Introduction:Undescended testis is an important childhood condition that requires early surgery due to increased risks of infertility, malignancy and torsion. The current approach to the time of surgery varies between centers but operating at 3 to 24 months is recommended.However, the orchiopexy age in our country is higher. We retrospectively evaluated the undescended testis cases treated surgically at 2 different pediatric surgery clinics in this study.

Material and Method:A total of 387 patients operated on at the Ordu Obstetrics Hospital and Denizli State Hospital Pediatric Surgery Clinics between 2005 and 2010 were retrospectively evaluated for the age of surgery and the treatment method used.

Results:A total of 387 patients (198 right, 160 left and 29 bilateral for a total of 416 testes) treated surgically by 4 different pediatric surgeons were divided into 2 groups as those operated before and after 2 years of age. The first group consisted of 238 patients (61.5%) and the second of 159 patients (38.5%). The mean age was 6.3 years (3-13 years) and 15.7 months (3-24 months) in the first and second groups respectively. The procedures used were surgery in 369 patients, laparoscopic orchiopexy in 11 patients where the testis could not be palpated and orchiectomy in 7 patients because of an atrophic testis.

Conclusion:We found that most operated undescended testis cases were older than 2 years while the mean age was bigger than 12 months in those operated before this age. The values from our country are quite high, taking into account the suggestion of lowering the age of undescended testis surgery to 3 months in recent studies. We recommend emphasizing primary care services, increasing the awareness of families and making use of pediatric surgeons in suspected cases.

Giriş:İnmemiş testis artmış infertilite, malignite ve torsiyon gibi riskleri nedeniyle yaşamın erken dönemlerinde opere edilmesi gereken çocukluk çağının önemli bir hastalığıdır. Güncel

yaklaşım olarak, operasyon zamanı çeşitli merkezlerde farklılık göstermekle birlikte, 3 ile 24 aylık dönemler arasında yapılması önerilmektedir.Ancak ülkemizde orşiopeksi yaşı belirtilen bu yaşlardan daha yüksektir. Bu çalışmada 2 farklı çocuk cerrahisi kliniğinde cerrahi olarak tedavi edilmiş inmemiş testis olguları retrospektif olarak incelenmiştir.

Gereç ve Yöntem:Ordu Doğumevi ve Denizli Devlet Hastanesi Çocuk Cerrahisi Kliniklerinde 2005-2010 yılları arasında opere edilmiş 387 hasta operasyon yaşı ve uygulanan tedavi yöntemleri açısından retrospektif olarak incelendi.

Bulgular:Cerrahi olarak 4 farklı çocuk cerrahı tarafından tedavi edilen 387 hasta ve toplam 416 testis (198 sağ, 160 sol, 29 bilateral), 2 yaşından sonra ve 2 yaşından önce ameliyat edilenler olmak üzere 2 gruba ayrıldı. Birinci grup 238 hastadan (%61,5), ikinci grup ise 159 hastadan (%38,5) oluşmaktaydı. Birinci grubun yaş ortalaması 6,3 yıl (3-13 yaş) 2. grup ise 15,7 ay (3-24 ay) olarak tespit edildi. Toplam 369 hastaya cerrahi, nonpalpabl olan 11 hastaya laparoskopik orşiopeksi, 7 hastaya ise atrofik testis nedeniyle orşiektomi uygulandı.

Sonuç:Bu çalışma sonucunda opere edilen inmemiş testis hastalarının büyük çoğunluğunun 2 yaşından, 2 yaş öncesi opere edilen hastaların ise yaş ortalamasının 1 yaşından büyük olduğu saptanmıştır. Son çalışmalarda inmemiş testis cerrahi yaşının 3 aya kadar indirilmeye çalışıldığı düşünülürse ülkemizdeki bu yaş ortalamaları oldukça yüksek bulunmuştur. Bu durumun önlenmesi için birinci basamak sağlık hizmetlerine daha fazla önem verilmeli, aileler bilinçlendirilmeli ve şüpheli vakalarda çocuk cerrahları devreye girmelidir.
