RISK FACTORS FOR THE DEVELOPMENT OF END STAGE RENAL DISEASE IN CHILDREN WITH POSTERIOR URETHRAL VALVE

Aim: To determine the risk factors (RF) for end stage renal disease (ESRD) in children with posterior urethral valve (PUV).

Patients and methods: The medical records of 133 patients treated for PUV between 1990-2012 were reviewed retrospectively and, 81 had sufficient records with a mean follow-up of 7,7 years (r: 1,5-20 years). Potential RF for ESRD; prenatal diagnosis, serum creatinine level one year after treatment, initial DMSA scan, urinary tract infections (UTI) before treatment were statistically analysed (Chi-square, Fisher’s extract and McNemar test).

Results: ESRD was encountered in 17 of 81 patients (21%), 4 with prenatal diagnosis and 13 without (p>0.05). One year after treatment, serum creatinine remained high in 19 patients and ESRD  developed in 16 (84%) while ESRD was diagnosed in only 1 (2%) with normal serum creatinine levels (n: 62) (p<0.001).  The incidence of ESRD was 89% (n: 8) if there was significant bilateral cortical defects in the first DMSA scan (n:9) and 3% (n: 1) if there wasn't (n: 30) (p<0.001). ESRD was diagnosed in 8 patients (19%) with unilateral cortical defects with differential renal function <40% (n:42). Before the treatment of PUV, recurrent UTI was present in 56 patients, absent in 25 and ESRD was diagnosed in 17 of 56 (30%) (p<0.001).

Conclusion: Higher serum creatinine levels one year after treatment of posterior urethral valv, presence of bilateral multiple cortical defects on the first DMSA scan and presence of recurrent urinary tract infections before  the treatment are risk factors for end stage renal disease in children with posterior urethral valve. 

POSTERİOR ÜRETRAL VALVLİ ÇOCUKLARDA SON DÖNEM BÖBREK YETMEZLİĞİ GELİŞİMİNDE RİSK FAKTÖRLERİ

Amaç: Posterior üretral valvli (PUV) çocuklarda son dönem böbrek yetmezliği(SDBY) gelişimi için risk faktörlerini(RF) değerlendirmektir.

Olgular ve Yöntem: 1990-2012 yılları arasında PUV nedeni ile tedavi edilen 133 olgunun tıbbi kayıtları geriye dönük olarak incelendi. Kayıtları eksiksiz olan 81 olgu değerlendirildi. Ortalama takip süresi 7,7 yıl(1,5-20 yıl) idi. SDBY için potansiyel RF; prenatal tanı, tedavinin 1. yılı sonundaki serum kreatinin değeri, başlangıç DMSA görüntülemesi, tedavi öncesi idrar yolu enfeksiyonu (İYE) sayısı  istatistiksel olarak değerlendirildi. (Chi-square,Fisher’s extract ve McNemar testi)

Bulgular: SDBY 81 olgunun 17’sinde (%21) saptandı. Bu olguların 4’ü prenatal tanılı idi, 13’ünün prenatal tanısı yoktu (p>0.05). Tedavinin 1. yılında serum kreatinin değeri yüksek kalan 19 olgunun 16’sında (%84) SDBY gelişirken, serum kreatinin seviyesi normal olan 62 olgunun birinde (%2) SDBY görüldü (p<0.001). İlk DMSA görüntülemesinde bilateral kortikal tutulum bozukluğu olan 9 olgunun 8'inde (%89) SDBY gelişirken, tutulum bozukluğu olmayan 30 olgunun birinde (%3) SDBY gelişti  (p<0.001). Differansiyel renal fonksiyonu < %40 olan, unilateral kortikal tutulum bozukluğu olan 42 olgudan 8'inde (%19) SDBY saptandı. PUV tedavi öncesi tekrarlayan İYE 56 olguda vardı ve 25 olguda yoktu. Ellialtı olgunun 17’sinde (%30) SDBY izlendi (p<0.001).

Sonuç: Çocuklarda, PUV'in tedavisinden bir yıl sonraki yüksek serum kreatinin seviyesi, ilk DMSA görüntülemesinde bilateral kortikal tutulum bozukluğu ve tedavi öncesi tekrarlayan İYE varlığı, SDBY’ne gidişte etkili risk faktörleridir.
