A syndromic patient with cryptorchidism, urethral chordee, hypospadias: Wolf-Hirschhorn syndrome or variation?

Introduction:Urogenital anomalies may be associated with certain syndromes and chromosomal abnormalities. By investigating similarity with the cases in the literature, we aimed to present a patient with urogenital anomalies that are associated with finding oculo-cerebro-skeletial.

Case: A six year old male patient with findings of bilateral undescended testeses, glandular hypospadias and ventral chordee admited to our outpatient clinic. The patient’s  other findings were right microphtalmia , bilateral coloboma with microcornea, pes cavus deformity and slight pectus excavatum. There were sulci enlargement and cortical atrophy in right parieto frontotemporal  region, microphtalmia  in right, bilateral microcornea-coloboma and suspect hypophyseal  atrophy in MR study. Furthermore, routine biochemical tests, abdominal ultrasound, hearing test, electrocardiogram and echocardiogram were normal. Abnormalities weren’t determined in both cytogenetic examination and FISH analysis with 4p probe. Because It was cosidered that dysmorphic findings would be associated with a chromosomal syndrome, the patient's blood sample was sent to a private labaratory for molecular karyotyping  as a further investigation. The costs of this analysis were met by our. The test results did not come,yet. As the first stage of treatment, patient’s  undescended testes, hypospadias and chordee repaired.

Conclusion:İn the same cases of  the literature ,It is said that such a syndrome would be caused by environmental, hereditary factors and gestational infections. Even If our patient seems to similar with such as Wolf-Hirschhorn, Lenz mikrophtalmia syndromes, the current findings is incompatible with the any syndrome. As a result, according to the results of molecular karyotyping on this interesting case we plan to carry on our study.

Kriptorşitizm, üretral kordi, hipospadiaslı sendromik bir hasta: Wolf-Hirschhorn Sendromu veya varyasyonu ?

Giriş: Ürogenital anomaliler bazı sendrom ve kromozomal anomalilerle birlikte görülebilir. Biz, literatürdeki olgularla benzerliğini araştırarak, okulo-serebro-iskeletal bulgularla birlikteliği olan ürogenital anomalili bir hastayı  sunmayı amaçladık.

Olgu: 6 yaşında erkek hasta bilateral inmemiş testis, glanuler hipospadias ve ventral kordi bulgularıyla polikliniğimize başvurdu (Şekil-1). Hastadaki diğer bulgular; sağ mikrooftalmi, bilateral mikrokornea - kolobom, pes kavus deformitesi ve hafif pektus ekskavatum idi(Şekil-2). Hastanın beyin MR’sinde sağ parieto frontotemporal sulkuslarda genişleme ve kortikal atrofi, sağ mikrooftalmi-kolobom, şüpheli hipfofiz atrofisi  vardı.Ayrıca,rutin biokimyasal testler,abdominal  USG , İşitme testi,elektrokardiogram  ve kalp ekosu normaldi. Sitogenetik inceleme ve 4p probu ile yapılan FISH incelemesinde anomali saptanmadı. Dismorfik bulguların kromozomal bir sendromla ilişkili olabilecegi düşünüldüğünden, hastanın kan örneği  ileri bir  tetkik olarak  molekuler karyotipleme için özel bir laboratuara gönderildi. Bu testin giderleri tarafımızca karşılandı. Henüz test sonuçları  gelmedi. Tedavinin ilk aşaması olarak, hastanın inmemiş testis, hipospadias ve kordisi onarıldı.

Sonuç: Literatürdeki benzer olgularda böyle bir sendroma çevresel,kalıtımsal faktörlerin ve gestasyonel enfeksiyonların neden olabileceği söylenmektedir. Hastamız Wolf-Hirschhorn, Lenz mikrooftalmi gibi sendromlarla benzer gibi görünse de, mevcut bulgular hiç bir sendroma uymamaktadır. Sonuç olarak, moleküler karyotipleme sonucuna göre bu ilginç vaka üzerine çalışmalarımızı devam ettirmeyi planlıyoruz.
