Hot milk burn in the pediatric aged patients: 13 years of experience

Objective: The aim of this study was to retrospectively analysis pediatric aged patients who were hospitalized in the Burn Unit of Erciyes University Medical Faculty due to hot milk burn injury.

Methods: A retrospective study was conducted on 150 pediatric aged patients with hot milk burn injury that hospitalized between January 2000 and December 2012. All of these patients had the type of moderate or major burn trauma according to the ‘American Burn Association's Grading System for Burn Severity and Disposition of Patients’. Outpatients with minor burn trauma were excluded from the study group.

Results: The overall male-to-female ratio was 1.03:1. The highest-risk group for hot milk burn injuries were the children that under 6 years of age (n=139, 92.6%), with the greatest number of injuries occurring in the children who were 2 to 3 years (n=48, 32%).  Sixty percent (n=90) of the patients was scalded by immersion (falling into a cauldron or immersing extremities into a holder containing hot milk) and 40% (n=60) was scalded by the spilled hot milk. The mean burned body surface area of the patients was 19.1% and forty-six percent (n=70) received injuries in the range of 11% to 20% burned area. The trunk and lower extremities were affected most commonly. There were 27 patients with moderate and 123 with major burn trauma. Mean abbreviated burn severity index of the patients was 3.8. Highest incidence of hot milk burn was seen during July and August. The highest number of patients admitted was in 2001 (n=21, 14%) and 2006 (n=17, 11, 3%) Forty-four percent (n=66) of the patients recieved only wound dressing and fifty-six percent (n=84) received burn wound debridement and reconstruction with surgical closure techniques. The mean time between the injury and surgery was 8.3 days. Mean hospital stay was 15.8 days. Nine of the patients died with a mortality rate of 6%.

Conclusion: Hot milk burn injury should be considered as separately from other liquid burns due to its fat content. Its physical and chemical properties that gained by the fat component such as low viscosity, high specific heat and boiling point, provide it to damage deeper tissues; thus the morbidity and mortality increase. Therefore, the affected areas should be evaluated initially by skilled specialists for the management of treatment. Tangential excision and surgical closure techniques of burn wound should be performed immediately if it was indicated. The most important step for preventing childhood accidents is social education. Educational programs about burn care and prevention should be organized on national and local media, internet, and printed materials such as brochures.

 

Pediatrik hasta grubunda görülen sıcak süt yanıkları: 13 yıllık tecrübe

Amaç: Bu çalışmada; Erciyes Üniversitesi Tıp Fakültesi Yanık Ünitesi’ nde sıcak süt yanığı nedeni ile hospitalize edilerek tedavisi gerçekleştirilmiş hastaların retrospektif analizi amaçlanmıştır.

Yöntem: Ocak 2000 ve Aralık 2012 tarihleri arasında sıcak süt yanığı nedeni ile hospitalize edilmiş hastalar çalışmaya dahil edilmiştir. Bütün hastalar ‘Amerikan Yanık Derneği Yanık Derecelendirme Sistemi’ ne gore orta derece ve major yanık travması bulunan hastalardır. Ayaktan takip ve tedavisi gerçekleştirilen hasta grubu çalışma dışı bırakılmıştır.

Bulgular: Çalışmada erkek-kız oranı 1.03:1’ di. Sıcak süt yanığı nedeni ile en fazla risk altında bulunan hastalar 6 yaş altı çocuklardı (n=139, %92,6) ve en fazla travmaya maruz kalan yaş grubu ise 2-3 yaştı (n=48, %32). Hastaların %60’ ı (n=90) imersiyon nedeni ile yaralanmışken (süt içeren kazanın içine düşme ya da uzuvların süt içeren bir kabın içine daldırılması), %40’ ı ise (n=60) üzerine süt dökülmesi sonucu yaralanmıştı. Ortalama etkilenmiş vücut yüzey alanı %19.1’ di ve hastaların %46’ sının (n=70) etkilenmiş vücut yüzey alan değeri %11-20 arasındaydı. Gövde ve alt ekstremiteler en sık etkilenen vücut bölümleriydi. 27 hastada orta derecede, 123 hastada ise major derecede yanık travması mevcuttu. Ortalama yanık ciddiyet indeks skoru 3.8’ di. Travmanın en sık görüldüğü aylar Temmuz ve Ağustos iken; hastaların en sık hospitalize edildiği yıllar 2001 (n=21, %14) ve 2006 (n=17, %11.3)’ ydı (n=4, %2.7). Hastaların %44’ ü (n=66) sadece pansuman ile takip edilirken; %56’ sına (n=84) yanık debritmanı ve cerrahi kapama teknikleri uygulandı. Travma ve cerrahi arasındaki ortalama süre 8.3 gün iken; ortalama hastane yatış süresi 15.8 gündü. Mortalite 9 hasta ile %6’ ydı.

Sonuç: Sıcak süt yanıklarının, sütün yağ ihtivası nedeni ile diğer sıvılara bağlı olarak gelişen yanıklarından ayrı olarak değerlendirilmesi gerekmektedir. Yağ içeriği nedeni ile kazanmış olduğu düşük viskozite, yüksek özısı ve kaynama noktası gibi fiziksel ve kimyasal özellikler, sıcak sütün daha derin doku hasarı oluşturmasına ve böylelikle morbidite ve mortalite oranlarında artışa neden olmaktadır. Bu nedenle hastaların, erken dönemde yanık konusunda uzman hekimler tarafından, tedavi planı amacıyla hızlı bir şekilde değerlendirilmesi gerekmektedir. Tanjansiyel eksizyon ve cerrahi kapama teknikleri uygun zamanda vakit kaybetmeden gerçekleştirilmelidir. Çocukluk çağı travmalarının engellenmesinde en önemli basamak toplumun eğitimidir. Eğitici programların ulusal ve yerel medya, internet ve broşürler gibi basılı materyaller vasıtasıyla bireylere ulaştırılması sağlanmalıdır.
