Acquired Undescended Testis: Ascending testis

Purpose: An ascending testis refers to an acquired undescended testis that was previously identified as descended. The aim of this study was to describe clinical features of the patients with ascending testis.

Methods:  The clinical records of the patients who had undergone surgery between January 2013 and February 2015 for primary undescended testes were analysed retrospectively. Demographic, clinical, operative findings and outcomes of the patients with ascending testes were analysed. The patients with ectopic testis, history of inguinal region surgery and neurological deficiency were excluded from the study.

Results: In this twenty six-month period, 185 patients (94 right, 69 left, 22 bilateral) were operated for primary undescended testis. In the six (4 right, 2 left) of these patients ascended testes were detected. Half of these 6 patients had a history of testicular retractility. Mean age of the patients with congenital undescended testes and ascended testes were 3.1±2.9 year (range 1 month-15 years, median 2 years) and 6.4±2.8 year (range 1,5-10 years, median 6.5 years), respectively. Ascended testis was detected in 3.2% of all undescended testis patients.

At the operation of these 6 ascending testes, testes were found at the level of external inguinal ring in 4 cases and in the inguinal canal in 2 cases. The volumes of all ascending testes were found normal. Gubernaculum of testis was normal scrotal position in 4 patients, and it was slightly superior scrotal position in 2 patients. Patent processus vaginalis was observed in 5 of these 6 patients.

Conclusion: Careful examination, good documentation, giving detailed information to parents and long-term periodic follow-up especially in the patients with retractile testis are essential to detect ascending testes. Thus, children and surgeons are protected from adverse effects of undescended testes and potential medico-legal issues.

Edinsel İnmemiş Testis: Asendan testis

Amaç: Asendan testis, daha önceki dönemlerde inmiş olduğu tespit edilen bir inmemiş testisi tanımlar. Bu çalışmanın amacı, asendan testisi olan hastaların klinik özelliklerini ortaya koymaktır.

Yöntem: Kliniğimizde Ocak 2013 ile Şubat 2015 tarihleri arasında primer inmemiş testis nedeniyle opere edilen hastaların kayıtları geriye dönük olarak incelendi. Asendan testis olan hastaların demografik, klinik, operatif bulguları ve sonuçlar analiz edildi. Ektopik testis, geçirilmiş inguinal bölge cerrahisi ve nörolojik sorunları olan hastalar çalışmadan çıkartılmıştır.

Bulgular: Kliniğimizde 26 aylık dönemde 185 hasta (94 sağ, 69 sol, 22 bilateral) primer inmemiş testis nedeniyle opere oldu. Bu hastaların altısında (4 sağ, 2 sol) asendan testis saptandı. Bu hastaların yarısında retraktil testis hikayesi vardı. Konjenital inmemiş testis ve asendan testis olan hastaların ortalama yaşları sırasıyla 3.1±2.9 yıl (1 ay-15 yıl, ortanca 2 yıl) ve 6.4±2.8 yıl (1,5-10 yıl, ortanca 6.5 yıl) idi. İnmemiş testis olan tüm hastaların %3.2’sinde asendan testis saptandı.

Asendan testisi olan bu altı hastanın operasyonlarında testislerin 4’ü eksternal inguinal ring seviyesinde, 2’si inguinal kanal içinde idi. Tüm testislerin volümleri normaldi. Gubernakulum 4 hastada normal skrotal pozisyonda iken 2 hastada hafif yüksek skrotal yerleşimliydi. Bu 6 hastanın 5’inde patent prosessus vajinalis saptandı.

Sonuç: Dikkatli muayene, iyi dokümantasyon, ailenin detaylı bilgilendirilmesi ve özellikle de retraktil testisli olgularda uzun süreli periyodik takip asendan testis tanısı için esastır. Böylece, çocuklar inmemiş testisin olumsuz etkilerinden, cerrahlar da olası medikolegal sorunlardan korunmuş olur.
