Our experiences in cases with hydronephrosis due to congenital ureteral stenosis

Purpose: Congenital ureteral stenosis is a rare cause of the urine flow impairment. The aim of this study was to discuss our experience with the management of four cases with hydronephrosis due to congenital ureteral stenosis.

Methods: Clinical records of the patients who had surgery for congenital ureteral obstruction between September 2010 and January 2014 were analysed retrospectively. Demographic, diagnostic, and operative findings were analised.

Results: Three male patients presented with antenatally detected unilateral hydronephrosis. In the fourth patient, who presented with urinary incontinence, hydronephrosis was detected incidentally when she was six years old. Obstruction was found in the right ureter in one patient and in the left in three patients (75%). Initially, all patients who had antenatal diagnosis were treated conservatively. Urinary ultrasonography, dynamic renal MAG3 scintigraphy, voiding cystourethrography and preoperative cystoscopy were done in all patients. Surgical indications of the patients were worsening of the hydronephrosis/hydroureteronephrosis, thinning of the parenchyma and poor function of involved side. Retrograde pyelography was done in one patient and it showed midureteral stenosis and significant proximal hydroureteronephrosis. Operations of these three patients, who had antenatal diagnosis, were done without any complications when they were 2, 10 and 21 months of age. Ureteroureterostomy was performed in the three patients with midureteral obstruction. One patient who had proximal ureteral obstruction treated with dismembered pyeloplasty. Double-J ureteral stent was used as internal drainage in all patients for 6 weeks.

Conclusions: Urinary ultrasound and dynamic renal scintigraphy have been sufficient for the diagnosis, as well as planning of the incision site in the patients with ureteral stenosis. Retrograde pyelography is useful in the diagnosis and the planning of the surgery especially in complex and comorbid patients. In the treatment of the ureteral stenosis individualization of the surgery, depending on the level of the stenosis, is important.

Konjenital üreteral darlık nedeniyle gelişen hidronefroz olgularında deneyimlerimiz

Amaç: Konjenital üreteral darlık, üriner sistem obstrüksiyonunun nadir bir sebebidir. Bu çalışmanın amacı, konjenital üreteral darlık olan dört hidronefroz olgusunda tanı ve tedavi sürecini tartışmaktır.

Yöntem: Kliniğimizde Eylül 2010 ile Ocak 2014 tarihleri arasında konjenital üreter tıkanıklığı nedeniyle opere edilen hastaların kayıtları geriye dönük olarak incelendi. Hastaların demografik, tanısal ve operatif bulguları analiz edildi.

Sonuç: Serimizdeki üç erkek hastanın antenatal tanısı varken, dördüncü hastada hidronefroz 6 yaşında iken idrar kaçırma şikayeti ile başvurusu sonucunda rastlantısal olarak saptandı. Obstrüksiyon bir hastada sağ tarafta iken üç hastada sol taraftaydı (%75). Başlangıçta antenatal tanılı hastalar konservatif olarak tedavi edildi. Bütün hastalarda üriner sistem ultrasonografisi, MAG3 dinamik böbrek sintigrafisi, işeme sistoüretrografisi ve preoperatif sistoskopi yapıldı. Cerrahi endikasyonlar hidronefrozun artması, parankimde incelme olması ve etkilenen taraftaki böbrekte fonksiyonun azalmasıydı. Retrograd piyelografi yapılan bir hastada midüreteral darlık ve proksimalde ciddi hidroüreteronefroz saptandı. Antenatal tanılı üç hastanın ameliyatları, hastalar 2, 10 ve 21 aylık iken herhangi bir komplikasyon olmaksızın yapıldı. Midüreteral darlık olan 3 hastada üreteroüreterostomi yapılırken, proksimal üreterde darlık olan bir hasta dismembered piyeloplasti ile tedavi edildi. Tüm hastalarda internal drenaj için double-J stent 6 hafta süreyle kullanıldı.

Tartışma: Üreter darlıklarının tanısında ve ayrıca insizyon seçiminde üriner ultrason ve dinamik böbrek sintigrafisi yeterlidir. Retrograd piyelografi özellikle kompleks ve ek anomalisi olan hastalarda tanıda ve cerrahinin planlanmasında faydalıdır. Üreter darlıklarının tedavisinde cerrahi yaklaşımın obstrüksiyonun seviyesine göre seçilmesi önemlidir.
