Factors affecting SWL treatment in children with renal stones: May age be predictor its success?

Objective: To investigate the effectiveness of SWL in the treatment of children with renal stones and factors affecting its outcome.

Material and Method: Treated with SWL due to renal stones, 44 children were retrospectively evaluated between February 2012 and February 2014. Patients' demographics,  stone number, stone size and location in kidney, opacity of stones, placement of JJ stent, SWL shocks' number and energy stonefree rate and complications were also noted. Extracted stone specimens were transmitted for stone analysis.

 Results: Cases consisted of 24  boys and 20 girls. While mean age was 3.8 years (8 m-16 years), 20 were ≤ 3years, and 40 were ≤10 years. Though bilateral in 3 cases, stone location was at right in 17 and left in 24 cases as total 47 renal units. While 16 stones were located in renal pelvis, 31 were in calixes. Of renal pelvic stones, 27 were single and 20 multiple. Mean stone size was 11.56 mm (5-36 mm). Mean SWL shocks waves were 1163 (500-2000), and mean energy was 11.88kV (10-14kV).  After seances 1, 2 and 3, rates of stone-free and insignificance (≤4mm) stones were 42.5, 77.7 and 93.2%, respectively. Of 47 renal units undergoing seance 1, 27 cases underwent seance 2; among those undergoing seance 2, 10  cases were exposed to seance 3. JJ stents were placed in 33 (70%) renal units. While SWL was performed as adjuvant for URS in 27, primary SWL was performed in 20 cases. Familial stone history was present in 15 cases. As postoperative complications, urinary tract infection was seen in 2, hypertermia in 1, vomitting in 1 and hematuria in 9 cases. No statistically significant difference was detected between those with and without opaque stones; multiple and single stones; those with stone location in pelvis and calixes; those with and without familial stone history; and, those with stone sizes between ≤10 mm and ˂10 mm. Although no statistically significant difference was observed between those at ages of ≤3years and ˂3years, a significant difference was seen as to the need for seance 3.

Conclusion: SWL is an effective, safe and minimally invasive treatment modality. Due to its significant less requirement for seance 3, SWL can be used safely in children at ages of  3 and lower.

 

Çocuk Böbrek Taşlarında SWL Tedavisini Etkileyen Faktörler: Yaş Başarıda Belirleyici Olabilir mi?

Amaç: SWL'nin çocuk böbrek taşlarının tedavisindeki etkinliğinin ve sonucu etkileyen faktörlerin araştırılmasıdır.

Gereç ve Yöntem: Böbrek taşı nedeni ile Şubat 2012 ile Şubat 2014 tarihleri arasında SWL uygulanan 44 çocuk retrospektif olarak değerlendirildi. Olguların demografik yapıları, taşın böbrekteki yeri, büyüklüğü, sayısı, opaklığı, JJ stent uygulaması, SWL seans sayısı, şok dalga atım sayısı, uygulanan enerji miktarı, taşsızlık oranı ve komplikasyonlar değerlendirildi. Elde edilen taşların analizleri yapıldı.

Bulgular: Olguların 24'ü erkek, 20'si kız idi. Ortalama yaş 3.8yıl (8ay-16y) iken 20'si ≤ 3yaş,40'ı≤10yaş idi. Olguların 3'ünde taş bilateral yerleşirken 17'de sağ 24'te sol renal yerleşimliydi, toplam 47 renal üniteydi. Pelviste yerleşimli taş 16 renal ünitede iken, kalikslerde yerleşen 31'di. Renal ünitelerdeki taşların 27'si tek, 20'si çokluydu. Taş boyutu 11.56mm (5-36mm) idi. Ortalama şok dalga atım sayısı 1163 (500-2000) iken ortalama enerji 11.88kV (10-14kV) uygulanmıştır. 1. seans sonucu taşsızlık ve önemsiz taş (≤4mm) oranı %42.5 iken 2.seans sonucunda %77.7 ve 3. seans sonucunda %93.2 tespit edildi. 1. seans uygulanan 47 renal ünitenin 27'sine 2. seans ve 2. seans uygulanan ünitelerin 10 tanesine de 3. seans uygulanmıştır. Renal ünitelerin 33'üne (%70) JJ stent yerleştirilmiştir. SWL 27 olguda URS'ye adjuvan olarak yapılırken, 20 olguda primer SWL uygulanmıştır. Ailede taş hikayesi 15 olguda mevcuttu. Postoperatif komplikasyon olarak 2 olguda idrar yolu enfeksiyonu, 1 olguda hipertermi, 1 olguda kusma ve 9 olguda hematüri tespit edildi. Opak taşlarla opak olmayanlar arasında, çoklu ve tek taş arasında, taş yerleşimi pelviste ve kaliksler arasında, aile taş hikayesi olan ve olmayan arasında, taş boyut ≤10mm ve 10mm˂ arasında istatistisel olarak anlamlı bir fark tespit edilmemiştir. Yaşı ≤3y ve 3y˂ arasında istatistisel anlamlı bir fark olmamakla birlikte 3. seans gerektirmesi açısından anlamlı bir fark tespit edilmiştir.

Sonuç: SWL çocuk böbrek taşlarında etkili, güvenli ve minimal invaziv bir tedavi yöntemidir. 3. seansı anlamlı olarak az gerektirdiği için 3 yaş ve altında da güvenle kullanılabilir.
