COMPARISON OF TWO DIFFERENT INJECTABLE IMPLANT MATERIAL IN ENDOSCOPIC TREATMENT OF VESICOURETERAL REFLUX IN CHILDREN

Aim:  The aim of this studyis to compare the efficiancy of two different injectable implant material in endoscopic treatment of vesicoureteral reflux (VUR) in children retrospectively.

Patients and Methods: Between 2007-2010, 123 (75 female, 48 male) patients were underwent endoscopic treatment because of VUR. The injectable material were pyrolytic carbon coated beads [PCCB (Durasphere)] (Group I) and dextranomer-hyaluronic acid [DHA (Vurdex, Urodex, Dexell)] (Group II) in 23 (31 ureters) and 100 (136 ureters) patients respectively. On cystoscopy, we evaluated localizationsand anatomical appearances of ureteral orifices. Both groups were compared in terms of efficiancy retrospectively. Fisher’s chi-square test was used to compare the results  and demographic characteristics of both groups and p<0.05 was accepted as a statistical significance.

Results: The mean age of the patients is 7.04 (1-17 years) and 6.33 (1-16 years) years in Group I and Group II respectively. Patients were evaluated according to the localization of reflux, grade of reflux and types of ureteral orifices (Table I). Both groups were similar according to the age, sex, grade of reflux and types of ureteral orifices (p>0.0.5). Mean follow-up period was 1.2 years (6 months - 4 years). In both groups, there were no complications in early postoperative period. There are no stattistically signifficante difference  between two groups in terms of success rate (p>0.05) (Table II). Ureteral reimplantation was performed in 4 ureters and 17 ureters in Group I and Group II respectively.

Rigidity of the catheter, easily blokage of the catheter during the injection and loss of visualisation after the leak of the material were disadvantages of the group I.

Conclusion: Endoscopic injection is a simple, safe and effective treatment of VUR in children. The success rates of PCCB and DHA were similar.

ÇOCUKLARDA VEZİKOÜRETERAL REFLÜNÜN ENDOSKOPİK TEDAVİSİNDE KULLANILAN İKİ FARKLI ENJEKTABLE İMPLANT MATERYALİNİN KARŞILAŞTIRILMASI

Amaç: Bu çalışmada kliniğimizde vezikoüreteral reflünün (VUR) endoskopik tedavisinde uygulanan iki farklı enjektable implant materyalinin etkinlikleri retrospektif olarak karşılaştırılmıştır.

Hastalar ve Yöntem: 2007-2010 yılları arasında 123 hasta (75 kız, 48 erkek) VUR nedeniyle endoskopik olarak tedavi edildi. 23 hastaya (31 üreter) pirolitik karbon kaplı boncuklar [PKKB (Durasphere)] enjekte edildi (Grup I). 100 hastaya (136 üreter) ise Dekstranomer-Hyaluronik Asit [DHA (Vurdex, Urodex, Dexell)] enjekte edildi (Grup II). Sistoskopi esnasında üreterlerin mesaneye giriş lokalizasyonları ve anatomik değerlendirmeleri yapıldı. Her iki grup tedavi etkinliği yönünden retrospektif olarak karşılaştırıldı. Sonuçlar ve her iki grubun demografik özellikleri Fisher’in ki kare testi kullanılarak karşılaştırıldı ve p<0.05 olması istatistiki olarak anlamlı kabul edildi.

Bulgular: Yaş ortalaması Grup I’de 7,04 (1-17 yaş), Grup II’de 6,33 (1-16 yaş) idi. Hastalar reflü lokalizasyonları, reflü dereceleri, üreter orifislerinin tipleri yönünden irdelendi (Tablo I). Her iki grup yaş, cinsiyet, reflü dereceleri ve üreter orifislerinin tipi yönünden birbirine benzerdi (p>0,05). Hastalar ortalama 1,2 yıl (6 ay - 4 yıl) takip edildi. Her iki grupta da postoperatif erken dönemde herhangi bir komplikasyon gelişmedi. Her iki grup arasında başarı oranları açısından istatistiki olarak anlamlı fark saptanmadı (p>0,05) (Tablo II).  Grup I’de 4 üretere, Grup II’de ise 17 üretere reimplantasyon uygulandı.

Her iki materyal ile ilgili uygulama karşılaştırıldığında; kateterin rijit olması, kateterin materyal ile daha kolay tıkanması ve materyal dışarıya kaçtığında görüntünün daha kötü olması Grup I için dezavantaj olarak görülmektedir.

Sonuç: ÇocuklardaVUR tedavisinde endoskopik tedavi etkili ve güvenli bir yöntemdir. PKKB ve DHA enjeksiyonunun başarı oranı birbirine benzerdir.
