COMPLETE UROGENITAL NONUNION

The epididymal anomalies are frequently seen in patients with undescended testes. An otherwise healthy 7.5 years old boy admitted to our department with mother’s doubt of presence of the left testis. On physical examination the right testis was in scrotum. The left testis wasn’t palpable with a small mass compatible with testicular nubbin was noticed in scrotum. On ultrasound imaging, a testis-like structure was identified proximal to the internal ring of the inguinal canal which was measured 15x12x6 mm. in size. On surgical exploration vas deferens was found to be travelling through the inguinal canal, ending in the scrotum nearby an epididymis without any obvious testis or testis-like structure. Exploration extended to the abdominal cavity via the processus vaginalis and the testis was found near the sigmoid colon without any connection with the vas deferens and the epididymis. The testis was freed from surrounding structures preserving the testicular vasculature and orchidopexy was achieved. Postoperative period and follow-up period was uneventful. In cases of non-palpable testis with the vas deferens reaching to the scrotum, even in the presence of the epididymis in scrotum, an intraabdominal testis due to nonunion phenomenon must be kept in mind and extended exploration should be preceded.
ÜROGENİTAL TAM KAYNAMAMA

Epididim anomalileri, inmemiş testisle birlikte sıklıkla görülür. 7,5 yaşındaki erkek hasta, sol inmemiş testis şikayetiyle başvurdu. Fizik muayenede sağ testis skrotumdaydı, sol skrotumda şüpheli nubbin palpe edildi. Ultrasonografide karın içinde, iç halka proksimalinde, 15x12x6 mm boyutlarında testis olabilecek bir yapı saptandı. Eksplorasyonda inguinal kanal içerisinde kord, devamında skrotumda yerleşimli epididim bulundu. Processus vaginalis açılarak yapılan eksplorasyonda, testis sigmoid kolon kenarında batın içerisinde bulundu, testisin kord ve epididim ile bağlantısı yoktu. Testis damarlarıyla birlikte serbestleştirilip, skrotuma indirilerek orşiyopeksi yapıldı. Hastanın takiplerinde sorunu olmadı. İnguinal eksplorasyon sırasında normal yerleşimli kord, hatta skrotuma normal olarak inmiş epididim bulunmuş olsa bile, nonuniona bağlı intraabdominal testis olabileceği unutulmamalı ve mutlaka eksplorasyon yapılmalıdır.
