a dermal substitute was found to be a useful adjunct to severe post-burn facial reconstruction. This study shows that it permits to obtain a quick healing with satisfying aesthetic and functional results and permit an early discharge.
Çocuklarda ağır yüz yanıklarının "collagen-elastin matrix,(Matriderm®" kullanılarak rekonstrüksiyonu
Rekonstrüksiyon yapılmış derinin kalitesini artırmak, skar oluşumunu azaltmak ve yara kontraksiyonunu azaltmak için dermis yerine geçebilen ürünler geliştirilmektedir ve bu ürünler "split thickness" deri greftleri (STDG) için bir "döşek" gibi yardım ederler, böylece greftlerin kısa ve uzun süreli sonuçlarının güçlenmesini sağlarlar.
Ağır yüz yanığı olan 9 çocuk hastada "collagen-elastin matrix (Matriderm®)" tatbiki ve eş zamanlı olarak STDG uygulanmıştır. Bunlardan 4'ünde yüzün tamamı; 3'ünde %75'i; 2 sinde ise yüzün yarısında tam kat yanık vardı. Bunlardan 5' i geç, 4'ü ise akut olgulardı.
Elastisite, katlanabilirlik, doku ve renk bakımlarından greflerin kalitesi normal deriye yakın olarak elde edildi. İyi konturlar elde edildi ve hipertrofik skar görülmedi. Erken dönem estetik ve fonksiyonel sonuçlar cesaret vericidir.
Bir dermis alternatifi olarak Matriderm® 1mm, ağır yanık sonrası fasiyal rekonstrüksiyon için oldukça yaralı bulunmuştur. Bu çalışma ile estetik ve fonksiyonel olarak tatmin edici, hızlı iyileşme ve erken taburculuk gibi sonuçlar elde edilmiştir.
