Aim: Noncommunicating hydroceles (NCHs) are tight and large hydroceles, because the processus vaginalis closes behind the hydrocele and the secretion of peritoneal fluid continues. Our aim is to prove the injury in the testis and the necessity of early surgical procedure.  

Materials and methods: 13 testes with tight NCH (8 cases) were used in our study. 5 of these were bilateral NCHs and 3 of them were unilateral NCHs. Surgical procedures were performed while the cases were medium 6.5 months age (10 days-22 months). Testes findings were recorded during the operations. Durations of the follow-up were medium 37 months (2 months-4 years).   

Results: 4 of 13 testes (30%) were rather cyanotic depending chronic ischemia. It was identified that the sizes of 7 testes (54%) together with cyanotic testes were too smaller than normal testes. The sizes of 4 of 10 testes (40%) were very smaller than contralateral testes according to ultrasonograhpic findings at the end of follow-up.

Conclusions: Although medium operation age was six and half months in 13 testes with NCHs, we believe that surgical procedure should be performed before 6 months old in NCHs to prevent or decrease testicular injury when we consider these findings.

Giriş/Amaç: Nonkominike hidroseller, proksimaldeki prosessus vajinalisin kapanması ve kese içinde periton sıvısının salgılanmaya devam etmesi nedeniyle gergin ve büyük hidrosellerdir. Amacımız bu tip hidrosellerde testiste oluşan hasarı ortaya koyarak ameliyat zamanının öne alınması yönündeki  gerekliliği  ortaya koymaktır.

Gereç/Yöntem: Çalışmaya gergin nonkominike hidroselli  13 testis (8 olgu) dahil edildi. Bunların 5’i bilateral, 3’ü ise unilateral nonkominike hidrosel idi. Olgular ortalama 6,5 aylık (10 gün-22 ay arasında) iken ameliyat edildi. Ameliyatta testis bulguları kayıt edildi. Takip süreleri ortalama 37 ay (2 ay-4 yıl arasında) idi.

Bulgular: Ameliyat anında 13 testisin 4’ü (%30) kronik iskemiye bağlı olarak belirgin bir şekilde siyanotik idi. Siyanotik testislerle birlikte toplam 7 testisin (%54) boyutlarının normalden belirgin bir şekilde küçük olduğu tespit edildi. Ortalama 37 aylık takip sonucunda ise takip edilen 10 testisten 4’ünün (%40) ultrason bulgularına göre karşı testise kıyasla belirgin bir şekilde boyutlarının küçük olduğu belirlendi.

Sonuç: Nonkominike hidroselli 13 testiste ortalama ameliyat yaşı 6,5 ay olmasına rağmen elde ettiğimiz bu sonuçlar göz önüne alınacak olursa gergin nonkominike hidrosellerin testiste oluşan hasarları minimale indirmek veya önlemek için ilk 6 aydan önce ameliyat edilmesinin daha yararlı olacağını düşünüyoruz.
