Concurrent Vas Deferens Agenesis and Ipsilateral Renal Agenesis in a Patient with No Cystic Fibrosis

 Introduction;

Vas deferens agenesis is a relatively rare disorder and may be concurrently seen with seminal vesicle agenesis or ipsilateral renal abnormalities. Diagnosed of Vas deferens agenesis in childhood is usually incidentall. Despite the fact that agenesis of vas deferens has been reported in a few children with cystic fibrosis, a case in the young age group with no cystic fibrosis has not been reported.

Case;

A 6-year-old male patient was brought to our department with symptoms of left inguinal hernia. History revealed left renal agenesis and on physical examination, a left inguinal hernia was detected. Both testes were palpated to be in normal localization and of normal volume. Examination of the other systems was found to be normal. Urinary ultrasonography confirmed left renal agenesis. On left inguinal exploration, a left indirect inguinal hernia and left vas deferens agenesis were observed. The testes, epididymis and vascular structures were normal. A repair procedure was performed for the hernia. The patient was screened for cystic fibrosis gene mutation in the postoperative stage, but no gene mutation related to cystic fibrosis was determined.

Conclusion;

Vas deferens agenesis, a rare disorder in children, is usually described in cases with cystic fibrosis, but it should be considered that this congenital abnormality may also be seen in patients without a diagnosis of cystic fibrosis. Vas deferens agenesis may be encountered during inguinal surgical operations which are frequently performed in pediatric surgery departments. These cases should be investigated in terms of additional pathologies such as cystic fibrosis gene mutation and renal abnormalities.

 Kistik Fibrozis Saptanmayan Olguda Vas Deferens Agenezisi ve İpsilateral  Renal Agenezi Birlikteliği

Giriş:

  Vas deferens agenezisi oldukça nadir olup, seminal vezikül agenezisi veya ipsilateral böbrek anomalileri bu patolojiye eşlik edebilir. Çocukluk çağında vas deferens agenezi tanısı  genellikle  rastlantısal konulur. Birkaç kistik fibrozisli çocuk hastada vas deferens agenezisi  rapor edilmesine rağmen  kistik fibroz birlikteliği olmayan çocuk  olgu rapor edilmemiştir.      

  Olgu:

    6 yaşındaki erkek hasta kliniğimize sol inguinal herni nedeniyle getirildi. Öyküsünde,  daha önce tanımlanmış sol renal agenezisi bulunan hastanın fizik muayenesinde sol inguinal herni mevcuttu.  Her iki testis normal lokalizasyon ve volümde palpe edildi. Diğer sistem muayeneleri normaldi. Yapılan üriner ultrasonografisinde sol renal agenezi teyit edildi . Sol inguinal  eksplorasyonda sol indirekt inguinal herni ve sol vas deferens agenezisi görüldü. Testis, epididimis ve vasküler yapılar normal olarak değerlendirildi. Herni onarımı yapıldı ve postoperatif problemi olmadı. Hasta gen mutasyonu açısından tarandı, ancak Kistik fibrozis gen mutasyonuna rastlanmadı.

    Sonuç ;Çocuklarda nadir görülen vas deferens agenezisi, her ne kadar kistik fibrozisli olgularda tanımlanmışsa da,  kistik fibrozisi olmayan çocuklarda da görülebileceği unutulmamalıdır. Çocuk cerrahisi kliniklerinde sıklıkla uygulanan inguinal cerrahi esnasında vas deferens agenezisi görülebilir. Bu olgularda kistik fibrozis gen mutasyonu ve böbrek anomalisi gibi ek patolojiler yönünden araştırılmalar yapılmalıdır.
