Ureteral stones, stenosis and diverticulum in a four years old boy: Case report

Introduction: Ureteral  diverticula is a rare pathology in childhood period. It can be presented by urinary tract infection or stone formation. We present our case who became symptomatic for lower ureteral stones and to discuss the charachteristics of incidentally found ureteral diverticula and stenosis during the operation.

Case report: A four years old boy was referred from another hospital for abdominal pain for 45 days and non-opaque lower ureteral stones, grade 3  hydronephrosis and non-functioning kidney. Percutaneous nephrostomy catheter was introduced to the left kidney under ultrasound guidance.. Sufficient amount of urinary drainage was seen postoperatively. An operation was performed one week after the drainage, and when the middle ureter was opened a web type thickening which was partially obstructing and narrowing the lumen distally and a second orifice with a ‘Y’ type bifurcation at the proximal localization was seen. When it was filled with opaque solution a diverticular structure of  3-4 cm long as a blind pouch was seen radiographically. Two stones in the sizes of  1x1,5 cm and 1x2 cm were removed from the lower ureter and a wedge excision was performed for the distal stenosis and also the common wall between the diverticula and the ureter near the orifice was partially excised as invert ‘V’ type, to enlarge the orifice for easier drainage. The patient was discharged after 15 days of follow up and all the catheters were removed when we saw the passage of opaque material to the bladder at antegrade pyelography.

Conclusion: Ureteral diverticula is a rare condition generally seen in elderly people and sometimes clinically presented with stone formation. It may be congenital or acquired and very rare in children. It is generally seen as paraureteral bladder diverticula and becomes symptomatic as urinary tract infection. We preferred to improve the drainage by partial excision of the common wall in order to excise the whole diverticıula because of long-term obstruction, ureteral dilatation, periureteral fibrosis, renal function loss and for no stone location in the diverticula. Distal thickening may be congenital or due to  long-term obstruction and fibrosis. We presented this case for very rare occurrence in childhood, may be associated with stone formation and for the different treatment approaches.

Giriş:Üreteral divertikül çocukluk çağında çok nadir olan bir patolojidir. İdrar yolu enfeksiyonu veya taş oluşumu ile bulgu verebilir. Üreter alt uç taşları nedeniyle semptomatik olan bir olgumuzu operasyon sırasında tesadüfen saptanan divertikül ve üreter darlığı nedeniyle özelliklerini tartışmak  amacıyla sunduk.

Olgu sunumu:Dört yaşında erkek olgu 45 gündür süren karın ağrısı yakınması ile başvurduğu dış merkezden sol üreter alt uçta iki adet non-opak taş, sol grade 3 hidronefroz ve sol böbrekte fonksiyon kaybı tanılarıyla kliniğimize refere edildi. Sol böbreğe ultrason eşliğinde perkütan nefrostomi kateteri yerleştirildi. Kateterden yeterli miktarda idrar çıkışları olan olguya 1 hafta sonra yapılan operasyon sırasında üreter orta kısmında distale doğru üreter lümenini daraltan ve parsiyel bir obstrüksiyon yapan web tarzı bir kalınlaşma ve proksimalde ‘Y’ şeklinde bir çatallanma şeklinde ikinci bir orifis  varlığı görüldü. Opak madde ile grafi çekildiğinde bunun 3-4 cm’lik kör bir poş şeklinde divertiküler bir yapı olduğu saptandı. Üreter alt uçtan 1x1,5 cm ve 1x2 cm boyutlarında iki adet taş çıkarıldıktan sonra distaldeki darlık wedge şeklinde eksize edildi ayrıca  proksimaldeki divertiküler yapı ile gerçek üreter arasındaki ortak duvara ters ‘V’ şeklinde parsiyel eksizyon uygulanarak orifis genişletilip daha kolay drene olması sağlandı. Onbeş günlük izlemden sonra antegrad pyelografi ile mesaneye opak madde geçişi gözlenen olgu stent ve kateterleri çıkarıldıktan sonra taburcu edildi.

Sonuç: Üreteral divertikül daha ziyade yaşlılarda görülen ve zaman zaman taş oluşumu ile birlikte  klinik bulgu veren nadir bir durumdur. Konjenital veya edinsel olabilir. Çocuklarda ise çok nadirdir. Genellikle paraüreteral mesane divertikülleri şeklinde görülür ve idrar yolu enfeksiyonu ile belirti verir. Uzun süreli obstrüksiyon, üreteral dilatasyon ve periüreteral fibroz, içerisinde taş olmaması ve renal fonksiyon kaybı nedenleri ile divertikül eksizyonu yerine ortak duvara uygulanan minimal parsiyel eksizyon ile drenajı arttırıcı operasyon ve izlemi tercih ettik. Distaldeki kalınlaşma uzun süreli obstrüksiyon ve fibrozis ile ilişkili olabilir. Çocukluk çağında çok nadir görülmesi, taş oluşumu ile ilişkili olabileceği ve değişik tedavi yaklaşımı nedeniyle olgumuzu sunduk.
