Our Experiences with Endoscopic Dextranomer/hyaluronic acid copolymer (Deflux) Injection

Purpose:  We present the results of endoscopic use of dextranomer/hyaluronic acid copolymer (Deflux(R)) for VUR  grade I I-V in children.

Patients and Methods: Between  January  2007 and March  2011, 73 children (51 female, 22 male)  underwent  133 endoscopic  subureteral  injection of Deflux(R)  as an outpatient procedure. The median age was  5.7 years  (12 months  to 17 years). Reflux was  unilateral in 46 cases(30 left side, 16 right side)  and bilateral in 27, affecting 100 ureters. Reflux grades was II to V in 10 (10%) cases,  51 (51%),  19 (19%) and 20 (20%), respectively.  All patients  have been followed for 3 months to 2 years. Three months postoperatively a voiding cystourethrogram (VCUG) was performed. Second and third injections were performed  for the patients who had not resolved reflux after first injection of deflux. 29 ureters  had second and  2 ureters had third deflux injections. One unilateral reflux was associated with ureteropelvic junction (UPJ)obstruction,  one patient had multicyctic displastic kidney on contrlateral side. One grade V reflux was following ureterocel incision. Three reflux were associated with double collecting system of which one was on the upper pole and other on the lower pole. One grade III reflux was on the pelvic kidney . Five patients had bladder  sphincter  dyssynergy with persistant reflux and nine of 100 ureters had ureteral dysplacement.

Results:  The reflux was corrected in 46 (46%) of the 100 ureters after a single injection, and resolved after a second and third injection in 22 (22%) and 1 (1%) ureter, respectively. The overall cure rate per grade was 100% for grade II, 76.4 % for grade III, 57.8% for grade IV and 45 % for grade V. No intra or postoperatively untoward  effects  were seen in any of these patients with the use of dextranomer/hyaluronic acidcopolymer as an injectable material.

Conclusion:The endoscopic treatment of VUR with Deflux is a feasible outpatient procedure, requires minimal operating room time and is associated with low morbidity. Double collecting system, ureteral dysplacement , bladder sphincter  dyssynergy  and high grade of reflux had  adverse effects on cure rates.

Giriş: Vezikoüreteral reflü (VUR) saptanan hastalarda (2.-5.derece) endoskopik dekstranomer/hiyaluronik asid kopolimer( Deflux ) enjeksiyonu tedavisinin sonuçlarını sunmak.

Gereç ve Yöntem:Ocak 2007-Mart 2011 tarihleri arasında 73 çocuğa (51 kız ,22 erkek) toplam 133 endoskopik subüreteral  Deflux enjeksiyonu günübirlik prosedür olarak uygulandı. Hastaların ortalama yaşı 5.7 y (12 ay-17 yaş ) idi. Reflü 46 hastada tek taraflı (30 sol, 16 sağ) ve 27 hastada çift taraflı idi. Etkilenmiş üreter sayısı 100 idi. On  hastada 2. derece (%10), 51 hastada 3. derece (%51), 19 hastada 4. derece (%19) ve 20 hastada 5. derece (%20) VUR tespit edildi. Bütün hastalar 3 ay ila 2 yıl arasında takip edildi. Deflux enjeksiyonundan 3 ay sonra voiding sistoüretrografiler çekildi. Birinci enjeksiyonla düzelme sağlanamayan hastaların bir kısmına ikinci (29 üreter) ve üçüncü (2 üreter) enjeksiyonlar yapıldı. Tek taraflı reflüsü olan 1 hastada aynı tarafta üreteropelvik bileşke obstrüksiyonu, bir diğer hastada karşı tarafta multikistik displastik böbrek vardı. Beşinci derece reflüsü olan bir hastada reflü üreterosel insizyonundan sonra gelişti. Üç hastada reflü çift toplayıcı sistem ile birlikte idi. Bunlardan birisi üst diğeri alt pol reflüsü idi. Bir hastada pelvik ektopik böbrekte 3. Derece reflü vardı. Beş hastada inatçı reflü ile birlikte mesane sfinkter uyumsuzluğu vardı ve 100 üreterin dokuzunda üreteral displacement vardı.

Bulgular:Tek enjeksiyondan sonra 100 üreterin 46 sinde (% 46) VUR da düzelme görüldü. İkinci enjeksiyonda 22 üreterde (%22), üçüncü enjeksiyonda 1 üreterde (% 1) VUR da düzelme görüldü. Tüm enjeksiyonlar sonunda başarı oranı   ikinci derece reflülerde %100, üçüncü derece reflülerde %76.4, dördüncü derece reflülerde %57.8, beşinci derece reflülerde %45 idi. İşlem esnasında ve sonrasında hiçbir hastada deflux kullanımına bağlı istenmeyen bir etki gözlenmedi.

Sonuç:Endoskopik Deflux enjeksiyonu VUR tedavisinde günübirlik uygulanabilen, ameliyat süresi kısa olan güvenilir bir yöntemdir. Çift toplayıcı sistem varlığı, üreteral displacement, mesane sfinkter uyumsuzluğu ve yüksek derecede reflü varlığı başarı sonuçlarını olumsuz etkilemektedir.
