ENDOSCOPIC TREATMENT OF VESICOURETERAL REFLUX WITH POLYACRYLATE POLYALCOHOL COPOLYMER IN CHILDREN: PROMISING PRIMARY RESULTS

Purpose: In the last decade, endoscopic treatment (ET) of vesicoureteral reflux (VUR) has moved forward in the algorithm of VUR management especially with the evolution of non-biodegradable synthetic materials. Reports indicate that there is a strong tendency to inject more volumes of bulking agents for more successful results in literature. In this study, we aimed to present short term results of ET of VUR with a new bulking agent with high molecular mass.

Material-Methods: From August 2009, all VUR patients assigned for endoscopic treatment underwent subureteric injection with polyacrylate polyalcohol copolymer (PPC, Vantris®) in our institution. Hospital records of all patients are reviewed and demographics, symptoms, characteristics of ureters and bladders, injected volumes and postoperative symptoms with VCUG controls were evaluated.

Results: A total of 49 patients (25 girls, 24 boys) with a mean age of 6.12 years (1 month – 20 years) were included in the study. Febrile urinary tract infections (69.5%) were the major complaints. Fifty six percent of the patients aged above 5 years (n=30) had urinary incontinence and 7 (14.5%) male patients had antenatal hydronephrosis. In 23 (46.9%) children, VUR was secondary to bladder-sphincter dysfunctions (16) and posterior urethral valves (7). VUR was unilateral in 28 and bilateral in 21 cases. Thirty-five of the total 70 (50%) injected ureters had high grade (4°-5°) VUR. The mean injected volume of bulking agent was 0.98±0.58 ml. Reflux disappeared in 81.4% (57/70) of the ureters with a single injection at postoperative 3-9 months’ VCUG controls while it downgraded to grade 1-2 in 6 (8.5%). At the end of a mean follow-up of 11.2 months, VUR recurred in 1 (1.4%) and contra-lateral VUR developed in 6 ureters. In postoperative period, symptoms and urinary infections persisted in 19.1% and 12.7% of the patients, respectively. No complications occurred during the follow-up period.

Conclusion: Initial results of our complex group of patients suggest that PPC may be used safely and satisfactorily for correction of VUR in children. Injection volume and therefore the cost seem reasonably low even in high grade or complicated secondary reflux cases. Further prospective randomized controlled studies and long-term outcome data of these patients are needed.

ÇOCUKLARDA VEZİKOÜRETERAL REFLÜNÜN POLİAKRİLAT POLİALKOL KOPOLİMER İLE ENDOSKOPİK TEDAVİSİ: UMUT VAADEDEN ERKEN SONUÇLAR

Giriş: Son on yılda, özellikle vücut tarafından emilemeyen bio-sentetik materyallerin gelişmesiyle, vezikoüreteral reflünün (VÜR) endoskopik tedavisi bu hastalığın tedavisinde giderek daha fazla öne çıkmaktadır. Diğer yandan literatürde uygulanan enjeksiyon materyallerinin miktarlarının artmakta olduğuna da dikkat çekilmektedir. Bu çalışmada VÜR’ nün endoskopik tedavisinde kullanılmaya başlanan yüksek moleküler hacimli yeni bir materyalin kısa dönem sonuçlarının sunulması amaçlanmıştır.

Materyal-Metot: Ağustos 2009-Aralık 2010 tarihleri arasında kliniğimizde endoskopik olarak tedavi edilen VÜR hastalarına poliakrilat polialkol kopolimer (PPK, Vantris®) enjeksiyonu uygulandı. Olguların hastane kayıtları gözden geçirilip demografik özellikleri, şikayet ve bulguları, ureter ve mesane özellikleri, enjeksiyon miktarları ile postoperatif şikayet ve kontrol işeme sistoüretrografisi (İSUG) sonuçları değerlendirildi.

Bulgular: Yaş ortalaması 6,12±4,65 (1 ay-20 yaş) olan toplam 49 hasta (25 kız, 24 erkek) çalışmaya dâhil edildi. Ateşli idrar yolu enfeksiyonu en sık şikâyetti (%69,5) ve 5 yaş üzeri olguların (n=30) %56’sında üriner inkontinans, 7 erkek hastada (%14,2) antenatal hidronefroz öyküsü mevcuttu. Yirmi üç hastada (%46,9) VÜR mesane-sfinkter disfonksiyonu (16) ve posterior üretral valve (7) bağlıydı. 28 unilateral, 21 bilateral olmak üzere toplam 70 üreterin 35’ inde (%50) VÜR yüksek dereceli (4-5) idi. Uygulanan enjeksiyon materyali miktarı üreter başına 0,98±0,58 ml idi. Üçüncü-dokuzuncu aylarda yapılan kontrol İSUG’ larda tek seansta tam başarı oranı %81,4 (57/70), kısmi başarı (1-2. dereceye gerileme) oranı ise %8,5 (6/70) idi. Ortalama 11,2 (4-16 ay) aylık izlem süresince sadece 1 (%1,4)  üreterde VÜR’ ün tekrarladığı, 6 üreterde de kontralateral VÜR geliştiği gözlendi. Postoperatif dönemde şikayet ve üriner enfeksiyon oranları sırasıyla %19,1 ve %12,7 idi. İzlem süresince herhangi bir komplikasyon gelişmedi.

Sonuç: Komplike hasta grubumuzda dahi sonuçlar PPK’ in çocuklarda VÜR’ nün endoskopik tedavisinde güvenle ve başarıyla kullanılabileceğini göstermektedir. Uygulanan enjeksiyon miktarı ve buna bağlı olarak maliyet yüksek dereceli veya sekonder VÜR olgularında bile düşüktür. Bu konuda prospektif randomize kontollü çalışmalara ve uzun dönem sonuçlara ihtiyaç vardır.
