 Torsion of an epididymal cyst is an extremely rare condition. Here we present an 11 year old boy with epididymal cyst torsion which induced the torsion of the epididymis. The patient was admitted our clinic with acute scrotal pain and on surgical exploration a 3 cm necrotic appearing mass was identified attached to the body of left epididymis with 720 degrees torsion. The body of epididymis was also torsed due to the torsion of the mass. After detorsion of epididymis, the mass was excised from epididymis. Histopathologic examination revealed an encapsulated cystic epididymal tissue with features of coagulation necrosis and hemorrhagic infarction. Torsion of an epididymal cyst is one of the rarest causes of acute scrotum in children and to best of our knowledge our presented case is the second epididymal cyst torsion in English literature.

 Epididim kisti torsiyonu oldukça nadir görülen bir durumdur. Burada 11 yaşındaki bir erkek çocukta epididim kisti torsiyonu ve buna sekonder oluşmuş epididim torsiyonu olgusu sunulmuştur. Akut skrotal ağrı ile başvuran hastanın eksplorasyonunda, sol epididimin gövde kısmına bağlı, 3 cm çaplı, 7200 torsiyone olmuş, nekrotik görünümde bir kitle saptandı. Kitlenin torsiyonuna bağlı epididim gövde kısmında da torsiyon mevcuttu. Detorsiyon sonrası, kitle epididime bağlanma bölgesinden eksize edildi. Histopatolojik inceleme sonucunda kitlenin, hemorajik infarkt ve koagülasyon nekrozu içeren, kapsüllü, kistik epididim dokusu olduğu bulundu. Epididim kisti torsiyonu akut skrotumun en nadir sebeplerinden biri olup, bildiğimiz kadarıyla olgumuz, literatürde bildirilen 2. epididim kisti torsiyonudur.
