Congenital Hemiscrotal Agenesis: Case Report

Introduction:

Congenital scrotal agenesis is a very rare anomaly that is characterized absence of scrotal development. There are only six cases of scrotal agenesis reported in the literature to date. In this letter we report the first case of hemi-scrotal agenesis.

Case:

A two years old boy was admitted to our department complaining of left groin swelling. Physical examination revealed a left sided silk sign, with the absence of ipsilateral hemi-scrotal rugaes. Left testis was located subcutaneously where the left hemi-scrotum should be. Scrotal raphe was located in normal configuration. Other systemic examinations did not revealed any coexisting abnormality. Also ophtalmological examination and echocardiography of the patient were normal. Abdominal ultrasonography did not reveal any coexisting abnormality. Serum testosterone and LH levels were in normal levels and karyotype analyses indicated genotype of 46XY.

Surgical exploration was planned for the left sided indirect groin hernia, and high ligation was performed. Also, during the surgical exploration it is confirmed that, left testis was under the subcutaneous tissue where it should be located anatomically and attached to the subcutaneous structures. The anatomy of inguinal canal and testis were usual. The skin stretch was enough for the left testicular settle down thus, scrotal reconstruction was not considered.

Conclusion:

The cases of congenital scrotal agenesis reported in the literature coexist with undescended testis and other congenital anomalies. The case we report is unique just because he has unilateral scrotal absence without any coexisting congenital anomaly, and both the testes are descended and attached to normal anatomical localization.

Absence of androgen receptors in the labioscrotal fold during the embrional life is supposed to be the cause of congenital scrotal agenesis. Hemi-scrotal agenesis may be a part of congenital scrotal agenesis that is compatible with this theory.

 

 

 

 

 

KONJENİTAL HEMİSKROTAL AGENEZİ: Olgu Sunumu

Giriş:

Konjenital skrotal agenezi skrotum gelişiminin hiç olmamasıyla karakterize çok nadir görülen bir anomalidir. Literatürde total skrotal agenezili toplam altı olgu bildirilmiştir.

Biz literatürdeki ilk hemiskrotal agenezi olgusunu bildiriyoruz.

Olgu :

2 yaşında erkek çocuk sol kasıkta şişlik şikayeti ilebaşvurdu. Yapılan muayenesinde sol inguinal bölgede silk sign mevcut ve aynı tarafta sol hemiskrotum rugaları yoktu. Testis sol skrotumun olması gerektiği yerde ve cilt altı yerleşimliydi. Skrotal rafe normal konfigürasyondaydı. Penis normal, sağ testis sağ skrotum içinde olağan yerleşimindeydi. Diğer sistemik muayeneleri doğal değerlendirildi. Göz muayenesi ve kardiak ekokardiyografisi normaldi. Batın ultrasonografisinde üriner sistemde anomali saptanmadı. Karyotip tayininde genotip 46 xy olarak tespit edilen hastanın testesteron ve LH değerleri normal düzeylerdeydi..

Solda tespit edilen indirekt inguinal herniye high ligasyon uygulandı. Sol testisin olması gereken skrotal lokalizasyonda fakat cilt altında yerleşmiş olduğu ve cilt altına tutunduğu operasyon esnasında doğrulandı. Testis ve inguinal kanal anatomisinde olağandan farklı bir patoloji saptanmadı. Sol tarafa testis yerleşimi için yeterli cilt esnekliği olduğundan skrotal rekonstrüksüyon düşünülmedi.

Sonuç:

Literatürde bildirilen konjenital skrotal agenezi vakaları inmemiş testis ve diğer konjenital anomaliler ile birliktelik göstermektedir. Bizim olgumuzda skrotumun unilateral yokluğu mevcuttu ve her iki testis olması gereken lokalizasyondaydı. Eşlik eden ek anomali yoktu. Bundan dolayı da özgündür.

Konjenital skrotal agenezi, embriyonel hayatta labioskrotal foldda androjen reseptörlerinin lokalize yokluğu ile açıklanmaya çalışılmıştır. Bu teori ile uyumlu olabileceği düşünülen hemiskrotal agenezi konjenital skrotal agenezinin bir formu olabilir.
