Congenital Prepubic Sinus : Case Report

Congenital prepubic sinus is an uncommon congenital anomaly of the urinary tract, which is associated with discharge from an opening overlying the symphysis pubis, and affects both the males and females. We report an additional case with congenital prepubic sinus.
A 3-years-old boy presented with a history of intermittently discharging purulent fluid from a prepubic midline orifice in the radix of the penis. He had no history of urinary tract infection or any other symptom related to genitourinary tract. Fistulography revealed a noncommunicating tract toward the bladder neck. The surgical exploration justified a 5-cm long blind ending fistula, caudal to the inferior edge of the pubis. Simple excision was performed. Postoperative period was uneventful. Histological examination of the specimen found transional and stratified squamous epithelial lining, and smooth muscle cells within the sinus.
The anatomic and histopathological features of the congenital prepubic sinus are well established, but the etiology remains controversial. One of the most favored theories is anterior abdominal wall closure defect and the other is urethral development anomaly that could be a variant of dorsal urethral duplication. We favor that the present case is a part of dorsal urethral duplication Stephan’s classification type 3, which describes a sinus that simulates an accessory urethra and tract from the base of the penis behind the symphysis pubis and in front of the bladder toward the umbilicus. Also, we postulate that complete surgical excision of the fistula is crucial, whatever the etiology.

Konjenital prepubik sinüs, simfisis pubis üzerinde yerleşmiş akıntı bir açıklık olup,erkek ve kızlarda görülen nadir bir konjenital anomalidir. Konjenital prepubik sinusü olan yeni bir olgu sunuyoruz.
3 yaşında erkek hasta penis radiksinde aralıklı prülan akıntı öyküsü ile başvurdu. Üriner enfeksiyon öyküsü ve herhangi bir genitoüriner yakınması yoktu. Fistülografisinde mesane boynuna doğru uzanan, kör sonlanan bir fistül traktı izlendi. Cerrahi ekplorasyon 5 cm uzunluğunda, kör sonlanan, pubisin inferior kenarının kaudaline uzanan bir fistülü doğruladı. Cerrahi eksizyon uygulanan hastanın postoperatif dönemde sorunu olmadı. Spesmenin histopatolojik incelemesinde sinüs duvarının transizyonel ve çok katlı yassı epitelden oluştuğu ve düz kas hücreleri içerdiği görüldü.
Konjenital prepubik sinüsün anatomik ve histopatolojik özellikleri açıkça ortaya konmuş olsa da etyolojisi halen tartışmalıdır. En çok kabul gören teorilerden biri, karın ön duvarı kapanma defektiyken diğeri ise dorsal üretral duplikasyonun bir varyantı olduğu teorisidir. Biz bu olgunun penis tabanında pubisin arkasından başlayıp, mesane önünden umblikusa uzanım gösteren Stephan tip 3 sınıflamasına giren dorsal üretral duplikasyonun bir varyantı olduğunu düşünüyoruz. Ayrıca, etyoloji ne olursa olsun, fistül traktının tamamının cerrahi olarak çıkarılması gerektiğini düşünmekteyiz.
