Seizures Following Burns in Children: Report of Two Cases

Aim:Neurological complications in burned patients are not common. We reported two cases who developed post burn seizures in the patients who followed for burns between 2008 and 2013.

Case 1:The patient was 20 months old boy. He had been fallen into the stewpot which contained boiled milk. He had a previous history of convulsion which developed after gastroenteritis one year ago.   He had 20% second degree deep burn on his back and gluteal regions. The levels of serum electrolyte and glucose were in normal ranges.  Generalized tonic and clonic seizures occurred within one hour. Phenytoin sodium was given 15 mg/kg intravenously. Seizures ceased and did not recur after treatment.

Case 2:The patient was 26 months old boy. He had 25% second degree deep burn  on his back, gluteal, and thighs regions which developed due to fallen into the stewpot which contained boiled çökelek. The levels of serum electrolyte and glucose were in normal ranges.  Generalized tonic and clonic seizures developed two hours after burn. Diazepam was given 0.25 mg/kg  intravenously. Seizure ceased and did not recur after treatment.

Conclusion:Post burn seizures are more common in infants. Despite hyponatremia is the most common cause, the other causes are high fiver, sepsis, drug toxicity, history of convulsion and unknown etiology. One of our case had a previous history of convulsion, the other had unknown etiology.  

 

Çocuklarda Yanık Sonrası Konvülsiyon: İki Olgu Sunumu

Amaç:Yanık hastalarında nörolojik komplikasyonlar sık değildir. 2008-2013 yılları arasında yanık nedeniyle takip edilen olgular arasında konvülsiyon gelişen iki olgu sunuldu.

Olgu 1:20 aylık erkek çocuğuydu. Çökelek kazanının içine düşme sonucu sırtında ve gluteal bölgelerde % 20 ikinci derece derin yanık oluşmuştu. Özgeçmişinde bir yıl öncesine ait gastroenterit sonrası konvülsiyon öyküsü vardı. Serum elektrolit ve şeker değerleri normaldi. Yanıktan bir saat sonra yaygın  tonik ve klonik kasılmaları oldu. Fenitoin sodyum 15 mg/kg intravenöz başlandı. Tedaviden sonra konvülsiyon  tekrarlamadı.

Olgu 2:26 aylık erkek çocuğuydu. Süt kazanın içine düşme sonucu  sırtında, gluteal bölge ve uylukta % 25 ikinci derece yanık gelişmişti.  Yanıktan 2 saat sonra yaygın tonik, klonik kasılmalar meydana geldi. Diazepam  0.25 mg/kg intravenöz yapıldı.  Konvülsiyon kesildi, tekrarlamadı.. Serum elektrolit ve şeker değerleri normaldi.

Sonuç:Yanık sonrası konvülsiyon infantlarda daha sıktır. Genellikle hiponatremi en sık neden olmakla birlikte yüksek ateş, drug toksisitesi, , sepsis , bilinmeyen nedenler ve geçirilmiş konvülsiyon öyküsü diğer nedenlerdir. Olgumuzun birinde konvülsiyon öyküsü varken diğer olguda herhangi bir neden tespit edilmedi.
